NY Podiatrist Discusses Supportive Shoes and Orthotics
 
According to Samantha Landau, DPM, a foot and ankle specialist and assistant professor at the Touro University New York College of Podiatric Medicine, arch-supportive shoes should have firm heel counters and good side-to-side control, all of which help limit excess motion. She also recommends opting for shoes designed with forefoot rockers, as most people can benefit from them. “A forefoot rocker is beneficial as this allows and helps facilitate forward motion,” Dr. Landau said.

“If the shoe helps bend at the forefoot, then less stress will be placed on the foot to bend. This is especially beneficial in those with forefoot arthritis or forefoot injuries or pain.” According to Landau, the majority of people don’t have perfect feet and shoes alone are typically not enough to provide all the necessary support. For that reason, she finds pre-fabricated (off-the-shelf) or custom foot orthotic devices are often needed to “further neutralize the deforming forces, reducing the potential to cause injuries during gait.”

UK Podiatrist Warns Against Failing to Moisturize Your Feet During Warmer Months
 
London-based podiatrist Maddie Tait warns against failing to moisturize your feet—especially during warmer months. "During the summer being on the beaches or on the sand or in the sun, our feet need a bit more moisturizer," she said.

"We can develop cracked heals or corns or callus, so make sure you moisturize your feet throughout the year, but definitely in the summer," says Tait.

Dotinurad More Effective Than Febuxostat in Gout - Phase 3 Trial
 
A multicenter phase 3 trial compared dotinurad 4 mg/d with febuxostat 40 mg/d in 441 Chinese patients with gout. Dotinurad achieved a higher responder rate at week 24, with 73.6% reaching target serum urate levels (≤ 6.0 mg/dL) compared to 38.1% for febuxostat. At week 12, dotinurad 2 mg/d was noninferior to febuxostat 40 mg/d in terms of the responder rate (55.5% vs 50.5%). 
 
The mean percent reduction in serum urate levels at week 24 was greater with dotinurad. Both treatments had similar safety profiles, with low incidences of severe adverse events. These findings emphasize the potential of dotinurad as an effective urate-lowering therapy for gout patients. The study was funded by Eisai Co., Ltd.
 
The Takeaway: Dotinurad offers a new urate-lowering option for gout patients, with good safety.

How much of a patient's foot must be missing before they qualify for a Q7 modifier? Is missing one or more metatarsals a Q7? If they've had a transmetatarsal amputation, do they qualify for a Q7? If I'm reading the modifier correctly, it seems to imply that the entire foot must be missing to qualify for a Q7

Response from thedoctorline.com: Q7 is defined as "Class A findings are specifically defined as a non-traumatic amputation of the foot or a part of the foot's skeletal structure." So, as you can see, it can be any part of a foot, not just the whole foot.
