NY Podiatrist Discusses Arthritis of the Foot
 
If your lateral foot pain is usually worse in the morning and improves with motion throughout the day, you might have arthritis, a degenerative joint disease. More specifically, arthritis in the ankle joint could be causing your side-of-the-foot aches. "When the pain is in the lateral ankle, this typically involves post-traumatic arthritis, or joint disease [that occurs] after injury to the ankle or ankle ligaments," Nelya Lobkova, DPM says.

"Lateral foot pain could also be caused by subtalar joint arthritis," Dr. Lobkova adds. It's possible to develop arthritis in the subtalar joint (the joint just below the ankle joint) if there's an associated flat foot deformity, she says. That's because a "severe flat foot demonstrates an abnormal angling of the heel bone, which can lead to lateral impingement of the subtalar joint," Dr. Lobkova explains.

USNC Grants Scholarship to Medical Student
 
Stephen L. Barrett, DPM and Sequioa DuCasse, DPM, MS presented a $5,000 scholarship to Ms. Bailey Boyd for her outstanding contribution to clinical medical research during her tenure at US Neuropathy Centers (USNC) in Marietta, GA. Ms. Boyd will be starting medical school at Mercer University in August, 2022. She started as a student in Dr. Barrett’s class for pre-med undergraduates at Kennesaw State University and then worked at US Neuropathy Centers as a medical assistant and clinical research intern for one year.

During her time with US Neuropathy Centers, she played an integral role in data collection and analysis, and study method design in a prospective randomized double-blinded study comparing the vasodilatory properties of papaverine to lidocaine. Article submission is expected in the 3rd quarter of 2022.

Few Complications for Exactech's Total Ankle System
 
The two-year minimum clinical outcome study—performed by Aaron T. Scott, MD, Matthew A. King, DPM, and Bryanna D. Vesley, DPM at Wake Forest Baptist Medical Center in Salem, NC—included 22 patients with an average of 24.6 months follow-up. They reported positive results and demonstrated a statistically significant reduction in pain scores, low complication rates, and a 100 percent implant survivorship at short-term follow-up.

The study, published in Foot & Ankle Surgery: Technique, Reports & Cases, is the Vantage Ankle’s third outcome publication but the first for its fixed bearing total ankle arthroplasty system. The prior two studies focused on its mobile bearing counterpart, which is used overseas.

Dr. Rosenblatt's statement that his rabbi called the act of podiatry an ultimate mitzvah or good deed, strikes home for me. In the late 70s, I was pursuing two careers, one was a rabbinical degree and the other was the act of going for a masters in biochemistry. I was fully intent upon pursuing both careers until my grandfather developed a painful foot problem. His MD and the orthopedist he sent him to, could not help him. My grandfather was really suffering. Someone suggested that I take my grandfather to a local podiatrist. 
 
I didn't know what a podiatrist was. I took him anyway and the foot doctor was able to alleviate my grandfather's suffering in one visit. I was so impressed that right then and there, I resolved to become a podiatrist. Now, whether I am lecturing at podiatry seminars, writing for PM News, or seeing patients in my office, I am directly or indirectly doing for others what that podiatrist over 40 years ago did for my grandfather.

It was the good old days of 1976. I was rotating through the local VA with a group of medical students. We had grand rounds with an eminent surgeon. Question of the day: venous ulcerations. The med students sat there staring into space. I pop up my hand and proceed to answer. As I finish, the professor asks, “Who are you?” I am Steve, the podiatry student. His eyebrows cross and he says, “You will have to leave." Fast forward 4 years. l had surgical privileges at 2 local hospitals. I shared an office with a plastic surgeon.

Concerning that “Medicare coverage policy regarding frequency of visits for nail avulsions has gone from 12 weeks to 8 months.” In my almost 40 years of practice, I have never billed 11730 twice on any digit. Perhaps I missed out on a huge revenue source, but the criteria for 11730 means the digit is locally anesthetized and either the entire nail is avulsed or a margin (or margins) is avulsed to and through the eponychium (into the matrix region). 
 
I understand that a recurrence of an ingrown nail is possible, at which time I would perform a permanent matrixectomy (11750). What other indications are there to perform repeated avulsions on the same digit and why is it a problem that the coverage policy frequency has gone from 12 weeks to 8 months?
