CA Podiatrist Recommends Oil-Based Ointments for Dry Skin
Sandal season is upon us, which means we're keeping our nails trimmed, trying our best to avoid chipped polish, and making sure our feet are soft and moisturized. But that lightweight summer lotion you use on your body may not cut it for your heels and soles where skin tends to be thicker, which is where foot ointments come into play.

“In summer, when we are barefoot or using things like open-heeled shoes, our skin can become very dry and cracked,” says Diane Koshimune, DPM, a podiatrist in San Jose. To get even the dryest of feet moisturized, she recommends pulling out the big guns—oil-based ointments. She says the best time to apply them to your feet is right after a soak. But if they are very dry and cracked, they may need to have some filing of the thickened, callused skin first. Do this in a bath or dedicated foot soak, using a pumice stone, for example, followed by a moisturizer, she says.

Billing Agreements: Take These Steps to Avoid Painful, Expensive Mistakes
Many physician groups use an outside billing service to assist in collecting fees for professional services. However, it is not unusual for physicians to become unhappy with their billing company for a variety of reasons. Physician practices should make sure to seek legal help when negotiating a billing agreement in order to be sure the terms are as fair as possible. Some possible issues to consider are the following:
4. Focus on the compliance aspect of the billing agreement. What ongoing education will the billing/coding team receive? What internal education and audit processes do they have in place? Will the billing team update the practice should applicable rules regarding documentation and coding change? How often will an audit of the client’s billing and documentation be performed to help assure accuracy and compliance? Who is responsible should there be an audit or recoupment related to the practice’s billing and is this liability clearly and fairly spelled out? These are important issues to outline in any billing agreement.

I would like to share with you, however, that my lived experience and that of others like me, are vastly different than yours.
I have been in practice for 18 years. I went to DMU, in Iowa. The first 2 years of basic science were the exact same as that of our DO colleagues. In fact, we were not separated except when they did OMM (skeletal manipulation), psych, and OB (3 classes). WE took the same tests in every subject, the same finals, and there were podiatrists who did take the DO board exams and...

I couldn’t agree more with Dr. Jacobs’ full response to this issue. I struggled with my professional identity in podiatry school until I received advice from a role model I admired during one of my externships in 1976. Dr. Joseph Seiler achieved full podiatric surgical privileges at the local hospitals where he practiced at a time when that was the exception rather than the rule. His advice to me was, “you don’t succeed by trying to be something you’re not.” I followed that advice during my career at an academic health science center, and most would say I did okay.

I read today comments by two of our most esteemed podiatrists Drs. Allen Jacobs and Bryan Markinson on podiatry's scope of practice. I'm certain they are very honest in their comments but their remarks are discouraging. These are two very bright and wonderful people. When you leave their lectures you always come away proud that you share the same cherished profession as them. They are smart. Allen can impress the best of us with his medical and surgical knowledge and insight. As a student at PCPM (TUSPM) in the '70s, I listened intently one day to a lecture he prepared on metal materials for surgical fixation procedures. When I returned home that evening, I didn't feel as smart as I did that morning and frankly if I needed a little more humility that day, he provided it. In my podiatry class at that time was another bright podiatrist to be, Charlie Arena.
Later, Charlie and I would be residents at Northlake Hospital, but before that time while we were doing externships, our paths crossed at a hospital in New Jersey. Charlie was on medicine that month and I was on the orthopedic service. By chance, I was walking in a hall outside a room where the externs on the medical service were meeting and the attending was belittling some of the medical students as he would ask them questions and when they couldn't answer correctly, he would then call on Charlie to answer. Charlie...

I always enjoy PM News and especially the weekly polls, but I thought this week’s question “what is the best way to achieve parity with allopathic physicians?” lacked the necessary clarity to produce a meaningful poll response.
I hope that the response “legislative action” includes changing our restricted DPM licenses in all 50 states to an unlimited, plenary DPM license. This pathway to parity will enable us to retain our unique perspective as podiatric physician (much like our DO colleagues) while creating true parity with MD and DO physicians at the appropriate level - state licensure. This approach is also the only one that has actually been worked on in a collaborative way with our physician colleagues via the California Task Force and, more recently, the National Task Force.
As has been mentioned several times in this forum, I also take exception to the term “allopathic physicians” to describe our MD and DO colleagues. I do appreciate the efforts of Dr. Block and PM News to keep this important issue at the forefront of discussion.

Editor's comment: Those who have followed this 40-year debate understand that legislative change means enacting laws that increase the scope of practice. Degree change means an MD or DO degree granted by an educational institution.

I have read most of the posts regarding this topic and have come to a conclusion. Recommending that anyone go into podiatry and/or medicine these days basically boils down to economics. Just take a look at your EOBs for each procedure you do and calculate your overhead. If at the end of the day, you are happy with those numbers compared with what it has taken you to get where you are, including your time and money, then by all means recommend podiatry/medicine as a career.
All of the topics that have been brought up are issues that our profession has been fighting with for decades. Some of the battles we have won, but we are still losing the war. Podiatry is a great profession, but would anyone recommend any career to someone where there are so many battles being fought and one's pay scale is continuously dropping while expenses continue to rise?
