APMA Scores Aetna Foot Care Advocacy Modifier -59 Win
Aetna has informed APMA that effective July 23, it will drop its external review program regarding use of -59 Modifier when submitted with CPT 11719-11721, G0127, and 11055-11057. This development follows more than two years of APMA’s advocacy through multiple communications and meetings with Aetna leadership.
Aetna had been inappropriately applying an edit to certain foot care claims. APMA’s Health Policy Committee formed a 59 Modifier workgroup more than two years ago to address this issue. Thanks to the work of that group, other APMA members, staff, and consultants all contributed to this latest victory.

FL Podiatrist Discusses Treatment of Corns
Corns are thick layers of skin that can be hard or soft, and are usually seen on the tops and side of the toes. Abraham Wagner, DPM says they develop because of bone pressure against the skin, like a hammertoe. Other causes are arthritis and wearing poorly fitting shoes, or putting excess pressure on certain toes. Though corns can become tender, they’re generally harmless, so most people just let them be, or they use corn-removal pads at home. “These pads should not be on the market,” says Dr. Wagner.

“Ninety-five percent of the patients who come to see me for corn removal have darker skin types and have used these over-the-counter ‘remedies,’ only to find that they burn the skin. Once the skin is burned, it turns white. Because of the contrast on darker skin, the patients think it looks ugly. Plus, the corns eventually come back.” In some cases, Dr. Wagner can surgically excise the corn lesions under local anesthesia and stitch up the skin. “Results can be seen immediately, though there may be a few months of swelling.”

OH Podiatrist Provides Advice on Picking the Right Summer Shoe
“Finding the right summer shoe requires getting fitted – make sure you know your exact shoe size,” explained Joy Rowland, DPM, a podiatrist with Cleveland Clinic. “You also want to make sure the shoe is made up of a supportive material that has an arch support and nice heel counter to hold your foot in place.”

Dr. Rowland said support is key when it comes to picking the right sandal or flip-flop.You shouldn’t wear something overly cushioned, as that could cause you to easily lose your balance. You also want to avoid sandals and flip-flops that can bend and twist easily. Rowland stressed shoes like this can get caught on a raised curb or stone. “Oftentimes, we see patients trip and fall if they aren’t wearing a supportive shoe,” she said. “They can easily sprain their ankle or stub their toe, among other injuries.”

Ted Ary, DPM, Albany podiatrist has written a book, Hellfire, Brimstone and Fried Chicken. Dr. Ary retired two years ago and got bored and decided to write the book last year.

The book is a remembrance, with some humor, about the way it was in the '60s, '70s, and '80s. He writes that the country needs to be rescued by returning God to the classroom and having parents again involved in their children’s education.

Mastering the Science of Patient Financial Engagement
Here are some ways physicians can more effectively encourage patients to pay their bill by leaning into a behavioral science-based approach.
6. Craft the right messaging to spark action. Consumers who miss the payment due date are more likely to take action quickly if the communication states “past due,” behavioral scientists have found. Meanwhile, patients who call with questions about their out-of-pocket cost are highly likely to pay at least a portion of their bill if staff ask for payment once the information is clarified. The key is to make staff comfortable with making the ask, such as by providing scripting that staff can leverage during those encounters.

Query: Laser Treatment Coded as an Office Visit
A Medicare patient follows up for plantar fasciitis. You treat the area with a laser for pain, and the patient reports improvement in the pain. You explain that more laser sessions are needed for treatment. The patient continues to follow up three more times for this treatment. Each time, the only modality you use is laser. The laser is working for the pain, and the patient gets better with each visit. Can you bill an E/M to Medicare for plantar fasciitis each visit, or must you not bill Medicare for this and charge the patient cash instead?

Response: The HCPCS Level II code for laser treatment for plantar fasciitis is S8948 which is defined as the following: Application of a modality (requiring constant provider attendance) to one or more areas; low-level laser; each 15 minutes. This is not a covered service by Medicare. In fact, it is not a covered service by any health insurance carrier. Therefore, when the service is provided to a patient as is described in the above post, it needs to be reimbursed out-of-pocket by the patient.
As far as billing an E/M service to Medicare for each visit that the laser is applied for plantar fasciitis, the simple answer is no. You can NEVER, EVER bill an E/M code in lieu of the appropriate CPT code. Clearly, if the CPT/procedure code is not covered, then the patient pays cash for the procedure that was performed.

The Cryopac systems are unfortunately getting difficult (nearly impossible) to maintain. It seems the weakness is in the internal gas pressure valves of which they have two. I have two of the units and have been seeking repair for nearly a year. You can reach out to KeelerUSA.com for the possibility of repair or parts, but if your model is similar to mine, they are no longer working on them even if they have compatible parts.
They are focused on the ophthalmic industry with new units that are very similar to what we have had available, just much updated. From them, I could buy parts to repair what appears to be the issue, diagnosed by submitting a video to them, but one would need a local repair facility to attempt installation, or if you are adventurous, you can try yourself. The probes time out and they have no replacements nor do they repair them any longer, I have 4 that have not timed out but no functioning console. This information is current as of early June of this year.
