PODIATRISTS AND POLITICS
CT Podiatrist Helps to Get Co-Sponsors for the HELLPP Act

Dr. Odin De Los Reyes plays a national role on the podiatry scene as the American Podiatric Medical Association PAC coordinator. In this role, Dr. De Los Reyes works to advance legislation that is in the interest of the podiatrists or their patients. This means working to “bend the ear” of legislators and trying to advance their interests, he said. “Part of what we do is every year we have to go down [to Washington, D.C] and we have to speak with each of our congresspeople and two senators,” he said, which means seven people in two days for Dr. De Los Reyes and another podiatrist.

Dr. Odin De Los Reyes

His biggest accomplishment in this role, which he will retain and he said he enjoys, is his work with the HELLPP Act, the Help Ensure Life- and Limb-Saving Life Access to Podiatric Physicians Act. He said, all five of the state House representatives have signed on to the bill. “When I became coordinator in 2010, we had nobody from our state who co-sponsored that bill,” he said. “The bill has not passed, it has to be constantly brought up, every two years. But it’s important to get co-sponsors because the more co-sponsors you have, the more chance it has to go to the floor, and then it can be voted on.”

PODIATRISTS IN THE NEWS
SD Podiatrist Discusses Blister Prevention

Keeping your feet dry and wearing properly fitting shoes can protect you against developing blisters, said podiatrist Dr. Kent Renaud, of Rapid City. “I see people who often make the mistake of wearing a shoe that’s too big,” Renaud said. “The tendency is to get a shoe that’s not going to rub, and everybody knows that tight shoes are an issue and can cause blisters. But also a shoe that’s too large is almost worse because if it flips around and ends up with friction, it ultimately can end up with hot spots and then blisters,” he said, adding that the other factor that contributes to blisters is moisture.
Dr. Kent Renaud

Cotton socks tend to hold moisture, which puts you at a higher risk for blisters. “I like to go with some of the man-made materials - Polypro, Lycra, and even the smart wool-type stuff is a good thing to consider just because it tends to wick moisture away from the skin and helps keep the skin a little bit dryer,” Renaud said.

PODIATRISTS OVERCOMING ADVERSITY
After Life-saving Transplant, VA Podiatrist Takes Cal Ripken’s Seats

Dr. Mike Fine liked to fashion himself as “the Cal Ripken Jr. of podiatry”. For years, Fine, named one of the top podiatrists by Washingtonian Magazine in 2012, came into his Arlington office every single day — regardless of whether he had a cold or a cough or wasn’t feeling well — to make sure he could attend to the nearly 40 patients he saw each day. This label especially started to stick in 2014 when Fine was diagnosed with non-alcoholic fatty liver disease. He stuck to his routine — until his health deteriorated to the point that he was in need of a transplant. The situation became dire. As Fine sat on the transplant list for months and months, waiting for a glimmer of hope, it all began to wear down on him.

His daughter, Lisa Fine, noticed her father beginning to slip away. She came up with a plan, one she thought was a long shot; she contacted Ripken, hoping the former shortstop could send some words of encouragement to her father. And on Father’s Day weekend 2014, a video landed in Fine’s email inbox. “I’m thinking about you,” Ripken said in the video. “I know you’re going through a tough time. Maybe someday we (can) go to a game together. We (can) sit in my front-row seats and watch a game together,” Ripken said. The video gave Fine strength. Nearly a year later, in May 2015, Fine finally got a donor to give him a new liver and kidneys.

PODIATRISTS AND DIABETES

AZ Podiatrist Saves Foot from Being Amputated

After an unbearable struggle for nearly a year with a wound that would not heal, Los Alamos resident Tom McHenry lost all hope and asked doctors to amputate his foot. “It was so hard to see him unable to walk or get around, she said, and he finally asked doctors to remove his foot.” Rather than oblige McHenry’s desperate request, the doctors treating him in Santa Fe referred him to the Southern Arizona Limb Salvage Alliance (SALSA), in Tucson … and that’s where he got his miracle. It took the doctor examining McHenry’s foot at SALSA just 10 minutes to come up with a game plan to fix it, he said.

Dr. David Armstrong

“They took fat tissue from my abdomen and processed it a specific way,” McHenry said. “It became the consistency of a paste and the doctor said, ‘I’m spackling tissue onto the bottom of your foot and I’m going to do it in three layers.’ He did it that way so I would have padding again.” SALSA Director Dr. David Armstrong said he has treated patients who have suffered with a foot wound for up to thirty years and has been able to fix it for them.

MEDICARE NEWS
CMS Issues Rule to Expand Access to Medicare Data

The Centers for Medicare and Medicaid Services has released final rules covering increased access to analyses and data that will help providers, employers, and others make more informed decisions about care delivery and quality improvement. To date, 15 organizations have applied for and received approval to be a qualified entity; of those, two have completed public reporting, while the other 13 are preparing for public reporting.

The new rules, required as a provision of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), are meant to augment the Qualified Entity Program, authorized by the Affordable Care Act, which lets qualified entities confidentially share or sell analyses of Medicare and private sector claims data to providers, employers, and other groups that can use the data to support improved care.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Elements for an Effective Healthcare Compliance Program - Part 2

With fraud enforcement initiatives increasing, medical practices that have not implemented compliance plans risk exclusion from commercial insurance plans and government healthcare programs. According to CMS, the implementation of a good compliance program will aid in better protecting a practice from the risk of improper conduct. The ACA has seven core elements for an effective compliance program:

4. Commitment to conducting formal education and training programs for all levels of employees.

5. Ongoing auditing and monitoring of systems to assess the effectiveness of the compliance program and identify issues.

6. Development of policies to enforce standards of conduct with disciplinary measures for employees who fail to comply with requirements.

7. When vulnerabilities are identified, corrective action must be conducted in response to potential violations.

CODINGLINE CORNER
Query: Injection for a Hypertrophic Scar

We have a patient who received an injection for a hypertrophic scar (L90.1). What is the correct procedure code for this diagnosis? We tried CPT 20550, but the diagnosis was inconsistent with the procedure.

Linda, Office of Lewis Giglia, DPM, Fairport, NY

Response: I suggest CPT 11900 (injection, intralesional; up to and including seven lesions). I also suggest you consider ICD-10 L91.0 which is hypertrophic scar. Your listed code, L90.1, is anetoderma of Schweninger-Buzzi.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Kevin Kirby, DPM

The Hoka One One (Hoka) running shoe is a “maximalist running shoe”. The thicker midsole of the Hoka maximalist running shoes offers more shock absorbency to the runner, which gives maximalist shoes a more “cushioned” or “springy” feel. However, the thicker midsoles of maximalist shoes also create many potential problems for the shoe, including increased shoe mass, decreased frontal plane rearfoot stability and increased stiffness in the forefoot portion of the shoe sole (i.e. decreased forefoot sole flexibility).

Hoka One Clifton

Specifically, in many Hoka maximalist running shoes, shoe mass has been kept to a...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Robert Scott Steinberg, DPM

I have to call Dr. Fullem, on the appearance of bias, when it comes to him recommending Hoka shoes. I have found them very unstable. They do have the unwanted tendency of excessively straining the calf and the Achilles. Because they are unstable, there is unnecessary overuse of certain muscles. Is there anything worse than getting or treating Achilles tendinitis if you are a runner? His use of the word "old", in an attempt to cast doubt on the proven protective properties of the classic 10-12mm drop, stability running shoe, further exposes his bias. Once runners get past the gimmicky nature of fad running shoes, they see the foolishness of some trends.
