| SUCCESS TIPS FROM THE MASTERS | |
Bret Ribotsky: The nail plate has been a barrier to get the antifungal medication to where the fungus is. Telll us about a new technology to overcome this treatment obstacle?
|
|
Dr. Rolf Thomas |
Rolf Thomas, PhD: As a biomedical engineer, I developed a drill that senses when the drill tip penetrates the nail plate and stops. This will allow a podiatrist to drill small holes, 0.4 mm in diameter. It's through these portals that physicians can use medication to treat the fungal disease.

| MACRA UPDATE | |
Holy MACRA! Half of Docs Have Never Heard of Medicare Payment Reform
Half of non-pediatric physicians have never heard of the Medicare Access and CHIP Reauthorization Act of 2015—a new CMS payment plan that will put 4% or more of their Medicare reimbursement at risk beginning in 2019, according to a new survey by Deloitte. With CMS preparing final rules this autumn, just 21% of self-employed or small-group physicians and 9% of physicians employed by hospitals or larger groups were even somewhat familiar with the pending reimbursement changes, the survey showed.
Physicians with a high share of Medicare payments were just as clueless about MACRA as those with lesser exposures, Deloitte found. The consultancy surveyed 600 physicians through its Deloitte Center for Health Solutions. Physicians and other clinicians have been caught flat-footed by the “transformational” changes portended by MACRA because for 20 years they have heard about major impending Medicare reforms that never came to fruition, said Anne Phelps, Deloitte principal and U.S. healthcare regulatory leader.

TSRI Study Finds Gene Crucial to Keeping Tendons Healthy
A new study by The Scripps Research Institute (TSRI) reveals a gene crucial in keeping tendons healthy. Scientists at TSRI are working to investige how tendons develop and stay healthy in the first place. “If we understand the molecular mechanisms of tendon development, we can apply the findings to develop a new regenerative therapy for tendon diseases and injuries,” said Hiroshi Asahara, a professor of molecular and experimental medicine at TSRI with joint appointments at Tokyo Medical and Dental University and the Japan Science and Technology Agency.
Now a new study from Asahara and his colleagues reveals the role of a gene called Mkx in maintaining and strengthening tendons in animal models. This gene also appears to prevent a debilitating tendon condition called “ossification.” The findings, published as the cover article in the July 12 edition of the journal Proceedings of the National Academy of Sciences (PNAS), may bring doctors closer to one day using gene therapies to grow and repair tendons.

MACRA, MIPS, and APMs — Are You Ready? - Part 4
2C. The Advancing Care Information (ACI) component will change several things about Meaningful Use measures:
• Simplifies reporting requirements for "Health Information Exchange" and "Public Health and Clinical Data Registry" reporting measures to a simple yes/no attestation.
• Eliminates the 90-day reporting period for first-time attestations, instead requiring a full calendar year reporting period for all stages and all providers.

Query: Nursing Home Nail Care
As I understand it, a nursing home patient or the family may request foot care without an order from the primary physician. Does the patient or family have to request foot care in writing every time?
Jennifer, Office of Charles Perry, DPM, Cambridge, OH
Response: That is correct, although most providers forget this. Note, though, that is true for a nursing home patient, but not a skilled nursing bed (SNF) patient (Medicare Part A stay).
An SNF patient is like an inpatient hospital patient - they cannot request their own care. And, no, to question 2, as in a regularly ordered patient, once you see a patient, you have a doctor-patient relationship, and may see the patient as you see fit for medical necessity. There has never been any rule that each separate visit must be ordered by an MD/DO.

The picture submitted showing a "chronic onychia" is the classical appearance of a monilial paronychia. Topical Nystatin cream should cure the problem.

RE: Walking Well Again, By Stuart Goldman, DPM
From: Richard Willner, DPM
Very few books are "game changers", that is, they change your thinking about your day to day practice life. Stu Goldman just published an 'overnight" success story, one that was some 25 years in development. Stu lays it out there. He says that if his technique does not initially work, the odds are that it won't work. But, if it works, you have done a wonderful thing, and you can take full credit for it. Share your success with your neurology friends, or the orthopedists, and they will be scratching their heads wondering how you got great results.
Look at the incredible testimonials on the cover and the inside. Those testimonials were earned by the keen original observations from Stu Goldman. This is a polished book, especially for the first edition. I suspect that the physicians who wrote the testimonials for this book also improved and expanded this book so it is such an outstanding read. I have ranked this Goldman book as a 10 on the 5 scale! It's that outstanding! I urge you to buy the book and to experience the gratitude of your patients.
Richard Willner, DPM, New Orleans, LA
