Feet Need Sunscreen and Examination to Prevent Skin Cancer: MA Podiatrist

Many people do not think about their feet when applying sunscreen, but did you know that the skin on your feet is highly susceptible to melanoma and other forms of skin cancer? The American College of Foot and Ankle Surgeons (ACFAS) warns that skin cancer of the foot is prevalent and can even be fatal if not caught early.

According to Boston foot and ankle surgeon Thanh Dinh, DPM, FACFAS, early diagnosis is key to effective treatment for the condition. But because people aren’t looking for the early warning signs or taking the same precautions they do for other areas of the body, often times skin cancer in this region is not diagnosed until later stages.

IFAF Presents Awards

Dr. G Dock Dockery, Chairman of the Board, International Foot & Ankle Foundation for Education & Research, USA (IFAF), presented the 2013-14 IFAF Awards to two outstanding physicians at the 43rd Annual NW Residency Consortium Graduation Ceremony (representing the Swedish and St. Francis Podiatric Residency Programs). The program was held at the beautiful Alani Room of Palisade Restaurant in Seattle following the IFAF Seattle Summer Seminar, June 29, 2013.

The National Service Award and the Outstanding Teaching Award winners were all voted on by the Swedish and St. Francis Podiatric Surgical Residents, the teaching faculty of both programs and the members of the IFAF Board of Medical Advisors and the Board of Directors. The National Service Award was presented to Mary E. Crawford, DPM. The Outstanding Teaching Award was presented to Ryan Bierman, DPM.

The Texas Podiatric Medical Foundation (TPMF) is the charitable arm of the state association. At the annual state meeting reception on June 29 in Horseshoe Bay, Texas, Dr. Bill Van Pelt (outgoing Foundation president) and Dr. Don Falknor (incoming Foundation president) presented 4 resident grant awards.  The recipients were:

Weekend and Evening Hours in Doctors' Offices Lessen Burden on ERs

Primary care physician practices that offer evening and weekend office hours can help patients avoid trips to the emergency department while cutting overall health system spending, suggests an emerging body of research.

A recent study in The Journal of Pediatrics examining the pediatric population confirms what earlier studies among adult patients have found: When patients with urgent health problems have the option to see their regular physician, they will skip ED visits that can involve long waits, big out-of-pocket costs, and unneeded testing.

PRACTICE MANAGEMENT TIP OF THE DAY

3 Things NOT to Delegate

One of the best practices a podiatrist can adopt is to delegate. Delegating to employees increases their skills, while freeing your time to focus on big-picture issues. That said, you should never delegate when:

1. You need to deliver bad news. You will have to make tough decisions that will adversely affect your staff. You must deliver tough messages, be present to answer equally tough employee questions—and take the heat.

2. The objectives of an assignment are unclear. If you can’t articulate the goals of an assignment, don’t expect an employee to be able to complete it to meet expectations. Always clearly communicate the goals and expectations for any assigned work.

3. Employees don’t have the resources needed to complete a job. Asking people to do something without giving them everything they need to succeed is practically guaranteeing failure—and ensuring employees’ frustration.

RE: Surgical Intervention in a 12 Year Old With Flat Foot

To start, this is not a weight-bearing lateral x-ray, nor is there a weight-bearing AP for evaluation. It is very difficult to make a determination as to what procedure(s) you should perform for this patient being that we don't have proper radiographic representation, including a video of her gait.

That being said, and regardless of the patient's age, you need to perform the procedures that address the deformity. I like using the Hyprocure implants, although an MBA is appropriate as well. This will do well to elevate the talus and reduce pronation of the rear foot. I rarely do these as a stand-alone procedure. Even though she is only 12 years old, it appears that complete osseous maturity has been achieved, and you should have no problem doing aggressive bone work. Again, perform the procedures that address the deformity.

Disruptive physician is a term whereby physicians may be more verbal in expressing their opinions or more anxious to initiate needed changes in hospitals' bylaws. Some of these attributes may indeed be desirable if hospitals make decisions that do not benefit a patient's right to choose the physician who treats them.

For example, if a hospital has a bylaw requirement that accepts one board and not another for certification or hospital privileges, a complaining doctor may be labeled as disruptive. Have any physicians ever had arguments with administration? Who determines what is disruptive behavior, the administration in charge or an independent panel of physicians?

RE: Unmatched Residency Placements Currently Stand at 86 (Jon Purdy, DPM)

In regard to Dr. Purdy’s post of July 4, it is rather disturbing that he has not received the message. AACPM is diligently working to assure that there are enough programs to educate all qualified graduates of colleges of podiatric medicine.
 
For the past 18 months, I have been working as the project director of the National Residency Facilitation Project of the AACPM. This project also employs a full-time individual as the project manager whose dedicated function is to match hospitals with potential residency directors and facilitate the establishment of residency programs.
 
Since the inception of this project, over 250 new hospitals and potential residency directors have been identified. The process of working through hospital committees and finance departments is an arduous and lengthy one. To date, 10 brand new spots have been opened, 8 new sponsor hospitals have applications being evaluated by CPME. 24 institutions are currently working on the applications for submission to CPME. Additionally, 92 potential residency directors are currently working towards receiving approval from administrations of their institutions to proceed with the application process.
