PA Podiatrist Provides Summer Foot Care Tips
 
Summer is the season for letting loose, flip-flops and walking in the grass or sand. But there are some things you need to do to keep your feet healthy and looking their best. While it may be tempting, Dr. Michelle McCarroll, podiatrist and owner of That Foot Doctor in Allentown recommends against walking barefoot because of the risks of stepping on something like glass or a wood splinter and injuring your feet.

“Going barefoot also means an increased risk of getting athlete’s foot or plantar warts, which are acquired by direct contact with the skin,” she says. “Wear properly fitted shoes or Vionic sandals to help avoid heel and arch pain.” Make sure to thoroughly clean and dry between toes, too, to avoid athlete’s foot fungus, which can spread from the skin into the nails and lead to fungal toenails.

For 20 Years KS Podiatrist Operates Weekly Clinic at NMRMC
 
Inspired by Dr. Harold Sauder, a medical partner who is in his 90s and still seeing patients, a foot doctor in Chanute said he intends to practice as long as he possibly can. Dr. Barry Wesselowski of Independence, known as "Dr. Wess", has provided an outpatient podiatry clinic at Neosho Memorial Regional Medical Center (NMRMC) for more than 20 years, and said he has no plans to retire.

Wesselowski came to Independence in 1994 and joined Dr. Sauter. They realized a lot of their patients came from Chanute and many elderly patients had difficulty making the trip. In 1995, he began to offer the Chanute clinic. At different times, he has added a day to the Chanute schedule. For 17 years, Dr. Jeffery Hogge has also been a partner in the practice and provides clinics in Parsons and Neodesha.

CMS’ Open Payments Program Posts 2015 Financial Data
 
The Centers for Medicare & Medicaid Services (CMS) has published the 2015 Open Payments (sometimes called the “Sunshine Act”) data, along with newly submitted and updated payment records for 2013 and 2014, at https://openpaymentsdata.cms.gov. The Open Payments program requires that transfers of value by drug, device, biological, and medical supply manufacturers to physicians and teaching hospitals be published on a public website.
 
In program year 2015, healthcare industry manufacturers reported $7.52 billion in payments and ownership and investment interests to physicians and teaching hospitals. This amount is comprised of 11.90 million total records attributable to 618,931 physicians and 1,116 teaching hospitals. To find out what any doctor received in 2015, click here.

MEDICARE NEWS
CMS Proposes Expanding Diabetes-Prevention Model
 
In addition to a slew of changes to Medicare's physician payment policies, the CMS on Thursday proposed expanding a program aimed at helping people avoid diabetes. The CMS suggests starting the program in 2018 and is seeking comment whether to launch the effort nationally or in additional select markets.
 
The program began in 2013 and enrolled beneficiaries in eight states: Arizona, Delaware, Florida, Indiana, Minnesota, New York, Ohio, and Texas. It is the first from the CMS Innovation Center, which was created by the Affordable Care Act, to be proven successful enough to be elevated from a demonstration and rolled out to the full Medicare program. The ACA allows the CMS to expand programs that prove effective without the approval of Congress.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Elements for an Effective Healthcare Compliance Program - Part 1
 
With fraud enforcement initiatives increasing, medical practices that have not implemented compliance plans risk exclusion from commercial insurance plans and government healthcare programs. According to CMS, the implementation of a good compliance program will aid in better protecting a practice from the risk of improper conduct. The ACA has seven core elements for an effective compliance program:
 
1. Establishment and adoption of written policies and procedures to promote the organization’s commitment to compliance.
 
2. Identification and appointment within the organization of an individual to serve as compliance officer, who will be responsible for monitoring compliance efforts and enforcing practice standards.
 
3. Establishment of reporting systems to encourage individuals to make complaints regarding compliance items without fear of retaliation.

Hoka has made the largest increase in sales in the specialty shoe market several years in a row. Anyone interested in keeping up with the latest running shoe information should join AAPSM. Members currently enjoy a huge discount on Hoka, Altra, and 361 shoes.
 
Hoka has a rocker sole in addition to a 4-6mm heel drop and an extra amount of midsole material known as stack height. They are not only light, but you will not feel that you are elevated at all. I think this shoe is an excellent choice for plantar fasciitis patients as well as those with arthritic knee and hip joints. I have run in 4 different Hoka models over the last 4-5 years and love the shoes.
 
Altra is another newer shoe that all podiatrists should be aware of due to a wider toe box that mimics the shape of the foot, an excellent choice for patients with neuromas and HAV deformities. Altra features a zero drop as well. There is no difference between rearfoot and forefoot as opposed to the old standard where most shoes were 12mm; therefore, it is important to warn the patients to gradually break them in or they may suffer some Achilles or calf issues.

I am a retired podiatrist who is now a personal trainer/TRX instructor. I had a severe forefoot injury this past winter that required a walking boot for 6 weeks, and I still had significant residual pain. I found Hoka One Shoes and they were like a magic pill. Within about 10 days of use, my forefoot pain had significantly reduced to the point that I could start running again. I resumed my full schedule of teaching and my personal exercise schedule.  
 
The Bondi model is one of the max cushioned models I use. Hoka One also has models for trail running, spikes, and hiking shoes. The main drawback of the Bondi model is that there is very poor lateral stability due to the very thick sole. I do not recommend it for any sport that involves a cutting movement or unstable surfaces. I love these shoes and the fact that I can now run without foot or knee pain.

After Kevin Kirby, DPM reviewed Hoka shoes a few years ago and discussed this new concept of maximalist shoes, I bought four pairs. The superior shock absorption helps my low back significantly while walking 4 miles in an hour.  At $130 each, not all patients can afford them. I don't like that the Clifton II (which I wear) only comes in a D width. I had to go up one size to a 9D, but this has worked for me with thick moisture wicking socks. The Clifton is a more flexible shoe.  
 
For plantar fasciitis, I recommend that patients pick a model with a rigid forefoot and shank (as just mentioned in PM's article by Dr. Kor). They have worked out well for OR techs, postal employees, and others with standing jobs. I am not a runner, so I can't comment. With many other companies now jumping on the maximalist bandwagon, perhaps Dr. Kirby can comment. If you Google "maximalist shoes", you'll see there are a lot of other brands.
