Samuel Adegboyega, DPM (2006), Associate Professor and Chair of the Department of Medical Sciences at the New York College of Podiatric Medicine (NYCPM), has announced that Svetlana Lopata, DPM (2018)  joined the department on July 5.

Dr. Lopata completed her residency at St. Joseph’s Medical Center in Yonkers, NY, where she served as chief resident and completed a fellowship in advanced wound care and tissue healing at St. John’s Episcopal Hospital in Far Rockaway, NY. She is board certified by the ABPM and board qualified in foot and reconstructive ankle surgery by the ABFAS.

NY Podiatrist Discusses Causes of Dry Feet
 
It’s important to note that there is a wide range of causes for dry feet. Sometimes, lifestyle habits are relevant, but other times what’s going on inside the body is the primary cause. A few examples of the latter, according to Brooklyn-based podiatrist Greg E Cohen, DPM, include dehydration, a lack of sweat gland production, diabetes, and aging.

There are lifestyle factors that can lead to dry feet, regardless of what’s going on internally, too. Here, a few to note: Extra hot showers, low humidity, soap that contains harsh cleansing agents, and walking barefoot.

Billing Agreements: Take These Steps to Avoid Painful, Expensive Mistakes
 
Many physician groups use an outside billing service to assist in collecting fees for professional services. However, it is not unusual for physicians to become unhappy with their billing company for a variety of reasons. Physician practices should make sure to seek legal help when negotiating a billing agreement in order to be sure the terms are as fair as possible. Some possible issues to consider are the following:
 
3. It is important to understand the termination and/or remedy process in the agreement if the practice believes that the billing company is not performing as expected. Most billing agreements can be difficult to terminate because they provide limited breach terms or allow the billing company to “cure” breaches repeatedly so the contract can never be easily terminated. A billing agreement should provide a specific list of reasons a practice can terminate the contract and should limit the opportunities provided by the billing company to cure an alleged breach. Additionally, both parties should have a right to terminate the agreement without cause, as long as there is sufficient notice and a clear transition process included in the agreement.

Query: A Pedorthist and DME For Medicare Patients
 
If a private practice podiatrist hires a certified pedorthist to work in their office, is the pedorthist able to fit, mold, and dispense DME braces for patients with Medicare and other insurances without the doctor being present during this time period?

Response: This question is one which the National Supplier Clearinghouse (NSC) can answer and cannot be answered here as simply yes or no. I will do my best to answer the question with the limited information provided. Important questions to consider would include:
 
1) Is the pedorthist going to be billing as the provider of service as an independent contractor or is the podiatrist billing for the services?
2) If the podiatrist NPI facility is the billing entity, is the pedorthist a W2 employee of the podiatrist?
 
There are also innumerable NSC regulations with respect to licensure, facility accreditation, surety bonding, and location sharing for the pedorthist which may make having the pedorthist working as an independent contractor to the podiatrist impractical. Furthermore, if the pedorthist is in a licensure-required state, the pedorthist must be licensed by the state board for pedorthists. This may be run by either the podiatry board (e.g. Tennessee) or another medical board. My suggestion is to run the exact scenario by an expert in DME enrollment and/or by contacting the NSC.

This is an interesting study, but not necessarily a "green light" for cortisone injection in the treatment of Achilles tendinitis. It still should be used only after conservative treatment, when heel lifts, stretching, etc. are not effective. If cortisone injection is used, it should be applied judiciously, not repeated on a regular basis for a long period of time, and short-acting steroids are preferred. Additionally, the degree of tendon disruption should be considered prior to the use of cortisone therapy.
 
Finally, what is the lifestyle and the degree of stress the individual is expected to apply to the Achilles tendon apparatus post-injection therapy? The two extremes would be a ballet dancer and a librarian. The long-term potential negative impact on the structure intended is of much more concern with a ballet dancer and others who are extremely...

The USMLE or "equivalent" exam taken by podiatrists as discussed by Dr. Sherman will never materialize and if it does, who would take it and accept the limited scope of podiatry while others would have their choice of every medical specialty? Not enough to fill any podiatry school. 
 
The AMA and NBME say simply, just go to medical school. The quoting of Frederick Douglas' reference to "injustice and wrong" as a parallel to the AMA or NBME stance on podiatrists is a bit of a stretch and may be offensive, though I am  certain that none was intended.

Dr. Sherman‘s quote of Frederick Douglass is hardly applicable to the “parity” discussion regarding podiatry and the MD and DO degree. Simply stated, it is disingenuous to suggest that the medical education of a podiatry practitioner is an any way equivalent to that of an MD or DO. It is not. Dr. Sherman knows this, PM News readers know this. A podiatrist is trained to be a limited licensed practitioner. That is reality.
 
This is not a matter of political power as Dr. Sherman suggests. This is a matter of public safety. It is part of a continuing illusion perpetuated by some in our profession. The training of a physician is more generalized and vigorous. Unlike Dr. Sherman, I have been continuously involved in podiatric education, and practice. I treat patients daily. I interact at the clinical level with...

I agree with Dr. Neuhoff regarding residency length. I’ve been practicing for 27 years and the scope did not change when the length of residency changed. I did a one-year residency where frankly my Co-Directors were not interested in bringing cases for us to perform. I did gain a lot of routine “primary care podiatry” skills. The course of study is prohibitive. My sister became a P.A. after I started practicing, and made more than I did in far less time.

In response to Dr. Neuhoff's recent post as it pertains to my CHOSEN profession, let me say that I respectfully disagree with her supposition as well as her suggestion to reinstate the one year residencies of previous years. For those of us who worked tirelessly in order to "further" our profession at both the State and National level as well as at the local level, her suggestions are completely "off base."  
 
The hundreds of man/woman-hours performed by those before me and after me in order to elevate our profession to what it is today, is a testament to what can be achieved by demonstrating the abilities that we have garnered through intensive study and training. This includes as it pertains to the local hospital hierarchy, the local physicians, and yes, the public at large. To even suggest that individuals are choosing to obtain a PA licensure as opposed to applying to our profession, or to even enter either the fields of podiatry and dentistry because it was a "lower investment of time and money," is both erroneous and laughable.
