Walking Barefoot is Risky, Especially for Diabetics: CT Podiatrist
"Going barefoot, whether it’s in spring or summer or all year round, can cause a multitude of injuries. Most injuries are seen in the summer months. Some of the most common barefoot injuries are lacerations and abrasions, as they relate to glass and sharp objects (such as seashells) on the beach," says podiatrist Robert Weiss, DPM.

"People diagnosed with diabetes are more prone to infection from a cut and have a more difficult time healing the infected wound. This is due to circulation of the lower extremities in the more advanced cases. If it’s a minor cut, the individual will be fine after the foreign body is removed and the wound is cleaned with antiseptic and topical antibiotic cream. Sometimes, dry sterile dressings are helpful. But, if the wound is deep and bleeding doesn’t stop after 10-15 minutes with pressure applied, then it is time to seek emergency medical help," says Dr. Weiss.

For six years, Dr. Daly served as the podiatry coordinator for the Chicago portion of the Avon 2 Day Breast Cancer Walk. Prior to being coordinator, she was a volunteer for the previous nine years. She and her volunteer podiatrists and podiatry students treated more than 300 walkers at the 40-mile, two-day trek that raised funds for the fight against breast cancer. She is also one of the original walkers in the Beverly Breast Cancer Walk in 2000.

APMA Brings Premier Foot and Ankle Meeting to Salt Lake City
The APMA Annual Scientific Meeting (The National) wrapped up Sunday in Salt Lake City after four days of outstanding education, networking, and hands-on training. This very successful meeting was an outstanding opportunity to bring The National to a location convenient to APMA’s West Coast membership. Meeting highlights included a keynote address by Barry Scurran, DPM, who shared how his career path led him away from rock stardom and toward medicine and surgery. Surgical blitz sessions featured 10-minute lectures on the hottest topics in foot and ankle surgery today. The third annual Podiatry School Student Quiz Bowl was a standing-room-only event with a nail-biting, double-overtime finish that saw Tung Dao, Kent State University College of Podiatric Medicine Class of 2020, emerge as the winner.

Controversial debates and lunch-and-learn sessions allowed time for attendees to engage in thoughtful exchanges about timely topics, and the oral and poster abstracts once again brought cutting-edge science to The National. Podiatry Management inducted its two newest members, Samuel Mendicino, DPM, and Rembrandt Jackson, into its Hall of Fame during an awards luncheon. The National ended with the renowned Harry Goldsmith, DPM, Memorial Coding Seminar. The meeting also played host to prospective podiatry students during the Step Into Podiatry event, which included a very popular suture workshop. Save the date for the 2020 Annual Scientific Meeting, July 23–26 in Boston.

Questions to Ask about wRVU Compensation - Part 1
Physicians employed by a medical group, hospital, or academic institution are commonly compensated using a productivity formula based on work relative value units (wRVUs). Since wRVUs drive your paycheck, it’s to your advantage to ask questions in order to understand how the system works — and specifically how your employer counts wRVUs.
1. Are wRVUs credited on codes submitted or codes paid? In most cases, you will be credited for the CPT codes submitted, regardless of the reimbursement received. But get clarification about that, anyway. Furthermore, understand if reports are generated using date of service or the date the charge was billed out. This will make a difference for those services performed in one month that are billed out the following month. Remember: Delays in billing, whether by you or by the billing team, can delay wRVU credit for your compensation.

Query: QPP participation Status for MIPS reporting year 2019
When I go on qpp.com.gov to check my QPP participation Status for MIPS reporting year 2019, it states under my NPI Reporting Requirements, “this clinician is not required to report because they do not exceed the individual low-volume threshold. Because they and the practice exceed one or two elements of the low-volume threshold, however, the clinician or practice can opt-in to report as an individual or a group.”
I take this to mean that I do not have to participate in MIPS for the 2019 reporting year. Do I need to sign anything or do anything that states that I have chosen not to report or am I good to go?

Response: That is correct. Based on that information, you do not have to participate in MIPS in 2019. There is nothing else you need to do. You do not need to sign anything, send anything, or indicate that you are not doing it. They know you are exempt and nothing else needs to be done.

Stucco keratosis is a lesion that actually is unique to the lower extremity, ankles, and dorsal foot. It is usually no more than a cosmetic concern. Research has failed to pinpoint a cause. Some of the lesions are associated with HPV wart virus but it is only an association. Typically in my experience, there is evidence of sun damage in most people who have stucco keratosis, but this gives rise to the question of why they are not seen on the face. Others have written that they are variants of seborrheic keratosis based on molecular studies.
I have a theory that they are also related to hygiene issues; (not necessarily poor hygiene), that we simply do not as vigorously wash the skin of the legs and feet. I have had success with telling patients to use a soapy abrasive sponge in the shower (NOT BATH) and then using topical moisturizers of their choice. Liquid nitrogen and cryotherapy can resolve the lesions if necessary. Topical imiquimod and etretinate have had unimpressive results (healthyapron.com).

Locum Tenens and Long-term Coverage
A podiatrist in town will be medically disabled for at least six months and needs coverage for his office to try to maintain some income and keep his patient base. I and others are willing to keep some of his hours and his office open for him but do not want to risk running afoul of CMS, etc. We are looking for information regarding locum tenens (is it a total of 60 days per physician or can we have separate physicians for 60 days each?) and also the ability to see patients in HIS office which would be a new place of service for us (does this require a new POS and CMS 850 form?).

Editor’s comment: PM News does not provide legal advice. Services provided by an individual locum tenens cannot exceed a continuous period of more than 60 days. Each podiatrist can have a turn as a locum tenens. Claims payment is made under the name and billing number of the physician or the practice that hired the locum tenens physician. Every claim must have a rendering provider, so the practice would still use his or her name and NPI with modifier Q6 - services furnished by a locum tenens physician appended to the procedure code to indicate the service was furnished by a locum tenens physician.
The practice must keep on file a record of each service furnished by the locum tenens physician, with his or her NPI or Unique Provider Identification Number (UPIN).
