According to Conway McLean, DPM, "The latest evidence appears to point rather clearly to the possibility of curing yourself of pre-diabetes. Is it easy or common? No. It means significant lifestyle changes and they are hard to make (or keep). Radically altering your diet is necessary for many of these people since it is primarily their unhealthy dietary regimen which led to the disease."

OH Podiatrist Tells How to Avoid Foot Injuries When Running Long Distances
 
Whether you’re gearing up for a marathon or just want to run longer distances, there are some things you can do to avoid foot injuries. Joy Rowland, DPM, a podiatrist with Cleveland Clinic said starting with the right running shoe is key. “The shoe should be cushioned. It should have a heel flare to absorb shock in the heel area, and it should have some cushion by the metatarsal,” Dr. Rowland said. “You want to make sure that the toe box area is big enough, so your foot has a little bit of room. You don’t want your toes hitting into the shoe.” Dr. Rowland said finding a couple of good running shoes to rotate between is important.

Well before your run, Dr. Rowland said it’s crucial to build up your foot strength by doing daily exercises and stretches. For example, you can flex your foot up and down to resistance – or try toe presses where you push your big toe to the ground while lifting your other toes up. “Not only do foot strengthening and stability exercises help you as you prepare for your run, but they also help you while you’re running,” Dr. Rowland said. “The better prepared your feet are, the more you reduce your risk of injury and long-term problems like stress fractures.”

Here are some ways physicians can more effectively encourage patients to pay their bill by leaning into a behavioral science-based approach. 
 
3. Avoid digital fatigue and decision fatigue by establishing the right cadence for messaging. Studies show rates of decision fatigue—the exhaustion that comes with making decisions over and over—increases when consumers grapple with financial challenges. In fact, 71% of consumers say they have ignored emails and messages from service providers due to economic and social pressures. Digital fatigue, which comes from spending too much time looking at screens, affects 64% of consumers. For physician groups, this makes establishing the right cadence for financial communications vital to securing engagement and protecting patient satisfaction.

Query: Non-union of a Lapidus Bunionectomy
 
I performed a Lapidus bunionectomy that went on to a subsequent non-union of the arthrodesis site. We are planning a return to the operating room to revise it. Any suggestions on the best ICD-10 code and best CPT code to use in this situation?

Response: The technical terminology in ICD-10-CM belongs to the code M96.0 Pseudarthrosis after fusion or arthrodesis. The only time you can use a “non-union” is when it is associated with a fracture, which your scenario is not. Depending on the complication with the hardware, I suggest one of these as the top 4 choices:
 
1) T84.126S Displacement of internal fixation device of bone of right lower leg OR T84.127S Displacement of internal fixation device of bone of left lower leg
2) T84.213S Breakdown (mechanical) of internal fixation device of bones of foot and toes
3) T84.223S Displacement of internal fixation device of bones of foot and toes
4) T84.293S Other mechanical complication of internal fixation device of bones of foot and toes
If these do not match your circumstance, look at the entire T84 series.
 
CPT Coding: If you revise an arthrodesis site, then typically you will use the same CPT code but in this case, you are not re-doing the bunionectomy, only the arthrodesis; so select CPT 28740 Arthrodesis, midtarsal or tarsometatarsal, single joint. If you are within the global period, then you append modifier -78 - Unplanned Return to the Operating/Procedure Room by the Same Physician Following Initial Procedure for a Related Procedure During the Post-operative Period. You would not use modifier -76, as you are not repeating the whole procedure again, only a part of it. If outside the global period, use only the RT or LT anatomic modifier.

I received an Episource request for 66 charts. I charged $50 per chart. I told the representative I would forward the chart notes when the check for $3,300 was received. I refused to negotiate and received the check a week later. I will now charge $100 per chart, hoping Episource or Ciox will refuse payment so the chore will not take office staff away from their regular duties. Please note this chart request was not an audit.

Dr. Zicherman has voiced concerns pertaining to the Medicare billing of CPT codes for toenail avulsion. ThIs link contains current national guidelines that explain the current utilization guidelines for the avulsion codes. There may be some variations among the individual MACs. 
 
I was one of several podiatry Medicare HCFA Contract Advisory Committee (CAC) members who received the first publishing of the revised original nail avulsion guidelines. According to what we were told, these guidelines were mandated by the Medicare Office of the Inspector General, not by HCFA. The reason given for the change was to control "aberrant billing."

Thank you Dr. Zicherman for writing that detailed analysis of nail surgery. Medicare’s constant changing of the regulation was to stop fraud and lower payments for this procedure. One can argue all day about 8mm.vs.9mm. but this is what it is really about. Medicare guidelines changed it up with the words, ”local anesthesia.”
 
This simply means the use of an injection to the toe. When some podiatrist makes the fraud headlines, most of these cases are about nail surgery. Around 1976, Dr. Arthur Helfand mentioned to me that one day this billing through Medicare would become problematic.
