WI Man Goes from DJ to Podiatrist
 
Long before the "great resignation," James Shea, DPM, started making a career move of a lifetime. He was a successful radio DJ, but in 2005, he went to Iraq to cover the war and it changed his attitude on life. “I wanted it to be different and significant.”

He started podiatry school at Barry University in South Florida at the age of 58, an age when many are planning for retirement. The University says the average age of students in its podiatric medical program is 30. At 62, Dr. Shea is in residency. “The hours are long, but the satisfaction great. As long as I’m licensed. I can practice until I’m a hundred years old and I plan to do it.”

South African Women is First Black Podiatrist in KaNyamazane
 
Being the first podiatrist in her community and one of the few in all of Nsikazi, Dr. Felicia Sandlane is one of Mpumalanga’s shining stars. Sandlane, a qualified podiatrist, is hailed by her community as a true example of black excellence following her rise to fame.

According to the 27-year-old, being one of the few young black women to have completed a course in podiatry is something out of the ordinary and a clear indication that women can make it in this scarce field. Sandlane added that podiatry is a rare skill and that there are currently 515 registered podiatrists in South Africa, with only roughly 300 practicing for a population of 53 million.

5 Value-Based Care Strategies for Leveraging Patient Data
 
Health plans and providers remain invested in using data to achieve improved care coordination while lowering costs, tenets of value-based care (VBC). As stakeholders team up to garner deeper, more holistic views of each patient to provide optimal treatment, here are five adoption habits that will guide the essential transition to VBC.

As regulation continues to support innovation in using healthcare data, the safe and secure availability of such data is driving better care. Query: Coding A Slant Back Procedure
 
If I have a relatively healthy established patient who complains of an ingrown nail every month or so, I perform a “slant back” procedure on the ingrown nail. What code should I use? I do not use injectable anesthesia and I will also have some discussion of wearing proper fitting shoes during the visit.
 
Can I just bill an E/M code even though a procedure was performed? CPT 11719 doesn’t seem to reimburse unless it’s a diabetic patient with Q codes. CPT 11730 requires injectable anesthesia and cutting to the eponychium, according to the LCD form First Coast Service Options. CPT 11720 does not apply because the nail is not mycotic. Any suggestions would be appreciated.

Response: This is routine foot care so most likely CPT 11719, CPT 11720 or HCPCS G0127. Based on your comments and saying First Coast Service, have the patient pay, have them complete an ABN and assign this as CPT 11719 GY. GY means it is statutorily not covered. There is no covered service for a “slant back” by itself.

I joined a private practice after finishing residency 7 years ago. So I am relatively young in age and experience. I do not use any EMR/EHR. For the first 6 months, I used a pen and paper templates. Then I created Microsoft Word templates and have been using them and printing notes and placing them in paper charts since. I take the MIPS/QIP, etc. hits from insurances, have cases of printer paper in the stock room, and have cabinets of paper charts. I could not be happier. My notes are not the canned, generated generic notes I get from so many other doctors/nurses.
 
“neck supple, trachea midline, no RRR, normal S1/S2, no bowel distension, AAOX3, sclera non-icteric, no jaundice, normo-cephalic, no Babinski sign.” Give me a break. I throw those notes out when they...

A permanent correction (11750) is the proper treatment for a chronic ingrown nail. But I was under the assumption that we don't do a phenol procedure if the ingrown nail is infected because it could spread the infection and increase the amount of post-op inflammation. I was told that was the standard of care, at least it was here on the East Coast, not to do a phenol on an infection, 
 
Your patient comes in with an infected ingrown nail. You do a 11730 and tell them if it returns, they would need a permanent correction, so they should return just when it starts to bother them before it becomes infected, or to come back for a determination in 3 months. They come back 6 months later (less than 233 days) with the same infected ingrown because they didn't get around to coming in when it first started hurting before it became infected. What do you do? How do we know it will return after one episode? If we tell them to return in 3 months for the permanent correction, how do we know it is not just a one-off ingrown due to shoes or another factor? Would we be over-utilizing the 11750 if we performed it 3 months after all 11730s?
 
You, the doctor, and the insurance companies really can't control your patients' behavior or inattention. But It sounds like the insurance companies have just decided to penalize us and cut our payments again for the behavior of our patients, no matter how we try to help our them.
