MA Podiatrist Discusses How Foot Types Relate to Pain
 
Louis J. DeCaro, DPM, President of the American College of Foot & Ankle Pediatrics, recently appeared on Mass Appeal to teach the general public how to foot type themselves. “There are 24 different foot types. For simplification, each of them can be grouped into 6 main categories. Based on your foot type, you may be prone to certain orthopedic conditions throughout your life. Understanding which foot type you are is vital for understanding and prevention of these conditions," said Dr. DeCaro.

"Common pediatric issues such as gross motor milestone delay, growing pains, Sever's disease, and shin splints have a specific origin based on one's foot type. Controlling one's biomechanics through orthotics can help solve these issues kids suffer from.” Dr. DeCaro talked about the importance of screening the foot type and biomechanics of kids at an early age. His mantra is “Prevention begets correction."

Garrett Nguyen, DPM recently joined Bradley Lamm, DPM at the Paley Foot and Ankle Deformity Center in West Palm Beach, Florida. Dr. Nguyen received his DPM degree from the Arizona College of Podiatric Medicine at Midwestern University and completed his residency at AsventHealth East in Orlando, FL.

On his podcast Fit Foot U, Ben Pearl, DPM recently interviewed Mike Donato, DPM. Dr. Donato was a former bartender in Boston and explained how this and other life experiences taught him to be able to empathize with his patients.

Donato comes from a hockey family. His brother is a coach and his nephew plays in the NHL. Donato says that hockey skates are getting more customized, like figure skates, because of the growing demand. He has put together a workshop on skate fitting and selection.

5 Coding Mistakes That Cost You Revenue
 
Accurate coding is one of the most intricate and often frustrating tasks that doctors and staff must do. Erroneous coding can bring significant financial duress to a medical practice, either in the form of decreased revenue, audits, or even clawbacks from private insurers and revenue audit contractors (RACs).
 
1. Inappropriate E/M Coding - Coding for evaluation/management services is often either too aggressive or too passive, and these coding errors are largely attributed to misinterpretation of E/M coding guidelines and the frantic pace of the clinical environment. Aggressive coding occurs when there isn’t proper documentation to prove out what was done. On the other hand, passive coding doesn’t take the entirety of the work performed into account.

Unless you are prescribing controlled substances, you do not need to pay the government the huge sum of money it is charging to renew your DEA license. This does not just apply to podiatrists. My allergist did not renew his DEA for himself and his medical staff because they do not prescribe controlled substances. Insurance companies tend to be understanding of this and will require that you provide them with the name and address of a colleague who can prescribe opioids for you in case the patient needs them. 
 
If you do opt to renew your DEA license, you need to take one of the courses on narcotics. Present Conferences is offering the course online at a reasonable cost and it comes with CME credits. Their course is listed in PM News

RE: Private Equity-Owned (PE) Practices

I have no idea how many DPMs are employed by a PE-owned practice, but I imagine it is slowly growing in popularity, as it is among other specialties. This article appeared on Medscape and is worth a read: Rising Patient Costs Tied to Private Equity Ownership.
 
For the majority who are still owned and operated privately, the challenges continue to pile up. Many podiatrists have chosen direct care over the rigors and stresses of insurance-dependency (as I did 23 years ago). But many have moved to, or are considering PE. The way I see it, if you do not have autonomy in your practice, you are likely to face contractual issues down the road. PE is not a panacea. I know quite a few MDs who went that route and are really sorry they...

George is correct, but this problem has existed for at least 4 decades. Virtual education is the new reality and saves podiatrists thousands of dollars. I loved the meetings, the interaction, and the renewal of friendships of in-person meetings. 
 
I understand the plight of the vendors. At APMA, we regularly discussed the annual meetings, the costs, the profits, and the need for corporate sponsorship. The companies always wanted greater access to the attendees. We always made every effort to increase exhibit hall attendance. 
 
The new era of virtual meetings creates a new set of challenges. I’m sure the meetings’ leadership will do whatever it can to engage in discussions.

Thank you George for shining a light on this important area. Covid was akin to a tornado that wrecked all live conventions, including medical conferences. Right now, the APMA, state societies, and other sponsors of conventions are cleaning up "the debris" by once again starting some live conventions even if they have to choose smaller hotel venues. You are correct, many doctors are still not attending. Let's be open and honest about the situation. To attend a live convention, the doctor is forced to pay travel expenses, hotel room expenses and meals, in addition to paying the tuition to the sponsor of the convention. It costs the doctor over a thousand dollars to go to an out-of-town convention. 
 
One of the things I loved when lectured at and attended live conventions was mingling with companies in the booth hall. I learned a lot about new products for my patients, and in many cases got to give them samples provided by the vendors. I assure you that most if not all of my colleagues looked forward to seeing you and talking with you and your colleagues at live conventions. The big question, however, is this: Do the benefits of attending an in-person event make up for going to a "Zoom" convention where the doctor can literally roll out of bed, flick on the computer, attend all of the lectures, and get his or her CME credits at a fraction of the price? This problem will not be resolved overnight but with innovation, maybe we can turn things around.

It is all about the money! The same goes for limiting the number of CMEs one can get online versus in person. If put to a vote in each state, I am certain that most podiatrists would eliminate the in-person requirement for CMEs. If put to a vote, I am equally certain that most board-certified podiatrists in all boards would want to be granted a recognition (tail) status.    
 
You paid your dues and met requirements your whole career! Imagine if PICA didn't offer a tail after paying your premiuns from day one for 40—50 years. PICA is not quite the same thing, but I think our colleagues get the point. In the end it is ALL ABOUT THE MONEY and nothing to do with what is best for us the practitioners.
