When you’re looking for a supportive sneaker, podiatrists typically recommend finding what works best for your foot type, says Bruce Pinker, DPM, a board-certified podiatrist at Progressive Foot Care, and an associate of the American College of Foot & Ankle Surgeons. “For example, if one has a low arch, generally, they should be looking for a sneaker with added arch support, an anti-pronator sneaker,” Dr. Pinker says.

NY Podiatrist Recommends Different Shoes for Walking and Running
Are running shoes good for walking? “Both running and walking are forward motions,” says NYC-based podiatrist Jackie Sutera, DPM. “For both activities you need cushion and shock absorption in the shoes.” With similar movements, improper footwear in either activity, Dr. Sutera reminds us, can eventually lead to similar foot problems — sprains, strains, stress fractures, tendonitis, and plantar fasciitis.

“Walking versus running have different gaits. Therefore, wear patterns will form on your sneakers at very different rates. So it’s really best not to use the same pair of shoes for the two different activities,” according to Dr. Sutera. “In my opinion, having two separate pairs — one dedicated to running and one to walking — is a better idea,” she says.

CMS Pitches Physician Payment Decrease for 2023
CMS released its annual proposed changes to the physician fee schedule for 2023 on July 7, which includes a proposed $1.53 conversion factor decrease. The proposed physician fee schedule conversion factor for 2023 is $33.08, down from $34.61 in 2022.
The proposal considers a statutory requirement that the conversion factor for 2023 remain flat as well, due to the expiration of the 3 percent increase in physician fee schedule reimbursement payments in 2022 that was required in the Protecting Medicare and American Farmers From Sequester Cuts Act. The 60-day comment period closes on Sept. 6.

5 Value-Based Care Strategies for Leveraging Patient Data
Health plans and providers remain invested in using data to achieve improved care coordination while lowering costs, tenets of value-based care (VBC). As stakeholders team up to garner deeper, more holistic views of each patient to provide optimal treatment, here are five adoption habits that will guide the essential transition to VBC.
2. Consider all the data.
While clinical and claims data are essential for healthcare decision-making, providers are also relying more on social determinants of health (SDOH), the non-medical behaviors, circumstances, and demographic factors that influence a patient’s outcomes. After the Centers for Medicare & Medicaid Services (CMS) issued guidance designed to drive adoption of strategies that address health disparities, clinicians have begun to use SDOH to further guide treatment decisions. CMS made this decision, in part, to advance VBC. Partly as a result, clinicians now are using data about factors such as environmental challenges, likelihood of adherence, motivational factors, and leveraging data to help stratify patients and tailor interventions.

Study Discusses Role of Shortened Proximal Phalanx
The shortened proximal phalanx in brachymetatarsia is 5mm shorter and influences the shortened clinical appearance of the toe, a new study suggests. The researchers conducted a retrospective study to evaluate the length of the proximal phalanx in the shortened ray. Overall, 97 feet with congenital brachymetatarsia were included in the study. The 66 patients underwent surgery to bring the metatarsal to the desired length of correction between January 2005 and February 2020, all at the same institution.
The study authors included Michael A. Thomas, DPM, Noman A. Siddiqui, DPM, MHA, Kelsey J. Millonig, DPM, MPH, Yumna Siddiqui, DPM, Cassandre Charles, DPM, and Bilal M. Khan, DPM, all of the International Center for Limb Lengthening, Rubin Institute for Advanced Orthopedics, Sinai Hospital of Baltimore.

I see multiple sports injuries a day including stress fractures. I feel that we have not really addressed the true issue [which] is that most of athletic shoes, including 98% of sports cleats are poorly constructed. I look for 3 basic tenets in shoes: 1: Stiff heel counter. 2: Stiff shank in the shoe which makes the shoe bend like a foot (novel idea). And the third is a wide enough toe box in width and depth. None of these issues address pronation. The only shoes that do this are a few good running shoes. So even the best cleat/ tennis/ basketball shoe does not address pronation and therefore needs a good orthotic.
I see a lot of pediatric stress fractures and it is often a mixed etiology of bad shoes/over-training and not enough Vitamin D / calcium. These kids get branded at a young age by...

The medical profession is under attack again by CMS. The only way out is to change Medicare reimbursement from budget neutral to cost of living! This needs to be the number one AMA, AOA, and APMA directive for all legal battles. When will medicine wake up and say we can’t afford to be viable at this rate or budget neutral position? We should all speak with ONE VOICE and make sure that all parties understand the disaster ahead in Medicine if we don’t stand up and fight now.
This constant attack on fees for doctors is not sustainable. I’m retired from clinical practice but believe, as a recipient of Medicare, that this will harm the public without any fight for the rights for doctors. Every area of practice has become more expensive but reimbursements are constantly being attacked and lowered! I’d like to hear the thoughts of those podiatrists in elected positions.

To reduce overall costs of care and provide useful programs to help patients, a provider’s clinical data and a payer’s claims data must be leveraged for a complete understanding of high-risk patients and their needs. By leveraging this rich data set, health systems can ensure patients are able to take advantage of member programs that best suit their needs. But since it can be costly to gather, normalize and act on data, providers must remember both to seek guidance of experts and use caution when entering into risk-sharing arrangements with health plans. If these arrangements are not run efficiently, it could lead to potential losses due to downside risk.

In addition to the payment decrease for 2023, CMS is proposing a disastrous revamping of the reimbursement for cellular tissue products in 2024. CMS' stated intent is to discontinue the use of "Q" codes for all CTP and reinvent them as "A" codes. These "A" codes would then be considered incidental to the provision of the appropriate CPT code.
The complexity and unconscionable absurdness of this proposal is beyond anything I have seen in the forty plus years I have been following CMS policy. This policy will cause undue harm to patients with...

RE: Insurance Companies and e-Prescribing

I sent in an Rx for clotrimazole and ciclopirox solution for tinea pedis and onychomycosis respectively. I got a call from the pharmacy telling me those two were not covered by the patient’s insurance and the patient asked they be substituted. I asked the pharmacist if they could give me a clue as to what is covered. They replied, “I am sorry, I cannot. You just have to keep sending.” How stupid is that?
With all the sophistication of technology and computers, is this where we are? I pay an EMR and have E-prescribing capabilities, but the computer (or possibly I) doesn't know the insurance formularies. If I am correct in what I am saying, this is ridiculous. Shouldn’t insurance companies have to play their role in EMR technology and integrate patient formularies with EMRs the same way doctors are forced to comply with MIPS and MACRA crap?

Query: Issues with the Radiology Department
My clinic is at the local hospital. I send x-rays to the radiology department. They are eventually read by a radiologist. However, I actually evaluate the x-rays and interpret them myself. Can I bill that component of the radiology fee? And if I do, will it affect radiologist reimbursement?

Response: Unfortunately, NO. Most insurances will pay for one reading and that goes to the hospital radiologist.
The next part is what you are going to do with the x-ray findings. If you are simply confirming that there is a fracture and it is going to be cast, then there is no additional reimbursement. If, on the other hand, you review the x-ray and need it to plan out a surgical approach, then that would be payable as part of the E/M service (data review component). This is a very tricky situation as you could make an argument that you need the x-ray to plot out any treatment. You should document why you need to review the x-ray. Keep in mind you only need two of three components to determine your E/M level, so a third data review scenario could be a moot point.

I charted, “Take resident to hospital emergency ASAP, STAT!” This would involve 1-2 staff members to escort the resident to the hospital. Rather than have the resident transported to E.R., the MD Rx’d 2 different oral antibiotics. Unfortunately. my advice was not followed. After 2 weeks, as the infection continued to spread, palliative care was provided. The resident passed away within 6 weeks.

As much as I respect legitimate practice management, the article by Mr. Horr borders on questionable care recommendations. With regard to onychomycosis, topical therapies dispensed in the office have little proven efficacy as isolated treatment. Systemic therapy, perhaps supplemented by topical therapy and concurrent treatment of tinea pedis, may be appropriate in some cases. However, laser treatment does not provide a cure for onychomycosis. Nor do office dispensed topical antifungal therapies as isolated treatment for advanced disease. Where is the literature to support these recommendations? Why do dermatologists or podiatric physicians as a profession not employ laser therapy for onychomycosis?
Similarly, the suggestion that a “subtalar joint stent "be utilized for the management of plantar fasciitis. Generally, with few exceptions, subtalar joint arthroereisis is not a covered service. The procedure is frequently and fraudulently billed as “open reduction and internal fixation of a peritalar dislocation." Worse yet, "modified subtalar joint arthrodesis." If one looks at the recommendations for the treatment of plantar fasciitis by ACFAS or AOFAS, where are the recommendations for pronation limiting surgery or the injection of agents other that corticosteroids?
Of course, there are occasional articles of variable quality published which support almost anything. However, if these therapies were indeed predictably helpful, why are they not standard of care? It is reasonable to posture that our therapies should not be restricted to those paid for by a third party. However, non-covered services endorsed to a patient should have a reasonable basis upon which that recommendation is made.
