PODIATRISTS IN THE NEWS - PART 1

Lab Working on Shoe Sole to ID People

A new lab is working to perfect special shoe insoles that can help monitor access to high-security areas like nuclear power plants or special military bases. The concept is based on research that shows each person has unique feet and ways of walking. Sensors in the bio-soles check the pressure of feet, monitor gait, and use a microcomputer to compare the patterns to a master file for that person. If the patterns match, the bio-soles go to sleep. If they don't, a wireless alarm message can go out.

Dr. John DiMaggio

One expert who is not connected with the CMU lab said the biometric sole seems promising. "I must admit I find this news very exciting," said Dr. John DiMaggio, an Oregon podiatrist who has worked with law enforcement to use foot information in forensic investigations. While it is too early to fully judge the CMU research plan, DiMaggio said using feet as a biometric identification source makes sense.

Pedicures Can Lead to Fungal Infections: NY Podiatrist

"You should treat your feet like you do your face," says New York City-based podiatrist Suzanne Levine, DPM. "You would never think of not washing your face or brushing your teeth before bed, but does anyone really pumice their feet?"

Dr. Suzanne Levine

The things that people do to care for their feet may actually do more harm than good. "Many people think they are taking care of their feet by going for a pedicure and submerging their feet in water that no one should ever submerge their feet in," she says. The result? Fungal infections.

SUCCESS TIPS FOR THE MASTERS

Editor's Note: PM News is proud to present excerpts from Meet the Masters.

Bret Ribotsky: Now that the Obama Healthcare has made it past a major hurdle, what do you think about some of the rules that are going into effect?

Dr. Laurence Rubin

Laurence Rubin: I don’t think anybody knows that. Nancy Pelosi said first you have to pass it, and then you will see what it’s all about. This never made sense to me. I’m a big fan of decreasing the cost of medicine. This means getting us doctors to quit practicing such defensive medicine. So, I’m not sure about Obamacare decreasing the cost of medicine, but if we don’t get rid of defensive medicine, that’s what I think is raising the costs significantly directly.

Dr. Marshall Solomon

Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is podiatric surgeon and residency director Dr. Marshall Solomon. You can register for future events by clicking here

RESPONSES/COMMENTS (CLINICAL) - PART 1A

RE: Non-Compliant Patient With Diabetic Ulcer (Brent Rubin, DPM)
From: Ron Raducanu, DPM

This is certainly a frustrating and difficult situation. My advice is to refer the patient out to a local wound care center (WCC) if that is an option available to you; not because the patient is non-compliant, not healing...etc, but because of how many eyes this person will have on him. There will be many more people there to document how non-compliant this patient is, and also will have much more documentation on the patient's care by more people. This is paramount in covering your bases malpractice-wise, and also after a certain amount of time of non-compliance, the WCC will make a contract with this person to attend and follow instructions. If the podiatrist or vascular surgeon that will see your patient there are worth their salt, they will make sure to send the patient back your way when/if the patient heals.

RESPONSES/COMMENTS (CLINICAL) - PART 1B

RE: Non-Compliant Patient With Diabetic Ulcer (Brent Rubin, DPM)
From: Michael M. Rosenblatt, DPM

A non-compliant diabetic patient with an ulcer is a potential lawsuit against you. It's like a lit fuse. You are just waiting for the amputation to occur and the patient to find an attorney, or a family member or friend of the patient suggest he sue you. While I do agree with Dr. Rubin's alternatives, others are also possible.

As a single practitioner, or even the member of a group or wound care facility, you have exposure to lawsuits on a regular basis, since a percentage of your patients will be non- compliant by virtue of statistical probability. You should consider...

RESPONSES/COMMENTS (CLINICAL) - PART 2

RE: Laser for Patient With Pacemaker (Kel Sherkin, DPM)
From: Joshua Kaye, DPM

Considering that a laser is a light source (Light Amplification by the Stimulated Emission of Radiation) and a cardiac pacemaker is purely an electromagnetic device, I cannot see how a laser could possibly affect the pacemaker nor provoke a cardiac arrhythmia. If you wish to be 100% sure, check with the manufacturer of your laser.

RE: Artificial Toenail Innovation/Successes (Eric Hart, DPM)
From: Michael Forman, DPM

Regarding Dr. Hart's question about fake nails, contact Maria Barle, Nail Creations, 440-944-5882. nailcreations1@gmail.com Be aware that these are very temporary and are meant to protect the nail bed after matrixectomy.

RESPONSES/COMMENTS NON-CLINICAL) - PART 2

RE: Best Laser for Onychomycosis (Greg Teles, DPM)
From: David Zuckerman, DPM

There is no BEST laser for the treatment of fungus toenails, whether it's FDA-cleared or not, that can state and prove that it is the best laser on the market. Factors that are important in selecting a laser are:

1. How long the laser has been on the podiatric market.

2. Cost of the laser.

3. Cost for maintenance.

4. What other conditions can...
