NY Podiatrist Discusses How to Avoid DVTs During Flights

There is a dangerous drawback of traveling that you may not be aware of: Being immobile for a long time, such as during a flight, can increase your risk of developing a life-threatening blood clot called deep vein thrombosis. Although DVT is usually easily treatable when caught quickly, the condition can become more serious if a piece of the blood clot breaks off and moves through the bloodstream to your lungs and causes a blockage to an artery there, called a pulmonary embolism. "In severe cases, this can lead to a stroke and can even be fatal," says Jacqueline Sutera, DPM, a New York-based podiatric physician and Fellow of the American College of Foot & Ankle Surgeons.

Dr. Jacqueline Sutera

Wearing compression socks is one preventative measure, but there are other ways to protect yourself from DVT, too. Don’t cross your legs when you’re flying, and make sure to move your feet and wiggle your toes every now and then while in your seat. Dr. Sutera suggests avoiding alcohol during a flight, as this causes dehydration, and instead just drink water. The best thing you can do, though, is to get up, stretch your legs, and take a walk up and down the aisle every hour.

PODIATRISTS AND SPORTS
OH Podiatrist Wins Medina Twin Sizzler 10K Race

It was a two-race day for podiatrist Dr. Jeffrey Wunning who started with a top 5 finish time of 16:00 minutes in the 5K race. Wunning wasn’t done after the 5K, nor was he satisfied with his placements at the Twin Sizzler last year, so he came back and won this year’s men’s 10K in 35:23.

Dr. Jeffrey Wunning

“I wasn’t in as good of shape last year,” the podiatrist and former collegiate runner originally from Illinois said. “I wanted to come back and see what I could do this year with better training. It’s a good course with a hill not only here at the end, but throughout the course there are some rolling hills,” Wunning said. “I enjoy running (the Twin Sizzler) because a lot of the community comes out and cheers you on.”

INTERNATIONAL PODIATRISTS IN THE NEWS
Heel Fissures are Major Summer Foot Problem: UK Podiatrist

“Feet are the most neglected of all body parts,” says Emma McConnachie, a podiatrist and spokeswoman for the College of Podiatry. She says that summer is the worst season for our feet and “there can be serious repercussions if you fail to protect them at this time of year.” So what are the precautions we should be taking? “The biggest problem in summer is cracks in heels that have been allowed to dry out excessively,” McConnachie says. “When you wear shoes that don’t cup the heel and hold the fat pad on your heel in place, the repeated pressure of walking causes it to splay out. The skin does the same and eventually splits.”

Emma McConnachie

Add heat and the skin loses moisture quickly, becoming excessively dry and cracked. It’s risky if the cracked skin begins to break and bleed because your feet will become more vulnerable to bacterial, viral, and fungal infections such as athlete’s foot. “In severe cases, it can lead to cellulitis, a serious bacterial infection that has the potential to cause fatal blood poisoning,” McConnachie says. To avoid infection, keep skin clean with regular washing and use a foot cream with urea to help prevent cracks and keep the skin supple.

PRACTICE MANAGEMENT TIP OF THE DAY

4 Ways to Reduce the Risk of a Lawsuit - Conclusion

Even as other risk areas are addressed, reducing the risk of a suit can sometimes be tricky. Unhappy patients and disgruntled employees are equally likely to file suit against someone they feel has wronged them, and who was vindictive or insensitive in the process. “I would say the leading trigger is the loss of employment, which may well be justified,” healthcare lawyer Michael Mustokoff says of claims filed by staff against a physician.

From the patients’ standpoint, sensitive discussions that take place in public areas such as waiting rooms, the hallway outside an exam room may appear thoughtless or mean spirited, even if that wasn’t the intention. “The number one thing would be a prompt investigation of all complaints, and a response made to the complaining party,” Mustokoff says. From there, physicians should continue monitoring the situation until it’s resolved.

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 1
RE: HIPAA and the Opioid Crisis
From: Joseph Borreggine, DPM

There is an opioid crisis and epidemic facing this country and many states are requiring medical providers register with their prescription drug monitoring program and utilize it when prescribing any opioids or Schedule II drugs to patients. I was wondering how appropriate it is, with the current HIPAA laws, if a medical provider can investigate a patient's opioid prescription history prior to their coming into the office as a new patient or even if a prescription opioid is not being written?

I ask because if a patient has an extensive opioid history that is unknown even in light of a pending prescription opioid Schedule II drug being written, then the care of this patient may become difficult in the future with respect to...

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 2
From: Allen Jacobs, DPM

We have always charged and received $20 per chart for requested records. Our office has always obtained full payment; in advance, I might add. Never, ever have we encountered any difficulty. On occasion, any reluctance to pay has been overcome with a clear positional statement of our policy to the requesting organization. In my experience, Dr. Kesselman’s advice and counsel in PM News regarding this matter is correct.

Generally speaking, a contractual obligation to provide necessary documents does not stipulate that such obligations be performed at your expense. You should recall that your contract is with the insurance provider and is NOT with the reviewing organization retained for auditing. Many states, such as Missouri, have enacted regulatory stipulations for minimum payment to you per CHART and/or per page or BOTH.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Boyd Chairs and Business Sense
From: Richard Rettig, DPM

I have a Boyd chair, which is my favorite of 3 different podiatry chairs. They left the podiatry market years ago, which makes little sense as they make dental chairs which are 95% the same. They sell parts for existing chairs, but recently stopped selling the 110v motors. If a motor goes, the chair becomes a large brick. Then you need to buy one from a chair competitor. It makes you wonder why a business makes the decisions that it does.
