CPMA Announces Officers and Directors

Dr. Carolyn E. McAloon, a podiatric physician and surgeon, was installed as the 101st president of the California Podiatric Medical Association (CPMA) during the organization’s Annual House of Delegates held in Anaheim in late June. In doing so, she became the fourth woman in the Association’s over 100 year history to be elected to lead the organization.

Good Hygiene Prevents Fungal Infections/Odor: Singapore Podiatrist

Podiatrists say the best way to avoid stinky feet, as well as a fungal infections, is to keep the feet clean and dry. "In the majority of cases, feet smell bad due to the by-products of bacteria acting on the sweat that our feet produce," said Ms. Chelsea Law, principal podiatrist at Khoo Teck Puat Hospital.

Good hygiene helps reduce the odor. Feet, including the area between the toes, need to be cleaned daily with soap and water, she said. It is also important to dry the feet thoroughly, particularly in between the toes, to lower the risk of getting a fungal infection, she added. So change your socks every day. Socks act as a protective layer between the feet and the shoes, helping to absorb moisture from the feet.

Sarah E. Dickey, DPM, has joined Loyola University Health System’s Division of Podiatric Medicine and Surgery. She is certified in diagnostic musculoskeletal sonography and uses ultrasound to diagnose pathology of the foot and ankle and to perform guided injections.

QUERIES (CLINICAL)

Query: Repair EHL Rupture?

A healthy, 48 y/o truck driver dropped an angle iron across the top of his left foot 3 weeks ago, went to urgent care, and had the laceration sutured closed. He states they said he did not lacerate any tendons. The patient states he has noticed the hallux is "floppy." On physical exam, he is able to dorsiflex his hallux at the MPJ with the use of his EHB, but he has a clearly palpable complete rupture of his EHL. I can feel the distal end of the EHL in the distal third of the first innerspace, and I cannot palpate the proximal end (which is likely to be under the retinaculum).

The patient has no complaints when he wears a shoe, and isn't really interested in surgery. After 3 weeks,  I am wondering how much more function I could restore with a delayed repair? Would an isolated hallux IPJ arthrodesis be a better solution (as his EHB strength and MPJ dorsiflexion is good), or should I push for the EHL repair?

I applaud Dr. Caringi for implementing the interpositional arthroplasty for hallux limitus and rigidus deformities. It is my opinion that joint implant procedures and arthrodesis are poor alternatives for the young or athletic patient. I have been using this technique since 1991, and in the November 1995 of the Journal of Foot and Ankle Surgery, you will find my article on "Tendon Interpositional Arthroplasty."

Each and every person who works in healthcare is required to be trained in HIPAA annually.  This includes those of us who work at 'Covered Entities' and those who work at 'Business Associates.' A Covered Entity is a doctor's office, hospital, lab (such as Labcorp, Bako, and Quest), claims clearinghouses (such as Emdeon and Gateway), and Health Insurance Companies (such as Blue Cross and Aetna).

A Business Associate is anybody that a Covered Entity shares patient information with such as orthotic labs, transcription companies, billing services, and EHR vendors.

I expect that most of the people who read PM News are required to have online training. I provide monthly webinars that provide this required training, and have provided this training at the Region IV conference in Ohio, The Region III conference in Atlantic City, and other meetings. When you get a request like this from an attorney, you should immediately contact your malpractice insurance agent and/or carrier for advice, especially since it is part of a lawsuit. Sometimes, these reports can provide information that may later lead to an allegation of malpractice. Your insurance provider will usually be interested in helping you with requests like this, as it can help avoid a potential claim down the line. In addition, the attorney should provide a signed release before any information is divulged.

When I started my practice 33 years ago, instead of spending $100 on a stainless steel kick bucket and rollers from a medical supply company, I saw a pile of hundreds of them stacked up alongside a barn while driving in Pennsylvania. I found the farmer, asked him what he wanted for one of them. Five bucks! Stainless steel. I then went to the local Electrolux store, and for another $5, got a round roller base which the bucket fits perfectly on. I still own this.

One word of caution: Working in risk management for the last 17 years has taught me if something were to go wrong with your Sears oscillating multi-tool and your patient gets injured, don’t count on Sears to share any damages with you, as their saw was never designed, marketed, or meant for medical use (although we all know it’s the exact same saw). A good percentage of just about anything we purchase, doctor and non-doctor alike, goes for product liability and insurance.

"It's disturbing to me that once a piece of equipment is labeled "medical", the price can be marked up ten-fold or more."

I had to laugh when I read Dr. Davis' comment about something labeled as "medical" being marked up. It reminded me of something that happened to me a few years ago. I have had three Great Danes and two Dobermans who have had their ears done. There is a round tubular material called drainage tile that we use to hold the ears in an upward position. As I remember, it sold for about $6.00 for a 50 foot roll in any hardware store.

 A few years ago, I had a rep come to my office and asked if I used toe crests. I told them I usually made them out of cotton rolls and elastoplast and just give them to patients. He said he had a new "special" medical grade material that wouldn't lose its elasticity and was nicely packaged in two three inch rolls for $11.00. You guessed it. Drainage tile. Great idea. I never thought of it. I've been buying it from Home Depot for my patients ever since.

RE: FL Podiatrist Who Took Kickbacks Loses License, but Won't Go to Jail

I recently went to a “private” dinner meeting sponsored by a skin substitute company. At that meeting, they openly flaunted rewards for any pod using their products. I also attended the sponsored lecture they gave the next day to hundreds of pods. In that lecture, they did not mention the rewards program they had. They are tricky bastards. I recognized the scheme right away and stayed away from that company. The company's officers should go to jail. 

I knew that what they were doing seemed illegal and so should most pods, but those freebies sure look good, and docs need to know the difference between right and wrong. I do not agree with the punishment handed down in this case. The APMA should appeal this on behalf of the podiatrist, as this is overkill and not just.
