APMA Sees Some Pluses in Proposed 2024 Medicare Fee Schedule
 
On July 13, CMS issued the calendar year 2024 Medicare Physician Fee Schedule Proposed Rule and Quality Payment Program Proposed Rule. APMA is still in the process of reviewing this almost-2,000-page document. The 2024 Final Rule is expected sometime in November. 
 
CMS proposes a CY 2024 conversion factor of $32.7476, down about 3.4 percent from 2023. Despite this change in the conversion factor, which will impact all physicians, podiatric physicians on average should see no additional aggregate decreases across the codes they typically use. While this outcome is better than recent proposals that decreased payment to podiatrists, APMA—along with other medical societies—will seek relief on the cut to the conversion factor, as has been the case in recent years.

Gary Lepow, DPM has been added to the Graduate Medical Education Committee of the Texas Institute for Graduate Education. Dr. Lepow was excited about the move for several reasons, including an intensive 3-year medical and surgical research program to promote and enhance podiatric research.

Ways to Successfully Gather Online Reviews for Your Practice
 
Patients often rely on Google searches to find a healthcare provider, and the number of positive reviews your practice has determines how well you’ll rank when searched. The more reviews you get, the more likely you are to bring in new patients. Here are somways to win more reviews for your practice and attract more patients over time.
 
1. Request feedback from satisfied patients.
 
Once you’ve established a relationship with your patient, one of the best ways to get a review is to ask for one in person. After their appointment, ask how everything was while they’re checking out and filling out final paperwork. If their response is positive, then ask if they can leave you a review. When you’re asking them in person, have something for them to scan like a card with a QR code that directs them to your Google review page. Or tell them you’ll be sending an email and/or text asking for their feedback.

Query: Post-Operative Infection
 
I have a patient who had a bunionectomy. The patient was diagnosed with a post-operative infection within the global period which required evaluation and management. I billed for an office visit but Medicare will not pay. What am I doing wrong? I used -24 as a modifier.

Response: In order to bill for an E/M service within the post-operative global period of a CPT/procedure code, whether the post-operative global period is 10 days or 90 days, a modifier must be added to the E/M code that is performed and billed. The modifier is the -24 modifier. The -24 modifier is defined as the following: UNRELATED E/M SERVICE DURING THE POST-OPERATIVE PERIOD – Use this modifier (only on an E/M code) when you perform an Evaluation and Management service during the follow-up period of an unrelated surgical procedure. You are entitled to bill for an E/M service performed during the follow-up period if that service is not related to the original surgical procedure. In this case, add the -24 modifier to the E/M service code. Make sure you reference this service code to the appropriate unrelated diagnosis on the billing claim.
 
A bunionectomy was performed on a patient. Following the performance of the procedure, a post-operative infection developed within the post-operative global period of 90 days. Despite the fact that the patient needed to be examined and treated during this E/M encounter, since the E/M service that was performed was clearly directly related to the procedure that was performed, that set the global period. This would be an incorrect use of the -24 modifier and the E/M service is not covered. That is precisely why the E/M service was not reimbursed. There is not another modifier that can be appended to the E/M service to allow for reimbursement.

I read with interest how Dr. Wagner helps darker skinned people with their corns after they use over-the-counter corn plasters. I have never heard of excising the “corn lesions” only as a treatment for corns. He said the results were seen immediately. From reading the article, I think he excises the white burned area from the corn plaster. Does anyone else do this, and is this surgical procedure acceptable for this problem?
