Singapore Podiatrist Provides Salon Safety Tips

A fungal infection can spread through tools that have not been properly cleaned. It can also spread through nail polish, if the person using it before you has a fungal infection. "Getting an infection from unclean equipment used during a pedicure happens quite often," said Ms. Jessica Tennant, a podiatrist at Orchard Clinic.

Jessica Tennant

Make sure the instruments used for trimming your nails are properly sterilized, for instance, by an autoclave sterilizer. If you can, buy your own tools and ask the pedicurist to use your own equipment. Or visit salons that use disposable items such as emery boards.

PODIATRISTS IN THE COMMUNITY

CA Podiatrist Holds Christmas in July Shoe Drive

Hundreds lined up outside the St. Mary's Interfaith Dining Hall on Saturday morning for the 11th annual Christmas in July Shoe Drive. Dr. Craig Garfolo founded the shoe drive in 2003. Garfolo, a podiatrist, said he first recognized the need for a shoe drive while serving as a volunteer at St. Mary's.

Dr. Craig Garfolo

"The main mode of transportation for these people is their feet," Garfolo said. "They're prone to stepping on things, cutting their skin, etc., and without good, protective shoes, that can lead to problems."

QUERIES (NON CLINCAL)

Query: Sudoscan

I just spoke to a reputable sales rep about the Sudoscan machine for detection of peripheral neuropathy. I think the test is easy to do and does decisively test, both quantitatively and qualitatively, the dysfunction of sweat glands. The test does what it says it does, and also seems billable to insurance companies, but is this a test DPMs should perform? Is this a screening test for sweat glands and just a test that only INFERS peripheral nerve dysfunction? If there is a result which requires a referral to a specialist, whom do we refer to? I would like anyone familiar with the unit to answer these questions.

RE: Repair EHL Rupture? (Joe Gonzalez, DPM)
From: Daniel Pollack, DPM

Get a stat MRI to see how far the proximal end of the EHL retracted, and then primarily repair it if possible, or else it may need a graft augmentation. Worst case scenario, it is a tenodesis, but it needs to be repaired.

RE: DVT Protocol
From: Zeeshan Husain, DPM

Here is the form that two of my local hospital systems use for DVT prophylaxis. The guidelines are based on the recommendations from the following articles:
Geerts, et al., Chest 133:381S-453S, 2008.
Seruya, et al., Plastics and Reconstructive Surgery 122(6):17 01-08, 2008.
Hatel, et al., Plastics and Reconstructive Surgery 122(1): 269-79, 2008.

The form evaluates the patient for risk factors and...

RE: Compensation for Medical Report
From: David Gurvis, DPM

I also charge a flat beginning fee of $1,000.  That will normally cover the basics to write a report. If I go over 2 hours, the charge is $500/per hour. I get my check in advance, having learned the "hard way" years ago.

If I am later asked to do a video deposition, the clock begins running again starting at $1,200. I have never had a problem getting paid.

RE: Inexpensive Cast Cutter (Judd Davis, DPM)
From: Paul Busman, DPM, RN

In my home shop, I have a Sears Craftsman 4-drawer rolling tool chest which costs around $150. Our hospital OR had several of these same carts which were used for difficult intubation carts, difficult catheterization carts, pediatric intubation carts, etc. Not long ago, the hospital, in its infinite wisdom, replaced those carts with "real" medical ones which look remarkably similar. I happened to get a look at the catalog from the supply house, and these carts cost nearly TEN times as much as the Sears version. And people wonder why healthcare costs are so high?

RESPONSES / COMMENTS (NEWS STORIES) - PART 1A

RE: Good Hygiene Prevents Fungal Infections/Odor: Singapore Podiatrist
From: Mike Boxer, DPM

Comment was made that foot odor comes from bacterial action on sweat. Any patient who has foot odor will tell you that they can wash their feet, dry them well, and within five minutes after putting on their footwear, their feet have a terrible odor. All patients with foot odor have hyperidrosis. The odor comes from bacterial action on the wet leather, and not bacterial action on sweat. Once the shoe is worn, the foot picks up the odor from the leather.

The only way for a patient to cure bromidrosis is to replace all footwear that has an odor. Often, the use of Bromi-Talc Plus (Gordon Labs bromidrosis powder) that contains Abscents Deodorizing Powder can rid the footwear of the odor. To prevent bromidrosis, hyperidrosis must be well-controlled to keep shoe leather from getting wet with sweat.

RE: Good Hygiene Prevents Fungal Infections/Odor: Singapore Podiatrist
From: Doug Richie, DPM

To clarify a slight error in the original posting of this story,  Chelsea Law is not an Indonesian podiatrist. She practices in Singapore. Through a partnership with the California School of Podiatric Medicine at Samuel Merritt University, I had the privilege of serving as a visiting professor for the entire Singapore podiatry staff in March this year. Ms. Chelsea Law was one of my organizers and hosts for this one-week experience. She is truly one of the most delightful, professional, and compassionate podiatrists I have known. Congratulations Chelsea!
