Foot Health Can Indicate Other Issues: MD Podiatrist
 
When it comes to health, pay attention to feet from the heels to the toes. Feet can reveal some surprising health problems. Dr. Gary Pichney, a surgical podiatrist, said feet can also signal a number of other health problems, including serious ones like cardiac disease. Other health problems that can show up in feet include arthritis, including osteoarthritis and rheumatoid arthritis, poor circulation, gout, and don't forget toenail conditions.

"Often times, toenails can change shape, can change size, and can have either spoon deformities, looking like a spoon, or will have little pits in them, perhaps, or they might have a black line that goes straight through them, or they might be yellow, brown, and thick, and each of those different things can indicate, perhaps, a psoriasis condition, perhaps a fungal condition, or even something as scary as malignant melanoma," Pichney said.

MO Podiatrist Unveils Commissioned Sculpture
 
Last weekend, St. Louis Mayor Francis G. Slay unveiled a sculpture by Zenon Duda, DPM which will be permanently displayed at a city park.

Duda, 60, is an artist, a sculptor — a creator, he prefers — but the “doctor” in front of his name isn’t just for kicks. He is also a board certified podiatrist who has been in the field for more than 25 years, working at Perry County Memorial Hospital for most of it, but now working at both Southeast Missouri Hospital and St. Francis in Cape Girardeau.

NY Podiatrist Comments on "Superior" Flip-Flop
 
Other flip-flops had better step up their game, because Oofos are oo-mazing. And even podiatrists who’d prefer to kick all flip-flops to the curb admitted these thongs have got it going on. “You can’t even compare this to the standard flip-flop. It is far superior,” said Midtown podiatrist Dr. Gary Evans, who is not on the Oofos payroll. “There’s no support, no structure to most flip-flops.”

That’s not to say that Oofos are perfect. There are risks associated with any open-toe shoe. “Any time you wear something open where the foot easily comes out, there’s a possibility of injury, slipping and sliding, twisting your ankle, bumping into a hard object, damaging a toenail,” warns Dr. Evans.

AAFAS Holds Its Semi-Annual Scientific Seminar
 
The Academy of Ambulatory Foot and Ankle Surgery (AAFAS) held its semi-annual Scientific Seminar June 16-18, 2016 at the LSU medical school in New Orleans. 71 podiatric and orthopedic surgeons from the U.S., Canada, Australia, Europe, Africa, and the Middle East participated in a cadaver lab and attended lectures on minimally invasive surgery of the forefoot, rearfoot, and ankle. Subjects included treatment of bone spurs, hammertoes, metatarsal problems, bunions, surgery for ulcers, antibiotics, stem cells, rearfoot procedures, involving cavus, flatfoot, tendon imbalances, and the use of the HyProCure implant.

International lecturer, Stephen Isham, DPM, MD demonstrated in the lab the latest minimally invasive bone and tendon procedures. Monroe Laborde, MD demonstrated rear foot tendon balancing techniques for the treatment of and prevention of pedal ulcers. Michael Fanous, DPM lectured.on stem cells in conjunction with surgery to avoid amputation. Rem Jackson lectured on marketing your minimally invasive practice. The use of office-based CAT scans, diagnostic ultrasound, and the avoidance of and treatment of complications were also discussed. The next meeting will be held January 12-14, 2017 at the LSU medical school in New Orleans.

The Pipefitter and Plumbers Union has well over 200 local unions in the U.S. and Canada. Local 290 in Portland does place an emphasis on Retiree Wellness Information and has a section on their website dedicated to it. The National United Association also has a website at http://www.ua.org/ that may offer some background that could be helpful to you. 
 
You should be pro-active in this. Reach out to the local union and “interview them” to find out: 1) What are common foot ailments that may be unique to the locale and occupations? 2) Find out the prevalence of diabetes and MRSA in that local union or in the local population. 3) What footgear does the union recommend? Does it provide adequate protection and prevention from water exposure or from burns from welding sparks? (etc., etc., etc.) 
 
Create a list of questions that are specific to each presentation and do your research and due diligence so you provide relevant, specific information that pertains to their trades. You do have an opportunity to mention the services you provide at your practice and where it is located, because you never know!

RE: The Passing of Thomas Sgarlato, DPM
 
I cannot express how saddened I am to hear of the passing of Tom Sgarlato. When he came to England to lecture in the 1970s and '80s, he enthused a generation of UK podiatric surgeons. He taught us to value biomechanics and understand foot function, how foot surgery could work if you never lost sight of basic principles. After lectures, he was funny, self-deprecating, entertaining, and great company.
 
We are internationally too few to lose such valuable colleagues, and are all the poorer for his passing. My condolences to his family and his colleagues.

RE: The Passing of Thomas Sgarlato, DPM
 
We read many wonderful notes about this fine man who advanced podiatry so very much. But I also wanted to include another note about one nurse at a hospital where he worked. In our California retirement community, a friend of mine, a registered nurse who came originally from Croatia, worked closely with Dr. Sgarlato, his residents, and other podiatrists. She states that, "Dr. Sgarlato was always kind and patient with her at a difficult time when her English skills were not yet well-practiced."
 
She regarded the DPMs whom she worked with as "her boys." (At that time there were very few female DPMs.). In contrast to some of the MDs she worked with, she stated her "podiatrists" were always thoroughly prepared and experienced in the OR, and in her opinion, were some of the "best surgeons" she worked with. She cried when she heard about his death.

RE: The Passing of Thomas Sgarlato, DPM and Nicholas Grumbine, DPM

It's sad to hear that two pioneers died so young. I interacted with Tom here in the San Francisco Bay area, particularly as we had a mutual interest in ankle equinus. He wrote words of encouragement as he read my research. I re-listened to his podcast on Bret Ribotsky's Success program, and I recommend more people do so as well. He noted that Achilles lengthenings were detrimental and that gastroc releases were safer, but he did not do them on runners, athletes, etc. He also recommended that DPMs do more research on biomechanical theories - good advice to all DPMs.

I first spoke with Nick Grumbine in 1987 about his innovative digitized orthoses. I have used devices from his lab (Plus Labs) throughout my 27 year career. Both he and Dr. Sgarlato were true pioneers in the biomechanics of foot and ankle surgery. I am grateful to both for pushing the envelope. Dr. Sgarlato was a founding father of the American Academy of Podiatric Sports Medicine, and Dr. Grumbine founded a company that has helped thousands of my patients with the orthoses he developed.

Dr. Udell brings up the government specs in his post. If you don't report on PQRS, you stand to lose a minimum of 2% of Medicare income, depending on how many years you miss, etc. Today, I asked five colleagues if they even knew what the acronym stood for. None knew. Physician Quality Reporting System. Let's also not forget the 2% that you are still being screwed on thanks to the fact no one in Congress knows how to balance a budget so we are still in sequestration. Why are physicians tolerating this 2% reduction, year after year? The politicians should be getting the reduction, not us....it's up to them to balance the budget, not us. 
 
I still am not comprehending how the information being supplied is of any quality at all. No one can seem to answer this question other than "we can only tell you what needs to be reported." At the end of the day, all this appears to be is a tax on doctors who don't do it. I am not sure why the ones doing it are happy, or complying. I am not sure how a law gets passed and no one fights it. I do recall hearing about people fighting to be part of the "stimulus package" and wanted to get their share of the 45,000 dollars that MDs were getting. People, nothing in life is free. Nothing, zilch. So, now we are part of this crap. How, do we get out of it? I recall Bruce Frankel, DPM (may he rest in peace) of NYCPM always walking through the halls saying, "are we having fun yet?" Dr. Frankel, that would be a resounding NO!
