UW–Madison Department of Surgery Welcomes 4 Podiatrists
 
At the beginning of July, the University of Wisconsin–Madison Department of Surgery welcomed four new podiatrists. John Finnell, DPM, has been a practicing podiatrist with University Podiatry Associates for the past 28 years, providing exceptional care to patients throughout the region. Jill Migon, DPM. completed her podiatric medical and surgery residency training in Denver at the Highlands Institute-Presbyterian/St. Luke’s Medical Center with an emphasis on reconstructive surgery and limb salvage.

OH Podiatrist Explains Why Properly Fitting Shoes Matter for Growing Children
 
Shari Nichols, DPM, a podiatrist for UHS and director of the UHS Center for Advanced Wound Care, says that children can quickly outgrow their shoes, especially during growth spurts. Dr. Nichols says shoes that are too small or heavily worn can lead to pain, ingrown toenails, and a lack of proper support. “If they're in shoes that are really worn out, they're going to get pain,” Nichols said. “A lot of people will say, ‘Oh, it's just growth spurts or growing pains.' But actually, it could just be that things are worn out.”

Dr. Nichols says that worn out shoes may no longer provide the shock absorption or support children need. Poorly fitting shoes can also contribute to knee or hip pain and cause problems with a child's toes over time. For families struggling to keep up with a growing child's shoe size, Nichols says that access to new shoes can make a difference beyond just physical health.

The book organizes high-yield surgical material into a focused and practical format, covering essential concepts in forefoot, midfoot, rearfoot, and ankle surgery. Places to Look for Revenue Cycle Leaks Before They Drain Your Practice
 
1. Decode the remark codes that signal payer down coding
 
Down-coding hides in plain sight on the remittance. Two codes are the tell. N22, which reads "this procedure code was added/changed because it more accurately describes the services rendered," means the payer swapped the CPT you sent on the 837 for a different one on the 835. CO-150, "payer deems the information submitted does not support this level of service," is the other fingerprint. Billed a 99214 and got paid a 99213? Sent a 99243 and got paid a 99204? That is the swap.
 
The behavior shows up across payers in different combinations: Humana pairs CO-131 with N22, Aetna and UnitedHealthcare use CO-45 with N22, Anthem leans on CO-186, Sunshine and other Centene plans use CO-150, and Cigna runs a two-step adjust-and-re-adjudicate trick with CO-45, OA-94, and N130. Payers strip modifiers, too: in one Cigna remittance, a 99204 billed with a -25 modifier came back with the modifier gone and the added service unpaid. Pull a report that surfaces these remark codes by claim so the swaps stop slipping past in the daily posting.

5 Ways To Increase Your Investment Income In Retirement
 
Finding income investments in retirement requires a careful balance. Here are five ways to boost your cash flow without taking on unnecessary risk.
 
2. Real estate investment trusts (REITs)
 
REITs let you earn income from real estate without managing physical property. These companies own or finance income-producing real estate such as apartment buildings, shopping centers, healthcare facilities, or data centers and trade like a normal stock. Since REITs are required to distribute at least 90% of taxable income to shareholders, they generally provide higher yields than other stocks.
 
The trade-off is that REIT income is not risk-free. Jason Fannon, a certified financial planner, cautions that REITs can be cyclical and closely tied to broader real estate conditions. Interest rates can also impact returns: "Rising rates can increase borrowing costs and make them less attractive compared with other fixed-income securities." REITs can also use leverage, which might amplify returns but also exacerbate losses, Roland Chow adds. REIT income is typically taxed as ordinary income, so it doesn't receive the more favorable qualified dividends tax rate.

7. Patients with dementia are often scared
 
One of the hardest but most important lessons: dementia patients aren’t just confused, they’re frightened. They sense something is wrong but can’t grasp why. This was especially true in vascular dementia patients who were painfully aware that their memory and function were slipping. The worst thing we can do is remind them of what they’ve forgotten or correct them repeatedly.
 
The better path: re-assure them, smile, and let them know you’re there to help. I was deeply moved watching the geriatric physicians I worked with—patiently listening to the same story for the fifth time, treating it as though it were being told for the first. That patience is its own kind of medicine.

I had a similar issue. My staff made some minor mistake on our re-application. We didn't find out until we were denied DME claims. She had fixed the mistake in time and there were no mistakes. They told us they were "still processing it". This went on for months. It started in May. In September, we contacted PICA because they cover things like this. The lawyer thought it was a big joke and did nothing. All I got was a huge increase in my PICA premiums. 
 
Finally, in January, we contacted our congressman. In about 2 weeks, the ball got rolling and our re-application was approved. They did pay for the denied claims from over the previous year. I imagine they will pay for your claims made after you re-applied but not in between lapses. You can bill the patients.

RE: WISeR Follow Up and AI

Since my last post on the House Appropriations Committee action on the WISeR program, the House Committee did in fact move forward with an amendment which would block further Federal funding for the program. Most recently, Virtix, the WISeR vendor in Washington State, who depends on AI to approve or deny prior authorizations, was cited having failed multiple audit subjects across five areas of complaints. This ultimately led to the WISeR program being unable to meet the 72-hour turnaround time. CMS has ordered the AI vendor to submit a Corrective Action Plan. How this may impact your claims and other WISeR models is still unclear.
 
If your Prior Authorizations are not sufficiently resolved (either approved or denied), you should report these incidents to your local and regional Medicare office in your MAC.

Kudos to Dr. Gary Dorfman on podiatric identity. As one of the older members of our profession, this post truly resonated with me. When I entered podiatry school nearly 60 years ago, I confess that I carried a degree of professional insecurity. Our profession was still working to earn its rightful place within the medical community. There were only 5 or 6 one-year internships/residencies. We were relegated to performing foot surgery in our offices, as surgical privileges in hospitals were unattainable.
 
Looking back, it has been a privilege to witness the remarkable evolution of our profession. As Dr. Dorfman posited, “podiatrists are highly trained physicians and surgeons who play an essential role in limb preservation, diabetic care, sports medicine, reconstructive surgery, and multidisciplinary healthcare.” Thankfully, we have come a long way baby. All of the discussions and angst about parity, plenary licensing, student debt, and decreasing enrollment in our schools are real but…
