OBITUARIES

Charles Fuselier, DPM

Dr. Charles Fuselier, known by many the world over as Dr. Chuck the foot doctor, died Sunday at his home in Texarkana, Texas. He was 56. A podiatrist at Community Foot Clinic in Texarkana, Fuselier was not a person who lived for himself. “In our lifetime, we know only a very few people who standout like Dr. Fuselier in their passion for God, kingdom work, medical missions and care for the down and out,” said Dr. Larry Hull, president of the international Volunteer Nazarene Medical Organization.

Fuselier was also the director of Damascus Road Restoration Center, a local drug and alcohol rehabilitation center under the umbrella of Samaritan Charities. He was a teacher at the Celebrate Recovery classes at First Church of the Nazarene. Fuselier led 12 different mission teams, in conjunction with the Volunteer Nazarene Medical Organization, to villages in Ecuador, Peru, Mexico, Thailand, Africa, and the Ukraine.

CMS Won't Tap Reserve Fund To Ease Medicare Physician Pay Cuts

Physicians would miss out on the chance to see their Medicare payment cuts shaved by two percentage points next year under a Bush administration proposal that opts against using a special reserve fund to lessen the reimbursement hit.

The proposed Medicare payment rule issued July 2 by the Centers for Medicare & Medicaid Services projects that next year's across-the-board physician cut will be 9.9%, the same as the previous estimate. The rule also announced for the first time that a $1.35 billion reserve fund Congress set aside last year to address physician payment and quality of care will not go toward decreasing that percentage.

The American Medical Association and 85 other physician and health professional organizations, including the American Podiatric Medical Association (APMA) had recommended strongly to the Bush administration that it use the full reserve fund to lower the size of the cut for all participants in Medicare Part B.

Query: 2 Vs. 3 Year Residency

As a second year student thinking about my future in residency training, I was wondering if I could get some opinions about the pro's and or con's of choosing either a 2 or 3 year residency. All input would be gratefully appreciated.

While the differences between these residencies are small now, we believe that one beginning a career should look to what will affect them ten or twenty years down the road. Just as the one-residency is now a thing of the past, we believe that the prophesy of Vision 2015 is that three-year programs will be the future standard. Logic dictates that one should opt for the maximum training available. Unfortunately, because of the limited number of three-year programs, every graduate will not be able to secure such a position.

Query: Ankle Fracture Coding

How would I code the following?

- Non-union repair of fibula at the level of the ankle joint with iliac crest allograft (the non-union site involves approximately 8-10 mm.).
- Non-union repair of the medial malleolus.
- Application of external bone stimulator

Also, hardware is in place from a previous surgery. Would I add an additional code for removal of deep hardware?

Response: CPT 27720 is repair malunion of tibia without graft. There is no current code for repair malunion of fibula - one will be coming out in 2008 - so an unlisted code is indicated at this time.

The removal of the hardware is at the same site, and does not involve significant extra work. I would recommend not coding for it.

I code for the insertion of an internal elecrical stimulator, but not for application of an external one at the time of surgery.
