NC Podiatrist Discusses Key Steps to Healthy Feet
According to Jane Andersen, DPM, "If you are outside where you need to protect your skin with sunscreen, don’t forget your feet. If you are barefoot or wearing sandals that expose the tops of your feet, they can get burned more easily than you would imagine. It is easy to forget that the tops of your feet need protecting just as much as the rest of your body."
|
Dr. Jane Andersen |
"Walking barefoot on a lush lawn or damp sand can be a delight. But skipping supportive shoes for long periods of time can put you at risk for plantar fasciitis. If you experience heel pain, especially in the morning when you first get up, you may want to rethink those bare feet. Choosing appropriate shoes to wear in the house or on the beach are important steps for healthy feet," says Dr. Andersen.

MEDICARE NEWS |
Medicare Rates in 2025 Would Cut Pay For Docs by About 3%
Federal officials on July 11 proposed Medicare rates that effectively would cut physician pay by about 3% in 2025, touching off a fresh round of protests from medical associations. The 2025 draft base rate, or conversion factor, is slated to drop to $32.36 from the current level of $33.29, the Centers for Medicare & Medicaid Services said.
This proposed cut is mostly due to the 5-year freeze in the physician schedule base rate mandated by the 2015 Medicare Access and CHIP Reauthorization Act (MACRA). Congress designed MACRA with an aim of shifting clinicians toward programs that would peg pay increases to quality measures.

RELEVANT RESEARCH |
Skin Substitute Developed at Temple Could Revolutionize Wound Care
A skin substitute developed by bioengineers at Temple University could be a breakthrough in wound care, pending its approval by the FDA. The technology uses soy proteins and it could change how burns, bed sores, diabetic foot ulcers, and other wounds are treated. The Temple bioengineers have produced a nanofibrous wound dressing from soy protein for burns, bed sores, diabetic foot ulcers, and other wounds, to help heal and regenerate healthy skin. Because it has a long shelf-life and does not need refrigeration, this treatment could be valuable in countries with limited medical care, military settings, and street medicine, Joseph Connell said.
Connell anticipates FDA approval by the end of the year, at which point Temple will begin human trials in its podiatry department, since approximately 15% of people with diabetes have foot ulcers that can become infected and sometimes lead to amputation.

PHYSICIANS AND THE LAW |
Surgeon to Pay $5 Million for Restriction of Negative Reviews, Directing Fake Reviews
A Seattle plastic surgeon who illegally restricted patients from posting negative reviews about his practice and directed his staff to post fake positive reviews will pay $5 million for violating Washington state’s consumer protection law. According to a July 1 consent decree, Javad Sajan, MD, and his practice Allure Esthetic must pay $1.5 million in restitution to 21,000 patients and $3.5 million to the state for manipulation of patient ratings.
The settlement resolves a federal lawsuit brought by Washington State Attorney General Bob Ferguson that accused the doctor of illegally suppressing patients’ negative reviews by “forcing” them to sign nondisclosure agreements (NDAs) before they received care. In an April decision, US District Judge Ricardo S. Martinez sided with the state, ruling that Allure Esthetic’s actions violated the federal Consumer Review Fairness Act (CRFA).

PRACTICE MANAGEMENT TIP OF THE DAY |
10 Things More Important Than Money
In the past few decades, healthcare has focused on money as a metric of practice success. Yes, we can only have a comfortable livelihood with money. Still, some areas of life are far more important than the girth of your investment portfolio or the size of your bank account.
7. Polish your reputation.
As physicians, we live and die by our reputations. We may spend years creating and polishing our reputations. What takes years to develop can be wiped out by an unhappy patient with just a few clicks on a review site or a lawsuit that implies that we were negligent in our care of a patient. As those around us chase more fame, money, power, and worldly things, they often devalue or underinvest in relationships, such as their friends, family, and others who made it possible for us to succeed. Look at your review sites and see what others say about you. If it is less than five stars, then make every effort to repair your personal as well as your professional reputation.

RESPONSES/COMMENTS (MEDICARE NEWS) |
From: Paul Kesselman, DPM
Considering the inflationary rates since 2001, this translates to a 22% reduction in pay in the last twenty years. This, combined with millions of baby boomers entering the Medicare system (57% by 2050) along with many providers retiring from practice, is going to create significant delays in accessing healthcare.
There clearly are indications that CMS is not really interested in the concerns of the provider, nor do they have the best interest of the patients in mind.

RESPONSES/COMMENTS (APMA NEWS) - PART 1A |
From: Jon Purdy, DPM
It’s a little laughable that our own national association has difficulty in defining who and what we are. I’m a podiatrist and I’m confused by this statement. Our state medical boards restrict us in different ways. Our training limits us to certain structures of the foot above and beyond state scope of practice.
The lumbar spine at minimum is a governing and related structure. We treat global manifestations of systemic conditions such as neuropathy, allergic reactions, and bloodborne conditions, to name a few. So we treat the leg below the tibial tuberosity but specialize in the foot and ankle? So now we are an orthopedic sub-specialty that further sub-specializes?
Our profession is archaic and continually trying to define itself. No other “physician” by definition has such issues. The MD and DO specialties are well defined, not restricted by scope, and perform within their specialty, secondary to agreement between the local healthcare entities and the physician’s level of training. It seems most of our discussions, be it professional practice, medical, surgical, or political, point us in the direction of achieving parity through inclusion in MD or DO modern medical practices.
Jon Purdy, DPM, New Iberia, LA

RESPONSES/COMMENTS (APMA NEWS) - PART 1B |
From: Robert Scott Steinberg, DPM, Daniel Chaskin, DPM
What is missing from this conversation is how the public and others in healthcare perceive us. At least once a month, a patient asks me, do you do surgery? Orthos are not referred to as orthopedic doctors, are they?
At Humboldt Park Health (formerly Norwegian American Hospital), our department, which used to be a section of surgery, is now called the Department of Podiatric Medicine and Surgery and has been for over 12 years. The Illinois Podiatric Medical Association has been renamed the Illinois Association of Podiatric Medicine and Surgery. I support the APMA's recommended change.
Robert Scott Steinberg, DPM, Schaumburg, IL
The APMA has finally succeeded in proposing a definition that would increase our scope of practice by including the treatment of systemic diseases as part of podiatric care. The APMA likely agrees that we are part of an honorable profession and our integrity will provide public safety. Podiatrists as well as other healthcare professionals understand even if they are licensed, a license is not a granting permission to give treatment for which a healthcare practitioner is not currently competent at performing.
This is one more reason why podiatrists should consider joining the APMA and our state societies.
Daniel Chaskin, DPM, Ridgewood, NY
