PODIATRISTS AND FOOTWEAR
Wearing Flip-Flops Can Lead to a Host of Foot Problems: MA Podiatrist

“If you wear flip-flops for a long period of time, the lack of support can lead to stress and strain on joints and tendons,” said Dr. John M. Giurini, Beth Israel Deaconess Hospital’s chief of podiatry in an online article.

Dr. John Giurini

“If you have a mechanically unsound foot to begin with, wearing flip-flops can actually lead to a host of lower extremity problems, such as arch pain, heel pain, ankle pain, or knee pain.”

PODIATRISTS IN THE COMMUNITY
Podiatrist Returns to His Community to Open a Practice

The Manteca medical community has a new homegrown podiatrist, Jack A. Harvey, DPM. Dr. Harvey is a graduate of Manteca High School. He attended the California School of Podiatric Medicine.  Harvey just completed a three-year residency within the Kaiser Hospital system involving everything in foot care from trauma and diabetic care to treating infections that might develop and need to be controlled.

Dr. Jack Harvey

In the Kaiser system out of Vallejo, he rotated through four different hospitals. During his residency he received valuable experience and an education in treating club feet in children, he said.  “Family is what I love,” he quipped, saying he looks forward to treating children as well as the adults who come to him.

HEALTHCARE LEGISLATION
VA Provider Equity Act Introduced

On July 9, HR 3016, the VA Provider Equity Act was introduced in the U.S. House of Representatives. This legislation would recognize podiatrists serving in the Veterans Health Administration under the Physicians and Dentists authority. Rep. Brad Wenstrup, DPM drafted the bill as the culmination of a long-term lobbying effort which was conducted in response to Resolution 17-11, which seeks to resolve long-standing recruitment and retention problems for podiatric physicians at the VA.

Dr. Brad Wenstrup

Recognition under the Physicians and Dentists authority will not only ensure that podiatrists are paid at the physician level within the VA, but that individual facilities will have the tools necessary to effectively and efficiently recruit and retain the most experienced providers. This recognition will also ensure that podiatrists enjoy the same responsibilities and opportunities as their allopathic and osteopathic colleagues within the VA.

PODIATRISTS AND E-HEALTH
AZ Podiatrist Helps Lead Charge Against ‘Medjacking’

It’s the stuff of primetime TV drama – terrorists hack into a vice president’s pacemaker and assassinate him with electrical shocks to the heart. While the storyline is a work of fiction, the potential for “medjacking” – or malicious medical device hacking – is real. David Armstrong, DPM, MD, PhD, professor, University of Arizona Department of Surgery, is joining forces with the U.S. Department of Homeland Security, the U.S. National Security Council, NASA. and other government agencies and industry leaders to create strategies to keep the world safe from medjacking.
Dr. David Armstrong

Dr. Armstrong is the lone medical academician on the Cybersecurity Standard for Connected Diabetes Devices Steering Committee, which meets for the first time July 20-21 in Bethesda, MD. While devices associated with diabetes are the initial focus, Armstrong said the committee is expected to examine the security of other medical devices. “As connected devices become more pervasive and powerful, the potential for malicious medical device hacking is becoming increasingly real,” Armstrong said.

SUCCESS TIPS FROM THE MASTERS
Bret Ribotsky: Should we be measuring for equinus in patients' normal footgear or barefoot?
Drs. Patrick DeHeer and Nick Campitelli

Nick Campitelli:  This has always been a concern with me. We work to stretch the Achilles tendon or lengthen it with the goal of getting the foot to dorsiflex to 10 degrees, and then we put the foot in shoes that have 8-16mm shoe drop.

Patrick DeHeer: The literature is full of studies (26 studies) of plantar fasciitis with equinus. Studies show 80% of patients with plantar fasciitis have equinus. There have been four peer- reviewed studies in orthopedic literature that show doing a gastric resection to treat plantar fasciiitis without touching the foot have approximately 80% success rates. I have been doing this for a few years and I am noticing similar results.

Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is to be announced. You can register for future events by clicking here

CODINGLINE CORNER
Query: Post-Operative Ulcer

If following an amputation, the patient develops an ulcer at the site of the amputation, is the treatment of the ulcer billable? Or is it considered a complication following the surgical procedure?

James Holdermann, DPM, Medford, OR

Response: The answer may depend on the payer and what needs to be done to treat the ulcer. For Medicare, it would probably be lumped into the global period. If the post-op ulcer care is more of a local dressing type treatment, then it may be not payable outside the global allowance. If you had to return to the operating room, then that would be payable and the global period resets.

Payment of ulcer supplies, dressing material, skin substitutes should be pre-authorized as best you can. You may want to enlist the help of home healthcare agencies to render some of the "post-op' ulcer care and take some of the strain off your office.

Tony Poggio, DPM, Alameda, CA

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Avoid the Sugar Crush
From: Hal Ornstein, DPM

Two years ago, I developed chronic pain in my shoulders and both upper extremities, and was feeling cold in the midst of summer in New Jersey while also feeling what one would call depression with difficulty sleeping. I searched for answers, visiting multiple top-rated medical specialists over several months with no answers and no treatments other than medication to control the neuritis and sleeping issues. For months, I went for physical therapy, yoga, massage, and acupuncture, and with all this, I had little relief and started losing hope.

A naturopath was recommended to me. After taking a short history and reviewing years of blood work, he hit the nail on the head. I had a systemic candida infection with...

RESPONSES/COMMENTS (PRACTICE MANAGEMENT)
From: Brian Kashan, DPM

In reading the responses to possible solutions for reducing no-shows, several people have suggested charging the patient's credit card. The information is obtained when the appointment is made, and if the patient doesn't show, a charge is submitted.

While this seems to address the issue quite well, there is additional risk with this type of transaction. Once you store credit card information, you have a responsibility to protect this information. There are HIPAA requirements and responsibilities that come with storage of credit card information. I suggest that, if one decides to use this method, they research the requirements and any possible data breach protocol required. As fines for not protecting this information adequately are exorbitant, it behooves everyone to research this, consult with the appropriate data security people, and determine what the requirements are in your particular area.

RE: Outrageous Shoe of the Day

And you thought your shoes were torture!

These shoes reflect the game of control between the sexes, 2015. The foot is placed in a high hill position inside a 'torture splint'. In a process of incarcerating ritual, the shoe is assembled around the foot.
