PAD Often Diagnosed Too Late: MI Podiatrist
According to Conway McLean, DPM, "Some manifestations to our health are not so obvious. One of the most serious but also best hidden is the reduction in blood flow to all the varied cells composing the human body. Many Americans are destined to develop some degree of peripheral arterial disease, aka PAD."

"Early identification is rare, allowing atherosclerosis to worsen, which it so often does. When PAD progresses on the spectrum of occlusive arterial disease, critical limb threatening ischemia (CLTI) follows. The term refers to the afflicted individual’s inability to heal skin injury, often allowing the development of an infection. Again, without an adequate supply of the life-giving fluid which is blood, providing our tissues with nutrients and oxygen, repair will not occur. In the face of CLTI, new tissues cannot be manufactured," says Dr. McLean.

Pemafibrate Takes a Small Step Forward in DFUs
Pemafibrate was tied to potential prevention of leg and foot ulcers in type 2 diabetes, a silver lining to the PROMINENT trial's otherwise disappointing cardiovascular results. In an exploratory analysis, pemafibrate was tied to significantly reduced lower extremity ischemic ulceration or gangrene compared with placebo (2.1 vs 3.4 per 1,000 person-years, HR 0.63, 95% CI 0.41-0.96) over a median 3.4 years of follow-up, according to Aruna Pradhan, MD, of Brigham and Women's Hospital and Harvard University in Boston, and colleagues.
"The results of this exploratory analysis are promising. This is particularly important because it comes on the heels of potentially promising data from fenofibrate for reduction in risk for foot-level amputation," said podiatric surgeon David Armstrong, DPM, MD, PhD, of Keck Medicine of USC, Los Angeles, who was not involved with the study. "Having a medically-based therapy to reduce risk for lower extremity complications like diabetic foot ulcers (which occur every second around the world) could yield significant public health benefits,"

In the past few decades, healthcare has focused on money as a metric of practice success. Yes, we can only have a comfortable livelihood with money. Still, some areas of life are far more important than the girth of your investment portfolio or the size of your bank account.
9. Be socially responsible. - The United States has the most robust economy on earth and the highest per capita income of any country. However, if you measure infant mortality, gun violence, divorce, or suicide, we are at the bottom of the OECD. We do well in some areas but have more than our fair share of problems. More important than money is to be part of the solution rather than part of the problem. There are many causes worthy of your voluntary time or donations. Examples include Mothers Against Drunk Driving (MADD) and Habitat for Humanity, which helps build affordable housing.

RE: The Passing of Allan R. Stark, DPM
It is with heavy heart and deep sorrow that I share the passing of my mentor, associate, and dear friend, Dr. Allan R. Stark. Dr. Stark graduated from USC followed by the California College of Podiatric Medicine. He served in the U.S Army active duty and reserves for 25 years and retired as a Lieutenant Colonel.

Dr. Stark worked in private practice in the City of Orange, California. He was a strong supporter of the profession, prolific in recruiting students to podiatric medical schools and working to help further the scope of podiatric practice in the US. He served as President of the California Podiatric Medical Association and also Director and Instructor of The California School of Podiatric Radiography.
Dr. Stark also felt very strongly about supporting his community. As a lifelong member of Rotary, he served as President of his local club and later as District Governor for the Southern California region. Generous to a fault, he was the definition of a gentleman and kind soul. Not enough good things can be said about Dr. Stark. He will be sorely missed.

This is a procedure that Medicare likes to crucify podiatrists. Look up the exact wording that Medicare uses and repeat it. Always use anesthesia, bandages, etc. They will accept a typewritten procedure note in lieu of handwritten notes.

Anytime I had a Medicare audit, I got in touch with my malpractice carrier (PICA). They referred me to one of their healthcare consultants. It happed to be Robert Weatherford. If you have legal defense with your malpractice carrier, you must get in touch with them to go over your audit. Do NOT take this on yourself. They will go over your charting, billing, etc. and guide you through the audit. The CPT 11730 procedure code is one that I'm sure Medicare still scrutinizes closely.
You do not want your 4 chart request to turn into a bigger sampling once Medicare receives your charts, especially if you are billing this procedure more frequently than your peers. You are correct in your process of typing your handwritten charts without any alterations. If Medicare cannot read a handwritten chart, some auditors will not give you a favorable ruling. I had to do that with one of my audits. Back when I had to submit charts, the general advice I got was to submit both the original handwritten chart along with the typed version.

CPT code 11730 is defined as the following: "Avulsion of nail plate, partial or complete, simple; single." Unfortunately, it is one of the most frequently audited CPT/procedure codes due to the improper performance and documentation of this procedure. Here are the important facts regarding the performance and documentation of CPT code 11730:
1. Documentation must describe the symptoms and complaints which establish medical necessity for the treatment.
2. Nail or nail border must be separated and removed to and under the eponychium.
3. Local anesthetic (type and quantity) must be documented. If not used, provide rationale (i.e. neuropathic patient, patient refused, medical contraindications).
4. Post-operative instructions and follow-up care should be documented.
5. If the medial and the lateral borders are...

Does the ABFAS have third-party oversite outside of podiatry? I do not know the answer to this, nor was it addressed in the letter. I do know the American Board of Orthopaedic Surgery is partnered with the National Center for Quality Assurance (NCQA) and National Quality Foundation (NQF).
In our world, the American Board of Multiple Specialties in Podiatry (ABMSP) is accredited by the American National Standards Institute (ANSI) under the ISO International Standards ANSI/ISO/IEC/17024:2003 for Accreditation for Bodies Operating Certification of Persons, as well as accredited by URAC (former Utilization Review Accreditation Commission).

The recent “gatekeeper“ discussion in PM News is, in my opinion, reflective of the Dunning-Kruger effect in reasoning: there appears to be a failure to recognize one’s own incompetence. As noted by Dr. Tomczak and me in previous postings, the question is how far out of your lane do you wish to drive? Are you unconsciously incompetent?
The DPM degree is a restricted medical degree with restrictions on practice determined at the state level and locally by institutional delineation of privileges. A podiatrist today is exceptionally well trained for the evaluation and management of foot and ankle pathology. This is what you do. This is what you have been educated to do. This is what society, through its regulatory legislation, has determined, based on education and clinical training, has granted you permission and trust to do. You are entrusted to perform surgery, and to determine when surgery is or is not....
