MA Podiatrist's Dream Now a Reality
 
Anyone involved in Boston Irish events over the last three decades knew Bill O’Connell, DPM. The Dorchester-born podiatrist, one of five children of Irish parents, and his wife Rita (Layden) were beloved volunteers in all things Irish, from the AOH Plymouth chapter, the Corkmen and Lady’s Association, the Knights and Ladies of St. Finbarr, the Eire Society of Boston, and the Charitable Irish. And they helped found the Irish Cultural Centre in Canton.

When he first learned of the famine-era Irish who had been buried in a mass grave in Boston Harbor, he began raising funds to build a Famine Memorial on Deer Island. On Saturday, May 25, 2019, Rita and Dr. Bill O’Connell’s dream became a reality.

Making a Minyan an Increasing Challenge: MO Podiatrist
 
When Herman Spector was approaching death in 2000, Jerry Cohen, DPM was one of the only people allowed to visit him at his home in University City. Spector, who worked in the printing industry, was anxious and asked Dr. Cohen, a podiatrist, “Who’s going to take care of the minyan?” Like Spector, his father-in-law had for years helped make the morning minyan, a quorum of 10 Jews required for a person to say the Mourner’s Kaddish, a prayer praising God after the death of a loved one. “I said, ‘Rest in peace. I’ll take care of the minyan,’ ” recalls Cohen, 94, who is from New Jersey and retains some of its accent.

Cohen, the podiatrist, is now among those who no longer attends the daily minyan. For years, he picked up bagels and lox from Kohn’s Kosher Deli and brought them to Shaare Zedek six mornings each week. He said he did so primarily because of the promise he made to his friend Herman Spector. But Cohen no longer drives. Rabbi Noah Arnow said that since Cohen stopped attending morning services at Kol Rinah, “We haven’t found one single person, so now we have a rotation of different people.

Questions to Ask about wRVU Compensation - Part 5
 
Physicians employed by a medical group, hospital, or academic institution are commonly compensated using a productivity formula based on work relative value units (wRVUs). Since wRVUs drive your paycheck, it’s to your advantage to ask questions in order to understand how the system works — and specifically how your employer counts wRVUs.
 
5. Who is authorized to change the codes/modifiers I submitted? Best practice is for physicians to assign the CPT codes for services performed. In most cases, after you submit your codes, a coder or biller reviews the charges and potentially changes the code(s) or modifier(s) based on documentation and payer rules. This directly impacts physician compensation, so you should take interest in who is handling your claims. Understand the protocol for making changes and insist on a system for being notified. And, ensure that you, as well as the coders and billers, stay current on coding education.

CODINGLINE CORNER
Query: CPT 17110 Denied
 
UMR requested medical notes after paying claims and then denied the claims as not medically necessary. We had used the principal diagnosis of B07.0. We billed CPT 17110, (chemocauterization/destruction of verruca plantaris). We sent detailed notes with a copy of the claim for review. UMR responded stating that the service was not medically necessary. They reversed the allowable which satisfied the deductible. UMR now wants refunds for any payments made. I appealed immediately and now they state the denial has been upheld by a board certified physician.
 
I will appeal this upheld denial. UMR is owned by United Healthcare. UHC, BCBS, Medicare, Cigna, etc. have always allowed for treatment of verruca. Opinions/recommendations?
 
Response: This denial seems a bit ridiculous. It appears that the coding is appropriate and denial ludicrous. I would ask for a peer to peer review and for a policy in WRITING that says such a service is not covered. It could be a glitch, which they love to blame these denials on, but it seems it should be a covered service. “Not medically necessary” is a commonly used phrase when they just want to make you chase payment. I would ask that a physician peer review this claim. I would also get the patient involved in that the coverage they are paying for is not being tendered to them.

RE: Congenital Malalignment of Hallux Toenail

Thanks to PM News and Dr. Allen Jacobs, I am now aware of this diagnosis and have seen multiple cases over the past year. This is likely an under-diagnosed pathology in our profession. The challenge now is treatment. I have not been able to find any podiatrists or surgeons in my area who surgically treat this condition with the nail rotation procedure described in the literature. 
 
Congenital Malalignment of Hallux Toenail
 
I'd like to learn how to perform this procedure so I can offer this to my patients. I’m looking for someone who has performed this procedure and possibly has video on technique. Any help would be appreciated.
