Pedicures Can be Healthy if Done Properly: IA Podiatrist

A pedicure, or the care of the feet, can be beneficial because keeping nails clean is healthy, said Mason City podiatrist Dr. Steven Brau. “It leads to less problems.”  The only downside is that you run the risk of getting infection or fungus under the nail if the pedicure is not done properly, Brau said. For example, pushing the cuticle too aggressively can open the toes to infection. “Germs are everywhere,” Brau said.

Filing your calluses is fine, Brau said, but products designed to remove calluses should be used carefully or avoided altogether to prevent harmful side-effects. People with diabetes may need to have their pedicures done by a podiatrist to prevent infection. “People with diabetes should use extreme caution,” Brau said.

Fund-Raiser Held for Ailing NY Podiatrist  

Friends and family gathered for a benefit at Barbagallo's restaurant in East Syracuse Sunday for Dr. Suzi Campanaro, a local podiatrist fighting a rare form of kidney and liver disease. Campanaro has been in and out of four different hospitals in the last eight months. A kidney was finally found for her, but her body is currently rejecting it.

Friends at her benefit say the support from the community is much appreciated. "People are coming out of the woodwork who don't even know her, and she's blown away by it. She's receiving letters and checks in the mail from people; she has no idea who they are. And my heart is warmed by this and so is everybody here," said Sue Thomas. The benefit included a dance production, raffles, and food. All of the money raised will help with hospital expenses.

NY Podiatrist Pleads Guilty to Hiding Money in Swiss Account
 
A New York podiatrist pleaded guilty to failing to report his $3.11 million Swiss account at UBS AG (UBSN) to U.S. authorities. Dr. Anton Ginzburg, 51, admitted today in federal court in Brooklyn, New York, to failing to file a Report of Foreign Bank and Financial Accounts form, or FBAR, for 2007. “I knew that I should file this form but I did not do so,” he told U.S. Magistrate Judge Robert M. Levy.
 
Ginzburg’s case is part of a U.S. crackdown on tax evasion.  Zurich-based UBS, Switzerland’s biggest bank, agreed in 2009 to pay $780 million to defer prosecution for aiding tax evasion by U.S. clients. The bank admitted that its Swiss private bankers helped wealthy Americans evade U.S. taxes from 2000 to 2007.

Mobile Sensors Track Health of Elderly in Pilot Study
 
A small pilot study of wireless mobile sensors used by a group of retirees suggests the technology may be useful in detecting meaningful changes in seniors' health, researchers at Dartmouth College report.
 
Eight elderly residents in a continuing care community, whose average age was 85, wore the waist-mounted, two-inch sensors for 10 days while the devices continuously measured such factors as time spent walking, sitting, standing, and speaking with other people. These everyday behaviors often reflect physical and psychological health and potentially predict health problems like depression or dementia, study author Dr. Ethan M. Berke said.

Query: Interaction between NSAID's and SSRI's

I have recently been using the Medscape app on my iTouch to check potential drug interactions. It has been very easy and informative. The one interaction that comes up frequently is between NSAID's and selective serotonin re-uptake inhibitors (SSRIs). Today, I had a patient on Zoloft who was  concerned about using Mobic because of the reported interaction. I know that many patients on SSRI's routinely use both OTC and Rx NSAIDs without difficulty. Is there a real concern here for a "significant interaction"?

RE: Tenosynovitis, Posterior Tibial Tendon (Hartley Miltchin, DPM)

This suggests tibialis posterior dysfunction. As some may know, it has been a subject of special interest to me since publishing on the subject over the last 30 years. It is a complex of symptoms, which is progressive and, often unilateral. Gait analyses (antalgic gait, with medial column collapse, loss of propulsive power, forefoot supinatus at midstance, etc.) and simple testing of the TP muscle/tendon unit's strength (adducting in the transverse plane against resistance) are often more helpful than more expensive diagnostic measures. The insertional fibers of the TP tendon can have direct and indirect effects on the flexor retinaculum, inter-tendinous septae, etc., which, along with flexor substitution from the Achilles tendon, can lead to postero-medial heel pain - thus clouding the diagnostic waters at first.
 
Successful treatment outcomes are predicated upon: 1. understanding the etiology and contributing factors 2. staging/assessing the progress of the disease.
 
In this case, the TP tendon may be in the pre-rupture (Mueller Stage 3) phase, and, therefore, treatable with an orthosis specifically designed for this condition (e.g., the Mueller TPD orthotic), NSAID therapy, modified, rest, etc.
 
Without reviewing diagnostic/therapeutic algorithms, I would refer Dr. Mitchin to my paper "Biomechanical Aspects of Tibialis Posterior Dysfunction" for a thorough review of my experiences with the above, which appeared in JFAS. Also, videos are available from the APMA regarding its diagnosis and treatment, including proper casting techniques for the TPD orthotic, gait analysis, and surgical approaches.

RE: Silastic Implant Complication

From what is reported and without seeing x-rays, I think it is fair to conclude that the implant should come out. In the last posting regarding a failed Keller, I had recommended a fusion. In this particular case, you might consider removing the silastic implant, and using GraftJacket as a spacer to cover over the metatarsal head region which osteolysed. The GraftJacket may help induce osteogenesis (reported at Superbones, Atlantis conference lecture).

I had a similar case where a 1st MPJ hemi implant was seemingly destroying bone. I removed it, and used the GraftJacket to maintain some space, and it worked out nicely. The advantage in this case as opposed to a Keller is that the implant acted as a dynamic spacer and has "stretched out" the first MPJ region and is therefore at risk of getting a cocked-up hallux (Which is a potential complication of a primary Keller.) The key with this case is time - get the patient back in the OR and remove the implant; it will not get better until you remove it.

We are using practicefusion.com for medical scripts. It is free. Is it faster than Surecripts? I don't know. Computer prescription writing by virtue of having to enter data about patients and click on different windows has to be slower than writing a script by hand, but do we have a choice in the matter?
