Walking is Best Exercise for Most People: PA Podiatrist
When it comes to healthy feet, the adage “use it, or lose it” is sage advice, according to Khurram H. Khan, DPM, chairman of the Department of Podiatric Medicine at Temple University. If you are not on your feet regularly, you will lose muscle and bone strength. “We always tell patients, we want you to be active,” Dr. Khan said.

“Walking is probably the best exercise for most people, young and old. Bone grows along stress lines. So, wherever there is stress, the bone will grow or be maintained. We want patients to walk. We want them to maintain the calcium definition that comes from ambulation,” says Dr. Khan.

Micro-trauma Increases Risk for Onychomycosis: AZ Podiatrist
A fungal infection that Emily Splichal, DPM describes as a yellowing on the nail, or a pattern you don’t recognize should be diagnosed. “Many people see white and think it’s fungus, but it tends to be more yellow. What predisposes you to fungus is an injury to the nail,” says Dr. Splichal. “For example, if you often wear heels that squeeze your toes, and your foot is at an angle so you’re also sliding into the nails, it will create micro-trauma that can increase risk for infection. It’s also common for fungus to spread to other toenails.”

Dr. Splichal says the fungus sits under the nail plate—it’s not in the nail itself—so it can be very difficult to treat. “I’d compare it to mold in carpet—it’s really tough to get rid of—so you have to be really persistent. Most over-the-counter nail fungus medications cannot penetrate nail polish, so you’d need to take a nail polish ‘holiday’ to treat it, and even then, they may not work. Some doctors will use lasers, but I’ve always had mixed results. Therefore, I prefer to treat the fungus systemically with antifungal oral medications.”

Barbie Challenge on TikTok Poses Danger: FL Podiatrist
In the opening of the Barbie trailer, Margot Robbie gets off to the right foot by stepping out of her heels with feet still arched in a now iconic scene. Naturally, TikTok has turned it into a viral challenge that has some podiatrists concerned about its dangers. Folks participating in this particular trend have been attempting to recreate the scene by posing up on their toes and taking a few carefully balanced steps forward.

Jodi R. Schoenhaus, DPM at Foot, Ankle & Leg Vein Center in Florida warns against the risk of injury while attempting the challenge over a prolonged period of time. “If someone attempts the pose once or twice, they will likely be okay and produce a great TikTok video,” she said in a statement. “However, if attempting the pose and stride over long periods, there are some risks involved. The ankle is unstable, which can lead to ligament sprains and injuries, commonly seen with high heel use.”

4 Strategies to Deal with Difficult Patients
Here are four ways that physicians can deal with “difficult” patients.
4. Set appropriate boundaries
Patients have a variety of mental and physical challenges, and often they try to push to get antibiotics, narcotics, or treat your staff in a way that’s unacceptable. Don’t be shy about setting boundaries early to avoid bullying behavior. And if they do, don’t be scared to send them a letter informing them that they are no longer welcome at your practice.

Ronald T. “Ron” Osborne, 91, of Marysville, formerly of Beavercreek, passed away peacefully Saturday afternoon, July 1, 2023, at Monarch Skilled Nursing & Rehab after months of failing health.

He spent one year at Kent State University and while in ROTC, was a member of the Pershing Rifles, before being drafted into the United States Army during the Korean War. After proudly serving his country for 6 years, he went back to school and graduated from the Ohio College of Podiatric Medicine, becoming a podiatric physician and surgeon in Dayton for 32 years.

Query: Debridement in the Post-Operative Period
The patient initially had a gastroc recession at the right lower extremity. Our surgeon performed a debridement procedure of the right foot ulceration. It was performed in the clinic and we planned to code it as CPT 11042. However, the patient is in the 90-day post-op period from initial surgery that was performed 4 weeks ago. Is a modifier necessary to submit for payment? Is it -79?

Response: I recommend using the -58 modifier as this is likely a related procedure to the original surgery. The -79 modifier is for an unrelated procedure in the post-op period. I would clearly document the extent of this wound and what was actually performed. I would take a picture as well.
To use CPT 11042, you must have a full thickness wound and you debrided down to and including subcutaneous tissue. Cleaning up some mild dehiscence/scab on the wound would not warrant extra payment.

Dr. Neuhoff who is both a podiatrist and veterinarian, brings up an interesting point in this discussion. People in my own neighborhood are ready and willing to take their dog or cat to a veterinarian and pay thousands of dollars out-of-pocket for many procedures. Many of these same patients will bark and fight about a ten-dollar co-payment after the pain in their feet is alleviated by our good work.
The same applies to dentistry which by and large is independent of insurance companies. It applies even more so in the case of a patient who will not want to pay out-of-pocket for cream or salve that we dispense. That same patient will go down the block and pay a plastic surgeon ten or more thousand dollars for a facial procedure.
I salute those of us who were successful in declaring their independence from the insurance industry.
