Podiatrist Peter Allton, author of the book Undefeeted by Diabetes, is delighted that Diabetes UK has today launched a campaign called ‘Putting Feet First’ which aims to bring an end to preventable amputations caused by diabetes.

Viamet to Present at APMA Annual Scientific Meeting
 
Viamet Pharmaceuticals, Inc. today announced that data from a Phase 2a study of VT-1161, the Company's lead novel, oral antifungal compound, will be presented at the American Podiatric Medical Association 2015 Annual Scientific Meeting, to be held from July 23-26, 2015, in Orlando, FL. In addition, the company will also provide an update on the ongoing RENOVATE (Restoring Nail; an Oral VT-1161 Tablet Evaluation) study, a Phase 2b clinical trial of VT-1161 for the oral treatment of toenail fungal infection, also known as onychomycosis.

NY Law is Model For Helping Consumers Avoid Surprise Out-Of-Network Charges
 
If an out-of-network doctor or other provider doesn’t have a contract with a health plan that determines how much they get paid for services, they may bill the patient for any charges not covered by insurance. In those instances, the consumer could be on the hook for the rest of the bill, a practice known as “balance billing.” Although there’s no federal protection, about a quarter of states have laws on the books against balance billing by out-of-network hospitals, doctors, or other providers in at least certain circumstances, such as emergency care.
 
Among the most recent states to take action is New York, which has a new statute that is cited by consumer advocates as a model. That law, which became effective April 1, significantly expands existing consumer protections and may be the most comprehensive law of its type, says Jack Hoadley, a research professor at Georgetown University’s Health Policy Institute, who recently co-authored a study that examined balance billing protections in seven states. Under the New York law, patients are generally protected from owing more than their in-network copayment, co-insurance, or deductible on bills they receive for out-of-network emergency services or on surprise bills.

If you cannot convince your patient the only chance he has is to quit smoking, then why run up an expensive Rx bill?

It  would be wise to carefully monitor this patient's liver enzymes and even palpate to make sure there is no enlargement of the liver. This is important when treating a patient with alcoholic neuropathy who is also taking drugs which also are metabolized in the liver.
 
One case in our practice, which stands out vividly in my mind, was an alcoholic who presented with severe neuropathic pain. I ordered a serum vitamin B12 test and it came back borderline normal. I gave him a couple of vitamin B12 injections, and with two shots, his symptoms totally abated. My blood lab has subsequently added an addendum on its reports warning physicians that even low normal levels of vitamin B12 can account for some symptoms.

I have some concerns about a DPM (or for that matter any physician) who wants to add medications to an unrepentant smoker with serious, disabling neuropathy. One of the medications he takes is Trazadone, a serotonin modulator that has suicidal and violent ideation as just some of its side-effects. Although this is more common in younger patients, we already know the patient is depressed. But there are more: 
 
Sertraline (Zoloft) also has similar side-effects and concerns. Oxcarbazepine is usually used to treat seizure disorders, but also includes bipolar disorders. Gabapentin is also used to treat mood disorders. By any metric you choose, this patient is...

RE: Healthcare Insanity

An environment is being created where it is becoming increasingly difficult to practice. Pre-authorizations are at an all-time high, audits are at an all-time high, ridiculous sets of rules are being given, i.e, "Did your patient have six months of conservative care for that painful hammertoe that your patient can no longer put her foot into a shoe?" At what point, if any, is enough enough? 
 
ICD-10 appears to be a complete farce, nothing more than an attempt for RAC auditors to make more money off of our hard work. I was informed last week that an HMO I contract with is now contracting out certain codes with a subcontractor. Of course, they won't supply a list of codes that goes to either. So, while my patient got her hammertoe procedure approved (after she yelled at the nurse reviewer for giving me a hard time), we are still waiting to find out from the other company if the met osteotomy is allowed. You just can't make these things up. 
 
Insanity has been defined as the continuation of repeated behavior expecting different outcomes...or is that stupidity? Since, I must be insane, does anyone know if I can collect partial disability for that?  Or am I just stupid and out of luck? I'm guessing the latter.

Dr. Sullivan asks whether a person should be taken off statin drugs, which are metabolized in the liver, when placing a person on a pulse dosage of oral Lamisil which can also raise liver enzymes. The same question could be asked about other drugs. Should a podiatrist take a patient off of anticoagulants, even oral aspirin, when he or she is preparing to do a foot procedure?
 
As a rule of thumb, it is probably unwise to take a patient off of another doctor's ordered medications without communicating with that doctor first. Many years ago, one of my patients suffered a CVA because another surgeon took him off of his daily aspirin in order to safely perform an elective (non-podiatric) procedure. Unfortunately, things "fell through the cracks" and no one ever told the patient to go back on his aspirin regimen. The rest is history.
