Two Legends Inducted into the PM Podiatry Hall of Fame
 
The  PM Podiatry Hall of Fame Luncheon is one of the signature events of the APMA Annual Scientific Meeting. This year's sold-old event honored Meet the Masters host Dr. Bret Ribotsky and former ABPS (now ABFAS) Executive Director James Lamb with Lifetime Achievement Awards and induction into the PM Podiatry Hall of Fame. Ribotsky was "roasted" by ABPM Executive Director Dr. Marc Benard. A video of the hilarious presentation will be posted on several podiatric websites in the near future.

The ceremony, emceed by PM's editor, Barry Block, DPM, JD raised over $10,000 for the APMA Educational Fund's Student Scholarships. PA Podiatrist Appointed ACFAP Vice President at APMA National 
 
During the APMA National in Philadelphia, the American College of Foot & Ankle Pediatrics (ACFAP) appointed Dr. Nicholas Pagano as their new Vice President. Dr. Pagano is owner of Barking Dogs Foot & Ankle Care in Plymouth Meeting, PA. He is course director and adjunct professor of pediatric foot and ankle orthopedics and surgery at Temple University School of Podiatric Medicine.

Pagano is board certified by ABFAS and is a Fellow of ACFAP, AAPPM, and ACFAS. ACFAP President Dr. Louis J. IL Podiatrist Receives Dr. G. Dock Dockery Special Achievement Award
 
The Board of Directors of the International Foot & Ankle Foundation for Education and Research (IFAF) selected Sean Grambart, DPM, FACFAS, President of the American College of Foot and Ankle Surgeons, to receive the Annual "Dr. G. Dock Dockery Special Achievement Award" for 2016.

Stubborn Verruca? Fry it With Microwaves: UK Podiatrist
 
British scientists have hailed a new method of getting rid of stubborn verrucas – by zapping them with microwaves. They say the technique is far more effective than traditional methods, such as burning them with acid and freezing them with liquid nitrogen, and have achieved a success rate of more than 75 per cent. It involves blasting the troublesome area with a beam of microwave energy for five seconds at a time.

Three treatments, spaced out over monthly intervals, are usually enough to see them disappear, said Dr. Ivan Bristow, a podiatrist who has helped pioneer the technique. He explained how the narrow beams of microwave energy heated the verruca up to 70C, "creating cell damage which brings about inflammation in the tissue. We believe that makes the body’s immune system spring into life, so it recognizes that it has to fight an infection," he said.

MACRA UPDATE
CMS Chief Tells Senate Panel MACRA Delay Possible
 
Centers for Medicare and Medicaid Services Acting Administrator Andy Slavitt told the Senate Finance Committee Wednesday the agency would consider delaying the implementation of MACRA, the industry-overhauling bi-partisan legislation set to go into effect January 1 of next year. In his testimony before the Committee, Slavitt conceded that small, rural and independent practices will struggle with the new rules, and a central theme emerging from the public comment period was the need to design a program with special consideration for these groups. "They do not have the resources of larger groups and each new administrative requirement takes time away from patient care," Slavitt said.
 
He said another central theme from comments made asked that CMS look for flexibility to allow physicians and other clinicians and their communities time to learn about and prepare for the sweeping changes. "We understand new rules require adjustment and preparation," Slavitt said. As such, Slavitt said CMS is "open to multiple approaches", including alternative start dates, shorter reporting periods, and other ways for physicians and clinicians to get experience with the program before "the impact really hits them."

MACRA, MIPS, and APMs — Are You Ready? - Part 4
 
2. The Advancing Care Information (ACI) component will change several things about Meaningful Use measures:
 
• The performance score consists of additional points awarded for certain objectives (maximum of 50 points). CMS focuses the performance score on the Stage 3 Patient Electronic Access, Coordination of Care through Patient Engagement, and Health Information Exchange objectives. Here providers can report on the measures in which they perform best.
 
• MACRA removes the "Clinical Decision Support" and the "Computerized Provider Order" measures since these are consistently above 90 percent and therefore considered widely adopted. It also reduces the number of measures to an all-time low of 11 measures, down from 18 measures, and no longer requires reporting on the "Entry" measure.

Block the toe, remove the fibular 1/3 of the nail, excise the granuloma, and apply AgNO3. If need be, send the nail, matrix, and pyogenic tissue to a path lab. Long-term antibiotics don't work. Get rid of the mechanical cause.

Avulsion will effectively drain and remove the foreign body (nail). If you have a healthy patient with no contraindications to local anesthesia, removal of the offending nail will often be curative. An onychia present since March or before, unresponsive to antibiosis, requires proper surgical drainage. The chronic onychia is capable of damaging the matrix as well as increasing the risk of acute osteomyelitis.

I would like to see the toe x-rays. From the photo, it looks like the patient might have a dorsal exostosis on the distal phalange of the big toe, causing the problem.

Dr. Richard Cowin is quoted as saying, “Air and water are very good for healing.” This is contrary to current and established standards of wound care and the science behind it. The “air” delivered to the wound is oxygen from the vascular supply. Wounds left open to ambient “air” have been shown to dry out and retard epithelial migration, thereby slowing wound-healing and closure. I am not sure why “water” is “very good” for healing, although saline solutions are typically used for cleansing and hypertonic solutions are bacteriocidal - that’s why many of us use Epsom salt soaks for open wounds. Maceration of open wounds is to be avoided.
 
Perhaps Dr. Cowin was misquoted by Prevention Magazine, but I don’t agree with the stated advice to “Remove the dressing at bedtime to let the blister air.” Wounds heal best in a clean and moist environment. The moisture is often provided by various antibiotic topical agents or hydrogels, among others. And, by the way, “wet to dry” dressings have been changed to “wet to moist.”
