PODIATRISTS AND HUMANITARIAN CAUSES
Podiatry Overseas Association Returns From Africa

Podiatry Overseas Association (PODOA) recently returned from their third week-long medical/surgical mission trip to Togo, Africa. The traveling group was composed of three podiatrists (one attending physician and two resident physicians), one physician’s assistant, and one anesthesiologist.

(L-R) Carlos De la Hoz, MD, Evan Leonard, PA, Mario Cala, DPM, Romain Onteniente, DPM, and Gamal Ayouba, MD

Podiatry Overseas was able to perform 13 surgeries on a variety of lower extremity ailments including residual clubfoot, drop foot, and post-traumatic deformities. PODOA gave two separate lectures to the orthopedic and traumatology residents and professors from the University of Lome’ Hospital. Lecture topics included ‘Correction of Residual Clubfoot Deformity’ and ‘Ankle Fusion, Indication & Techniques’, with over 20 physician attendees. Each lecture incorporated a live case presentation with hands-on training in the operating room for the surgeons of Togo.

ICD-10 UPDATE
New ICD-10 bill in House Would Permit Dual Coding for 180 Days

Legislation proposed in the House would permit providers to code in either ICD-9 or ICD-10 for half a year after the ICD-10 implementation deadline, now set for Oct. 1. The bill, Coding Flexibility in Healthcare Act of 2015 introduced by Rep. Marsha Blackburn (R-TN) and co-sponsored by fellow Republican Rep. Tom Price (R-GA), a physician, would also require HHS to report to Congress 90 days after implementation of ICD-10 on the impacts the new codes are having on providers, patients, and other stakeholders.

The bill has already garnered support from the Medical Group Management Association, a professional association for physician group practice leaders and managers. In a letter to Blackburn and Price, Dr. Halee Fischer-Wright, MGMA president and CEO, said the bill would afford physicians “much needed flexibility and provide a window of time to address inevitable system issues to ensure claims are processed and paid.”

RESPONSES/COMMENTS (CLINICAL)
From: Art Hatfield, DPM, Richard A. Simmons, DPM

When my kids were small (30-40 years ago) and sucked their thumbs, we used to put a product call THUM on their thumbs to stop them.

Art Hatfield, DPM, Long Beach, CA

Editor's Note: A newer product called Mavala Stop is highly rated by Amazon reviewers.

The child needs to be evaluated by a pediatric psychologist. This is a typical Munchausen syndrome (a psychiatric factitious disorder wherein those affected feign disease, illness, or psychological trauma to draw attention, sympathy, or reassurance to themselves.)

RESPONSES/COMMENTS (ICD-10) - PART 1A
From: APMA Coding Committee

APMA has reached out to CMS for additional clarification about CMS’ recently announced “flexibility” with regards to ICD-10 coding and claims acceptance following the October 1 transition. Specifically, APMA is seeking a better understanding of what CMS means by “valid code from the right family.”  We will post the CMS clarification when it is received.

It should be noted, however, that CMS has emphasized that ICD-9 codes will NOT be accepted for claims with dates of service after September 30, 2015. APMA will provide updates to the podiatric community as more information becomes available.

RESPONSES/COMMENTS (ICD-10) -PART 1B
From: Josh White, DPM, CPed

It was good news that CMS recently announced that for the first year that ICD-10 is in place, Medicare claims will not be denied solely based on the specificity of the diagnosis codes as long as they are from the appropriate family of ICD-10 codes.

This means that Medicare will not deny payment for these unintentional errors as practices become accustomed to ICD-10 coding. In addition, Medicare claims will not be audited based on the specificity of the diagnosis codes as long as they are from...

RESPONSES/COMMENTS (PRACTICE MANAGEMENT) - PART 1
RE: Problems with No-Show First Appointments (Joshua Kaye, DPM)
From: Hartley Miltchin, DPM

Over the years, we have found a number of new patient no-shows, even though they are telephone verified 48 hours prior to their scheduled appointment. Nothing is as frustrating as booking out an hour for a new patient and they are disrespectful enough to not show up, without even as much as a phone call.

Approximately 3 years ago, our office began asking for credit cards from new patients in order to schedule an appointment. The patient is told that their card will not be billed prior to their appointment and we do not keep credit card information on the computer. They are informed that if they fail to cancel their appointment 48 hours prior, their credit card will be billed 50% of the consultation fee.

Since implementing this system, the number of new patient no-shows has dropped to virtually zero. Most patients are very understanding as to why we collect credit card information and there is very little hesitation. We have only billed 2 new patients in 3 years for a no-show. Neither of them called to dispute the charges.

RESPONSES/COMMENTS (EMR)
RE: Recommendation for EHR System (Steven King, DPM)
From: Matthew Werd, DPM

On January 1st, we made the commitment and transitioned from TRAKNet 2.0 to TRAKnet 3.0 and have been very pleased. Aside from being podiatry-specific, the software allows customizations that really help us to be more efficient. It is also for ICD-10 ready with its powerful ICD-10 Smartsheet technology.

Disclosure: I have no conflicts of interest with TRAKNet.
