NY Podiatrist Establishes BUSPM Award in Honor of Dr. Andrew Boulton

Scott Kantro, DPM, CEO of Arche Healthcare has pledged $25,000 to Barry University’s School of Podiatric Medicine (BUSPM) to honor the innovative contributions that Dr. Andrew Boulton continues to provide in the field of podiatric medicine. Dr. Boulton has published over 550 peer-reviewed papers and was the first awardee of the International Award in Diabetic Foot Research (1995).

He is currently the President of the International Diabetes Federation and Professor of Medicine (Diabetes) at the University of Manchester and consultant physician at the Manchester Royal Infirmary. The funds will be awarded to extraordinary students who show excellence in matters relating to the diabetic foot.

5 Value-Based Care Strategies for Leveraging Patient Data
 
Health plans and providers remain invested in using data to achieve improved care coordination while lowering costs, tenets of value-based care (VBC). As stakeholders team up to garner deeper, more holistic views of each patient to provide optimal treatment, here are five adoption habits that will guide the essential transition to VBC.
 
3. Integrate data for execution
 
Identifying appropriate data sources and partnerships is critical to VBC success, but it requires the ability to analyze and apply the data for informed decision-making. Inability to exchange data among many healthcare data silos creates numerous challenges and inefficiencies in this regard. Query: Dystrophic and Non-dystrophic
 
I frequently have patients that present with diabetes and request routine foot care. How do you code the situation where the patient meets the criteria for routine foot care and has 5 non-painful, dystrophic toenails but the remaining nails are normal in appearance?
 
I have billed CPT 11720 and CPT 11719 but was only paid $10 for the CPT 11719.

Response: Billing for nail care continues to be a very tricky endeavor as there are many moving parts. To begin with, being diabetic alone is not a criterion for coverage. There has to be vascular and/or neurologic loss.
 
You billed it correctly except that when billing this combination of codes, you must append the -59 modifier to the CPT 11720 code. Lastly, you did not include in your post the ICD-10 codes that you billed. This will obviously impact the billing and eventual claims processing. This could also explain why only one of two codes was paid.

Kudos to Dr. Morgan for his astute comments. I too am retired from clinical practice and agree that a single medical community voice to adjust fees to a cost of living basis is possibly the best way to protect the income earning capacity of current and future practitioners.

The featured article of the day is entitled, “Increasing Your Patient Revenue Pay” and discusses increasing use of cash-based treatment options. Considering a double digit inflation rate which means higher office rents, higher staff salaries and supply costs while third-party reimbursements are in decline, it is not hard to see the day that insurance-based practice will not be feasible. The addition of cash services is something we have done. Additionally, we are considering the feasibility of an all cash-based practice. There are a number of family physicians in our area who have converted to “concierge” practices charging a monthly fee to patients.  

I do not see specialists following that model, but looking back at the history of podiatry practice (yeah, I am now an old timer), office-based services have been our mainstay. Offering office-based services including procedures and some surgery, depending on how our offices are equipped, provides cost-effective services to patients. Augmentation of biomechanics-based services, ESWT, laser treatments, and the like enhances our offerings. The overhead added to our practices by our third-party reimbursement system is considerable. The system appears biased toward invasive procedures and the hospital industry. It would be great to hear from colleagues who have cash-based practices.

RESPONSES/COMMENTS (MEDICARE/MEDICAID NEWS) - PART 1B

Paul Kesselman stated in regard to Medicare cuts, and I’m paraphrasing, “to save costs which benefits the taxpayer.” Medicare is a monster that was created by the government to once again confiscate income and obtain control over the populous. Medicare is funded by withholdings paid into the system by individuals and employers throughout their working lives, not through taxation. As the government typically does, they consider it their money, and in this case, pit patients against doctors by calling it “physician spending.”
 
The sad truth is the government always mismanages the monies they take. There was a study done (I believe I remember the facts correctly) in Galveston, Texas in 1971, where 1,000 citizens were allowed to keep the withholdings and invest them over their working lives. They all realized a 300% gain in invested funds over that of the government. This allowed them to completely fund their medical expenses and use any monies left after death to pass on to family. In case you didn’t know, any funds not “used” at your death, the government keeps.
 
Medicare in the hands of the government was always doomed to fail and they knew that. So here we are with decreased reimbursements, discontinued services, increasing deductibles, and monies taken from other sources such as social security. And there are those still advocating for a single-payer system.

Dr. Jacobs is touching on a gray area when he asks, "Why do dermatologists or podiatric physicians as a profession not employ laser therapy for onychomycosis?" The gray area applies to many products that are not officially endorsed by our profession but the endorsement may be implied. If a young podiatrist or podiatric medical student attends one of our conventions sponsored by our national organization or regional societies and sees booths manned by suave salespeople and even colleagues, what is he or she to think about whether our profession endorses that product or not?
 
The sponsors of our conferences even encourage people to visit the booth areas and even have sponsored speakers tout a product from the podium in the lecture hall.  Perhaps a solution would be to have signs all over the exhibitors' halls stating that the sponsors of the convention do not endorse any of the products and that buyers should use them at their own risk.

RESPONSES/COMMENTS (CODINGLINE CORNER)

I too initially received a letter from Cigna stating they will be auditing all claims with modifier -25. However, yesterday, I received another letter from Cigna stating, “We are currently reevaluating this reimbursement policy change, which will delay implementation.”  “… policy update will not go into effect on August 13, 2022, as originally scheduled” “we will communicate a new implementation date and details after out internal evaluation is complete.” The letter does end with “thank you for the care you provide to our customers.”  Yeah, after a slap to the face last month.
 
I guess enough doctors complained, including me. I guess this small victory, albeit possibly temporary, is a reminder that sometimes, very rarely, providers can beat back the insurance companies. Luckily, Cigna is not a huge payer for me so when I told them I will terminate my contract or start to bring my patients in the next day/week to specifically avoid using a -25 modifier, I wasn’t afraid of the fallout.
