Derner Named VA Podiatrist of the Year

Dr. Richard Derner of Stafford Foot & Ankle Center has been selected Podiatrist of the Year by the Virginia Podiatry Medical Association. Derner graduated from the Ohio College of Podiatric Medicine in 1987 and completed a 3-year surgical residency at Highlands Center Hospital/Doctors Hospital in Denver, Colorado.

The California School of Podiatric Medicine (CSPM) Alumni and Associates have announced the 2009 recipients of the prestigious Heather Barton, JD Award; Gary Bianchi Award; and Philip Gardner, DPM Awards. These recipients were selected by a committee from nominations submitted by the CSPM podiatric medical community.

The Heather Barton, JD Award is awarded for outstanding service and educational contributions to podiatric medicine, the California School of Podiatric Medicine and the podiatric medical profession.  For 2009, the committee selected Dr. Kevin A. Kirby as the Heather Barton, JD Awardee.

RE: Lipoma or Liposarcoma (Richard Silverstein, DPM)

If the biopsy comes back as lipoma, take it out. If it doesn't, do one of two things: 1) If you are comfortable with this type of procedure and you know what to do, how to remove it, how deep to go, what the margins should be, how to tag it, how to transport it, who to NOW refer this patient to, and how to manage this patient post-operatively, then go right ahead and remove it. 

2) If not, then right after you receive the results from the frozen section, immediately close your incision (leaving the rest of the lesion behind), and refer your patient to a surgical oncologist. This could be the difference between a local procedure/excision and a BKA (or worse).

Some patients do not like or cannot tolerate NSAIDs.  Besides branched-chain amino acids, you can also try a good calcium, magnesium, Vitamin D supplement (Shaklee products, Citracal). Avoid generic OTC Cal-Mag products; most are not absorbed well and have little, if any, effect.

If you feel adventurous, you can make your own mixture of calcium gluconate (1 tablespoon), magnesium carbonate (1 teaspoon) and of apple cider vinegar (1 tablespoon). Mix well, then add 4 to 6 oz. of boiling water. Mix well, cool, and drink. This will reduce or eliminate your muscle and joint stiffness and pain and relax you for a good night’s sleep. Give it a try.

RE: Patient Privacy and Lack of Enforcement

I had my own issue with privacy rules last year in my office. My former office manager had the tendency to look in the medical records of her friends and family, and later on into the records of her co-workers and their spouses. I took the issue to the EMR people at the hospital and had them bring up all patient names that she had looked up under her ID. Lo and behold, a significant number of the people's charts she had looked into were not patients belonging to our practice but friends, family, co-workers, etc. I asked another employee whose name showed up if she had looked up her own record and she said that she had not, but knew who probably did it. I then went back to the EMR people to ask their recommendations about handling it. I was surprised, but they told me that they would leave the handling up to me. I was surprised that was the extent of it.

I fired the office manager after that, who I thought was a valuable employee, but soon after she was gone, we found a variety of financial misdeeds. I brought charges against her (not HIPAA-related), but nothing ever happened to her as far as the HIPAA violations were concerned, even though our system EMR people knew and the police knew.

So I ask; where are the teeth in this HIPAA rule? I thought this to be a very blatant infraction, but it was not pursued. Fortunately, the employees who were involved did not try to sue me (she did it on my time on my computer). I found out afterwards that this had been going on for a while and they all knew she did this.

RE: On-Line Back-up (Mark Stempler, DPM)

Dr. Stempler said he uses an external hard drive for back-up. They are much better than tape, but don't forget you need to store some backups off-site to protect against fire and theft, just as you used to do with tape. And don't leave it out where someone can just walk off with it.
 
I use a combination of all of these ideas. I back up the server to all of the computers on my network  (ALL data is always stored on the server; nothing is ever stored on a local workstation, so the workstations never need to be backed up. if one dies, we just buy a new one, install the software and the data is already available on the server), and also use Mozy online backup. Just be careful with open files. I use MS SQL server for my database. It has a function to back up the data. I back up the database twice a day, and it is these back-up files which then get backed up to the other computers and Mozy - not the live database files because they can't be restored  reliably.
 
Every once in a while, try to restore everything to your home computer to make sure the process works. Look at your data to make sure recent transactions appear, and recent charts were saved.  I have the system e-mail me a success/failure report every day. We now rely on our computers a little too much, so we have to be extra careful not to lose anything.
