Restrict Flip-Flop Wear for the Beach: FL Podiatrist

The American College of Foot and Ankle Surgeons estimates that about 15 percent of all adult foot injuries involve plantar fasciitis, that awful pain that comes from the tissue stretching from your heel, through the arch to the toes. Dr. Marc Katz says it's impossible to ban flip-flops, especially in Tampa's tropical environment, so he offers a compromise: "I say if you're going to the beach, wear your flip-flops.

Dr. Marc Katz

If you want to run over to Publix and you're wearing flip-flops, fine," he says. "But don't take your four-mile morning walk in flip-flops. Don't walk around Disney in flip-flops. Don't go to New York and walk around all day in flip-flops, because you're just looking for trouble."

INTERNATIONAL PODIATRISTS IN THE NEWS

Half of UK Women Willing to Suffer From Wearing Highest Heels in Europe

British women have tottered their way to the top by wearing the highest heels in Europe, with the average heel height at 3.3 inches. Some take it to even greater heights, with a quarter of British women surveyed (25%) wearing heels 4-6 inches high, and a skyscraping 3% braving heels that are more than 6 inches. In comparison, Spanish women wear an average 3.2 inch heel, Denmark 3 inches, Germany 2.7 inches and France 2.4 inches. 1 in 2 women (50%) are prepared to endure a level of pain they ranked as comparable with tooth ache and ear ache.

Lorraine Jones

Lorraine Jones, member of the Society of Chiropodists and Podiatrists says "As a podiatrist, I recognize that with fashion as it is, wearing foot-friendly shoes is not always easy. However, wearing fashionable shoes occasionally is fine, and there are plenty of easy foot care treatments available at the local pharmacy that can help treat feet at home and avoid a trip to the podiatrist's clinic. The bonus is that with healthy feet, a woman can stand as tall as she wants in the highest of heels."

Unknown Roadways Present Risks for Barefoot Runners: IL Podiatrist

Dr. Beth Jarrett, podiatrist and professor at Rosalind Franklin University of Medicine and Science, says that unknown roadways for barefoot runners present the real risk. “Probably the bigger issue in terms of injuries with barefoot running is the fact that you’re barefoot, and more likely to step on stones or glass,” she said.

Dr. Beth Jarrett

Dr. Jarrett says she would not generally recommend barefoot running, unless someone had access to a safe running environment to minimize the risks of injuries from unknown surfaces. Dr. Jarrett says, if people are not already runners, they shouldn’t try barefoot running out of the blue. “It’s important to remember that just as not everyone is meant to be a runner, not everyone is meant to be a barefoot runner,” she said.

VA Podiatrist Retires After 31 Years

Throughout July’s final weeks, Dr. Rodney Kander, 59, and Iris Kander, 56, will bid farewell to their 31-year-old business, Podiatric Care of Northern Virginia, before he retires Aug. 1. With his retirement, the Kanders will also leave behind his patients, who obviously care for the couple.

Dr. & Mrs. Rodney Kander

“People are so friendly, so nice,” Rodney Kander said. “It is so heartbreaking to say goodbye. But I’m not saying goodbye to anyone staying in the area—I will see them in civilian clothing, and they won’t have to take their shoes and socks off. They will just say hi.” Slipping into Rodney Kander’s shoes will be Dr. William Knudson, who will take over the practice. He has been working at Podiatric Care of Northern Virginia for the past eight months to make the transition smoother. “It has worked out really, really well,” Rodney Kander said of the switch over. “He is going to do very well. The patients have adapted to him.”

MO Podiatrist Indicted for Medicare Fraud

A local podiatrist, Dr. John M. Dailey, has been indicted for falsely billing Medicare and using a false Social Security Number on credit card applications, federal prosecutors said Friday. Dailey, 54, who had an office in Washington, MO billed for services that he claimed that he provided when he was actually outside the state, including on vacations to Las Vegas and Hawaii, prosecutors claim. He was indicted last week on three felony counts of making false statement to Medicare and two felony counts of using a false social security number.

QUERIES (CLINICAL)

Query: Non-Compliant Patient With Diabetic Ulcer

I have a  48 year old Type 2 uncontrolled IDDM male who came in to the wound care center with a large ulcer, 3 x 5 cm on the lateral plantar foot. He is bipolar and totally non-compliant. He has been walking on the foot for six month in a regular shoe and applying dry dressings. The ulcer is clean and has no active cellulitis. He did have a previous partial fourth and fifth ray resection so the ulcer is under the remaining stumps of the metatarsal heads. MRI reveals osteomyelitls of the end of the fourth metatarsal stump and osteomyelitis of the whole remaining fifth metatarsal.

Diabetic Ulcer in Non-Compliant Patient

Options are several: 1. refer patient out because of total non-compliance, 2. refer to vascular for BK amputation 3. surgically debride the osteomyelitis and VAC, etc. with either second procedure of peroneus brevis transfer now, or a staged transfer down the road if the ulcer heals. Any suggestions appreciated.

QUERIES (NON-CLINICAL)

Query: Protocol for Employee Cut by Used Blade

What protocol do you use for an employee who nicks her/himself with a used blade? Is the protocol different for a patient? I could see how it could be because only sterile blades are used on patients, and if an assistant nicks herself with a used blade, a different situation exists.

QUERIES (CLINICAL)

Query: Laser for Patient With Pacemaker

If you are using laser treatment for fungal nails, I would appreciate your advice on the following patient: An 82 year old white female whose only appreciable medical history is that she had a pacemaker in 2000. Her neurovascular status is WNL. The remainder of past medical history is unremarkable. She is not taking any meds. Is this patient a candidate for laser treatment for fungal nails?

RESPONSES/COMMENTS NON-CLINICAL) - PART 1

RE: Artificial Toenail Innovation/Successes (Eric Hart, DPM)
From: Janet McCormick, MS

Artificial toenails have been being applied for 30 years, actually. However, they were a hard acrylic, not a gel product, so the nail had to be very short to prevent damage if the client stubbed her toe or such. Gels flex, so are safer on a nail that is compromised.

Your patients will love them! Many of them would love to wear some sandals, but they will not due to embarrassment about the appearance of their toenails. Get them to where they can wear sandals instead of tennis shoes at the beach, and you will be their savior!

This service has a lengthy learning curve, but anyone can learn to...

Editor's note: Ms. McCormick's extended-length letter can be read here.

RESPONSES/COMMENTS NON-CLINICAL) - PART 2

RE: Best Laser for Onychomycosis (Greg Teles, DPM)
From: Jodi Schoenhaus, DPM

The research and development for the use of lasers has made significant strides in the past few years. Mechanism of action, treatment guidelines, and outcomes are being tested and determined. I agree that laser treatment for onychomycosis may be limiting in various cases and does not offer a permanent solution. It is our responsibility to patients to inform them of this prior to treatment. In severe cases, I tell patients that they will have an improvement in the appearance of the nail, but the nail will not look perfect, and most patients will require a maintenance program. When choosing a laser, do your homework. Not all lasers are created equal. Choose a reliable company.
