This Council for Podiatry Medical Education accredited program represents Village Health's commitment to fostering medical education and training and cultivating the next generation of professionals in medicine and podiatry.

MA Podiatrist Discusses Benefits of Zero Drop Shoes
 
“Zero drop shoes are shoes that have the same sole thickness from the heel to the toe, and therefore have no elevation,” explains Alissa Kuizinas, DPM, a podiatrist and ankle wellness specialist. Indeed, if you held a protractor against the shoe, you’d see that there is no angle at all between either end of the shoe.

“The benefit of zero-drop shoes compared to shoes with a heel drop is that they allow your foot to function and move from its natural position— which means you’re walking or moving as efficiently as possible,” explains Dr. Kuizinas. Zero drop shoes also spread your weight evenly across your heel and forefoot, rather than having increased pressure along the ball of your foot, she says.

PRACTICE MANAGEMENT TIP OF THE DAY
Mastering the Science of Patient Financial Engagement
 
Here are some ways physicians can more effectively encourage patients to pay their bill by leaning into a behavioral science-based approach. 
 
4. Give digital communications time to breathe. For instance, when sending a text-based reminder for payment, some physician groups choose to wait seven to 10 days to send an electronic reminder to avoid the risk that digital fatigue will prompt consumers to ignore the message. If payment is not made at this point, some will only then send a paper-based statement based on some patients preferring a paper statement; they will then choose to send an additional electronic reminder to arrive around the same time as the paper statement. The cadence of both electronic and paper statements sometimes must be adjusted on the comfort level of the physician group and the community’s response to a digital-first approach so plan to evaluate results early and often.

How do you code for a fracture of the tibial sesamoid? Is it any different if you considered it to be a symptomatic, bipartite tibial sesamoid? What about an avascular necrosis of the tibial sesamoid?

Response: A. ICD-10 code for a fracture of the tibial sesamoid? ANSWER: If fractured, the recommendation is to use either S92.811A Other fracture of right foot and/or S92.812A Other fracture of left foot.
 
B. ICD-10 coding for symptomatic, bipartite tibial sesamoid? ANSWER: There is technically no such diagnosis of a bipartite tibial sesamoid but I recommend if you feel it is acquired, then M21.6X1 Other acquired deformities of right foot and/or M21.6X2 Other acquired deformities of left foot. If you feel it is congenital, then code as Q66.89 Other specified congenital deformities of feet. If there is pain, then you would use M79.671 Pain in right foot and/or M79.672 Pain in left foot OR M25.571 Pain in right ankle and joints of right foot and/or M25.572 Pain in left ankle and joints of left foot.
 
C. ICD-10 coding for avascular necrosis of the tibial sesamoid? ANSWER: There is technically no such thing as avascular necrosis of a tibial sesamoid but your options are: M87.274 osteonecrosis due to previous trauma, right foot and/or M87.275 osteonecrosis due to previous trauma, left foot OR M87.874 Other osteonecrosis, right foot and/or M87.875 Other osteonecrosis, left foot
ICD-10 can be challenging and I always recommend using the ICD-10 verbiage primarily over podiatric terminology.

RE: The Medicare Orthotics and Prosthetics Patient-Centered Care Act

An important piece of legislation titled the Medicare Orthotics and Prosthetics Patient-Centered Care Act has been recently introduced to the U.S. House of Representatives. The three major provisions of this legislation would: prohibit “drop shipping” of custom orthoses and prostheses to Medicare beneficiaries; ensure Medicare beneficiaries can access the full range of orthotic care from one O&P practitioner rather than requiring patients to visit multiple providers when the treating orthotist or prosthetist does not have a competitive bidding contract and; ensure Medicare beneficiaries can access replacement custom-fitted and custom-fabricated orthoses when a change in their condition or clinical needs occurs.

I hope that this pending legislation will also be added to the list of current advocacy campaigns for APMA as the issue of "same or similar" has become a significant challenge for podiatric physicians and their patients.

I read with great interest: “Substance Abuse is Climbing Among Seniors” written by Paula Span of the New York Times on July 9, 2023 and reported by national news outlets the morning of July 10, 2023. Data is presented from experts in the fields of gerontology, sociology, psychology, and addiction medicine.
 
A number of salient points are offered by this narrative that include: "Despite alcohol playing a major role in elderly substance use disorder most substance use disorders among older people involve prescribed medications, not illegal drugs, 8.6 percent of substance use disorders involved opioids, mostly prescription pain relievers; 4.3 percent involved marijuana; and 2 percent involved...

I totally agree with everything Dr. Zicherman has concisely written. To understand Medicare's reasoning is as simple as Googling "The goose and the golden eggs". Sadly, it tells a story of a countryman who possessed a wonderful goose that laid golden eggs. Every day he would visit the nest to find one golden egg which he would take into town to sell. He began to get rich but as greed took over, he wasn't getting rich fast enough. He decided to cut open the goose to remove all of the golden eggs, obviously killing the goose.
 
While in podiatry school in the early ‘80s, I shadowed a podiatrist during his visit to a nursing home. He must have seen around 30 patients that day. As he would see a patient, he would utter the phrase "bilateral bilateral" to his assistant as he debrided the patient's...
