For lovers of history or shoes, Temple University has a collection that will knock your socks off, a hidden gem in the middle of Center City. Temple University's School of Podiatry created the collection 40 years ago for the Bicentennial Celebration, and it is a visual ode to footwear with shoes housed in cases and cases that line the school's corridors on 8th Street.

"That's the highest high heel that we have," says curator Barbara Williams as she shows the collection to a group of visitors. She is both tour guide and resident shoe expert. "These I love," she says as she shows off famous shoes from ballets, Broadway and television, "these are Lucy's."

NY Podiatrist Discusses Ingrown Nails
 
The best way to deal with an ingrown toenail is to keep one from forming in the first place. Unlike fingernails, which you can shape however you want to (be they square, oval, “squoval,” stiletto, whatever), toenails should have one shape and one shape only: straight across. “Rounding the edges, or the ‘corners’ of your nails, or cutting your nail too short can lead to ingrown toenails,” says New York-based podiatrist Dr. Suzanne Levine.

And if you’re squeezing your feet into too-small shoes, you’re basically asking for ingrown nails. Keep an eye on your big toes, which is where they most often appear. If your ingrown nail begins to look swollen, red, or generally inflamed, it’s time to call in a podiatrist, Levine says, because at that point, you might need surgery.

GPs and DPMs Both Play Important Roles in Foot Care:  NV Podiatrist
 
It may not always be clear if it’s better to see a foot doctor or a general practitioner for a foot concern, but both can be important when it comes to providing care, according to Timothy M. Mooney, doctor of podiatric medicine, who is board certified in podiatric surgery and a podiatrist with Northern Nevada Medical Group.

If  patients have foot issues and their insurance allows them to make an appointment without a referral, they can go directly to a podiatrist, he said. However, if a patient’s health insurance requires a referral, or if patients have several health issues to discuss, or if they are not quite sure if their issue is foot-related, they may want to start with a general practitioner, said Dr. Mooney. “Both play an important role,” he said.

CMS NEWS
CMS Says $42 Billion Saved with Fraud Prevention, Provider Screening Efforts
 
The CMS saved about $42 billion in fiscal years 2013 and 2014 through program integrity efforts, according to a report to Congress released Wednesday. The CMS attributed the savings to increased provider enrollment and screening standards as well as fraud prevention efforts established by the Affordable Care Act.
 
Fraud prevention efforts accounted for about 68% of the savings in fiscal 2013 and nearly 74% in fiscal 2014. A July 2015 report found that fraud prevention had identified or prevented $820 million in inappropriate payments since it was implemented in June 2011.

If your practice chooses to participate in an Alternative Payment Model (APM) rather than MIPS, here are two key points to consider:
 
1. Transitioning to a new payment entity
 
In addition to MIPS, MACRA has created new opportunities for physicians to develop and participate in alternative payment models, or APMs.
 
If you participate in an Accountable Care Organization (ACO), Clinically Integrated Network (CIN), or other entity that applies and qualifies as an APM, contact your entity's administrators to ascertain what plans are being made to meet the new model. Even though quality measures and criteria are still being defined by CMS, most organizations are already planning a clear direction.

Query: Surgery Denial Question
 
Our doctor saw a patient a day BEFORE his surgery for routine foot care (billed CPT 11056, CPT 11721). We billed the surgery CPT 28293-79-RT and CPT 28293-79-59-LT. We are getting a CO-151 (payment adjusted because the payer deems the information submitted does not support this many/frequency of service denials.

Response: The performance of routine foot care the day before surgery should not impact the surgical coding. The problem, as I see it, is your coding. I  suggest CPT 28293-50 at 1 unit.  
 
CPT 28293 is designated bilateral coding by Medicare, which does not allow for separate line billing of bunionectomy codes. Also, you do need need the "-79" modifier, unless you are in a separate global period for a surgery previously done.

The podiatrist who posted this query seems to have laid out a very good description of low grade chronic implant rejection. I advise an ultrasound-guided needle biopsy of the granulation tissue for histologic analysis. This may give insight as to whether or not implant rejection is in play here.
