NY Podiatrist Discusses the Benefits of Anti-Fatigue Mats 
 
Long days on your feet can have serious effects on your joint health over time, which is why these mats have many benefits for everyday life. New York City podiatrist Emily Splichal, DPM, shares, "Anti-fatigue or standing mats are designed to off-load stress to the foot and body associated with prolonged standing." She adds, "Some mats provide cushion, whereas others provide irregular surfaces or texture to stimulate and massage the feet."

Depending on the experience you're searching for, whether it be cushion or support (or a combination of both), Dr. Splichal recommends looking for a mat that can "activate, release and off-load the foot during prolonged standing." She goes on to explain that the ball of the foot and heels can often feel the most stress from prolonged standing, as this is where the most pressure is placed.

RELEVANT RESEARCH
Ultrasonography-Guided Corticosteroid Injection vs. Placebo Added to Exercise Therapy for Achilles Tendinopathy
 
Question: Compared with placebo injections, do ultrasonography-guided corticosteroid injections improve outcomes in adults with Achilles tendinopathy managed with exercise therapy? Findings:  In this randomized clinical trial of 100 patients, statistically and clinically relevant improvement was found in the group receiving exercise therapy combined with corticosteroid injections.
 
Conclusions and Relevance: Corticosteroid injections combined with exercise therapy were associated with better outcomes in the treatment of Achilles tendinopathy compared with placebo injections and exercise therapy. A combination of exercise therapy and corticosteroid injection should be considered in the management of long-standing Achilles tendinopathy.

Billing Agreements: Take These Steps to Avoid Painful, Expensive Mistakes
 
Many physician groups use an outside billing service to assist in collecting fees for professional services. However, it is not unusual for physicians to become unhappy with their billing company for a variety of reasons. Physician practices should make sure to seek legal help when negotiating a billing agreement in order to be sure the terms are as fair as possible. Some possible issues to consider are the following:
 
1. The agreement should spell out the exact services to be provided by the billing company, the timeframe in which the services are to be provided, and any items or services which are not included. For example, what reports will be provided each month by the billing company? How quickly will a clean claim be filed? What collection efforts will be made?

I read this JAMA Open Access article titled, "Effect of Ultrasonography-Guided Corticosteroid Injection vs. Placebo Added to Exercise Therapy for Achilles Tendinopathy: A Randomized Clinical Trial" with interest as it seemingly went against the previous notion of injecting steroid near the watershed area of the Achilles' tendon. Not only did they inject steroid, but they opted for an acetate as opposed to a phosphate. Any comments for discussion will be appreciated.

More food for thought regarding the DPM vs. MD debate. In this article, AMA is bragging about their efforts to stop what they call “scope creep”, which includes a bill expanding podiatrist scope of practice in Alabama and a Maryland bill that would allow podiatrists to use the term podiatric physician. Somehow, I doubt podiatry students taking USMLE exams will do much to change their minds.

I would like to give Dr. Werter my opinion concerning performing P&As on infected toenails. Like you, I was told in school to perform a nail avulsion in the presence of a paronychia. Somewhere along the way, I do not remember who or when but a long time ago, a fellow podiatrist told me that he did P&As on all of his patients' infected toes with NO sequelae.
 
I have been performing P&As on virtually all of my patients with a paronychia. I tried this and low and behold they all get better. Sometimes, in VERY severe cases, I will Rx an antibiotic when I do the procedure. Knock on wood, I have NEVER had a problem. I think the mere nature of decompressing the wound and taking out the offending nail is the answer. Also, the phenol kills everything, the root matrix as well as all bacteria it comes in contact with. I believe from a patient prospective, it provides great results and patients do not need a second surgery. Caveat: I still do nail avulsions when indicated, meaning for minor problems or if it is not a chronic problem.
 
Lastly as a private, NO insurance provider, I always let the patient make the final decision once they are informed of both procedures and the costs involved. I currently charge $1,000 for the first side and $500 for an additional side on the same toe which includes 3 post-operative visits.

I am a veterinarian who is also a podiatrist. I can tell you that nearly all students who get into vet school possess the credentials for admission to any medical school. They choose veterinary medicine because they passionately believe that is the profession they want to pursue. Veterinary medicine is struggling with a high suicide and burnout rate at this time, but I do not think there is a large pool of potential candidates for podiatry.
 
When I applied to podiatry school, all my pre-vet courses fulfilled the pre-med criteria. I was allowed to take proficiency exams for most of my first two-year courses, so I was able to complete my podiatry training in slightly more than 2 years plus a one-year residency. However, with the drive to obtain "parity", we have increased the length of time needed to obtain a DPM degree so that it is nearly the same as that needed to obtain an MD degree. Even with the kind of accommodation I received, it would be an additional 6 years for a veterinarian to become a practicing DPM. 
 
I think many of the persons who chose podiatry did so because, like dentistry, it allowed them to be a "doctor" with a lower investment of time and money. I believe this pool of candidates are now choosing to become PAs rather than podiatrists. Of course, we are still attractive to those who wish to become "foot and ankle surgeons", but I am not sure there are enough of these to support our profession. Perhaps we need to reinstate one-year residencies for those who have no desire to be MDs but would like to serve patients as podiatrists have for decades.
