"Flatform" Shoes Present Danger to Wearers: NY Podiatrist
It's the hottest shoe of the summer, half flat, half platform and it's called the flatform. But some podiatrists say that the flatform is outright dangerous. "So many patients who come in complain of ankle problems, heel pain, and Achilles pain," says podiatrist Dr. Rock Positano.

Dr. Positano says the elevation in combination with the inflexibility don't allow the foot to function in a natural way. "The normal foot should make contact with the ground and adapt to the terrain on which the person is walking," says Positano. He says that not only does this shoe not allow for normal shock absorption, it also leads to instability.

Barbados Podiatrist Uses New Drug to Save Diabetic Limbs
Hal Martin strides purposefully towards the steps of the Diabetes Foundation at Warrens, all the while engaged in animated conversation with American podiatrist Dr. Christopher Otiko. Just a few months before, the Barbadian financial consultant and businessman had been facing the possibility of amputation of his right leg, so deep and severely infected was the wound. Hal was introduced to Otiko by another podiatrist who had been treating him for over five months without success.

Otiko was trained in San Antonio, Texas, called “the amputation capital of the United States,” and diabetic foot care has always been his specialty. He believes the new drug, derived from the old antibiotic tetracycline, can give Barbadians in danger of losing a foot, new hope. The results from the first TetraStem trials by a doctor in Armenia were convincing enough to lead to approval and registration of the drug by the U.S. Food and Drug Administration in February this year. Buoyed by his experience, Martin gifted a small quantity of the drug to the Diabetes Foundation and Otiko said: “I know that the medication can save a lot of limbs and a lot of lives here.”

With its growth and success in helping people solve their foot issues, I saw an opportunity to serve more people by moving and converting the store into a New Balance concept store," said Levine.

The condition is cutaneous larva migrans. I saw it in a patient 5 years ago and I treated it in myself last year after a trip to Jamaica. The condition is somewhat painful and very pruritic. It responds quickly (within about 3 days) to oral ivermectin. Treat with a single 12mg dose. Here is a link to a study with a 12mg dosage: I also used betamethasone diproprionate ointment for pruritis.
If you question your patient, you will likely find that they stayed at a resort which was frequented by cats at dinnertime. While everybody sleeps, the cats bury feces in the sand and humans contact the sand the next day. It is then that they contract the hookworm infestation. I didn't notice mine until I was home for about 2 days.

This looks like creeping eruptions to me. We used to see this in San Antonio, and I have actually seen this in Pennsylvania. The treatment is mebendazole, one tab in the morning and evening x 3 days.

When coding because of additional time spent with a patient and father, in this case, the use of code 99354 would be appropriate. I agree, in this case, that CPT 99214 is not warranted. More appropriate would probably be CPT 99212 or 99213, depending on notes. Using these codes plus the addition of code CPT 99354 will increase the time allowance; therefore, the payment allowance and the provider will be able to meet the other criteria of the correct E/M procedure code.

I enjoyed reading Dr. Ribotsky's comment, but I really think a lot of the procedures he does are for pain and not all just for aesthetic appearances. There are very few patients who want hammertoe or bunion surgery for cosmetic reasons, but there are more patients who might want the fillers. These are some patients who want this surgery because they can't wear fashionable shoes without having pain.
If pain is the consideration, then I don't feel this is cosmetic surgery. For most patients who want this surgery and don't really have pain, I tell them...
