TX Podiatrist Discusses Salon Safety

"At my Houston podiatry practice, we see a lot of patients who have developed all types of problems because the nail salons they frequent did not operate at the highest standards for hygiene," says Gabriel Maislos, DPM. "Some of these establishments are among the so-called 'high-end' nail salons, but they've failed customers by not implementing all the measures designed to prevent infection, cross-contamination, and other issues."

Dr. Gabriel Maislos

Dr. Maislos points out that the state of cleanliness of a salon and its bathrooms can say a lot about other hygiene practices. It's also recommended that a customer ask to see the salon's autoclave machine. Furthermore, as service starts, the autoclave bag will have an indicator showing that the tools inside have undergone sterilization.

Podiatrists Volunteer at Peachtree Road Race

A group of podiatrists, residents, and students from the Podiatry Institute (DeKalb) and the Atlanta VA Medical Center took the time out of their holiday day-off to aid the participants of the annual Peachtree Road Race in Atlanta, GA.

INTERNATIONAL PODIATRISTS IN THE NEWS
There is an Information Gap Regarding Foot Health: Aussie Podiatrist

With the demands of modern lifestyles and a general lack of knowledge around podiatry having a major impact on foot health, local My FootDr podiatrists have warned that Australians have a giant step to take when it comes to looking after their feet and bodies. My FootDr podiatrist, Chris Lofts, said there is a clear information gap among Australians about the importance of foot health. “Most foot conditions are preventable, but they are being ignored because there a distinct lack of awareness around proper care of feet and knowing who to turn to for professional advice.

Chris Lofts

“Feet are a foundation for your body and taking them for granted could lead to permanent issues such as arthritis or conditions in other parts of your body including legs, knees, hips, and lower back,” he said. Podiatry is the only health profession entirely devoted to the study, diagnosis, and treatment of disorders of the foot, ankle, and lower extremity. Podiatrists offer a full scope of services including comprehensive foot assessments, gait analysis, foot orthotics and even laser treatment for fungal nail infections.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
UT Podiatrist Uses Cartiva Implant

Tom Benedict was one of the first Utahns to have Cartiva, a first-of-its-kind synthetic cartilage, similar to the size of a pencil eraser, implanted in the joint of his right toe in 2016. The biomedical polymer provides something for his bones to "bounce" on, instead of the bone-on-bone action the joint was getting due to a rare form of ankylosing spondylitis.

Dr. Clark Larsen shows how Cartiva is implanted (Photo: Steve Griffin)

Cartiva is made of polyvinyl alcohol and is FDA-approved for use to replace worn-down cartilage in the big toe joint. "It's a lighter weight material that has some give to it," said Dr. Clark Larsen, a podiatrist at Rocky Mountain Foot and Ankle in Murray, who has implanted more than 30 Cartiva units in the last year-and-a-half. "There's some squish to it. It's like a jelly bean. There's some flex to it."

PRACTICE MANAGEMENT TIP OF THE DAY

4 Ways to Reduce the Risk of a Lawsuit - Part 4

4. Consider the patient perspective - General communication skills can be improved by looking at interactions from the patient’s perspective. “There’s no element more important to avoiding a malpractice claim than a healthy doctor-patient relationship built on clear and effective communication between one another,” says Samuel N. Klewans, JD, a partner at Grad, Logan and Klewans, a law firm in Falls Church, VA. He suggests physicians try putting themselves in their patients’ shoes, viewing interactions from their perspective.

Putting time into patient relationships outweighs concerns about already too busy schedules. People expect timely replies to calls and e-mails. Don’t rush through appointments. “Your patients are very concerned with their health, and when they’re meeting with you, they want to believe you’re very concerned about them, too,” Klewans says. Showing empathy and taking the time to fully answer patients’ questions during the visit can go a long way toward making them feel comfortable.

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 1A
From: Paul Kesselman, DPM

With all due respect to this recent posting, just about every PCP and MD/DO specialist I have spoken with over the last few years who has asked for money has received payment from the third-party requesting the charts on behalf of the carrier. And all have told me that the carriers involved (excluding Humana) provided no resistance. Unless otherwise precluded in your contract, you have every right to ask for payment and what does asking really cost? I can’t speak for interactions with Humana as I am not a Humana provider and not privy to their contractual obligations.

Of course, whether they pay you and/or decide not to pursue further inquiries for charts now or in the future is quite another story. Asking costs nothing! Not asking costs you quite a bit!

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 1B
From: Daniel Chaskin, DPM

"The doctor must  provide such records at no cost to the carrier (usually stated in contract)."

I disagree with Ms. Moracka-Sawaki's statement. My guess is that HMOs are getting paid a rate per patient that includes all administrative costs. One of those administrative costs includes the fees required to pay for the release of records.

1. If podiatrists are not being reimbursed for the release of such records, then the actual fee received by the HMO from the federal government should be proportionately reduced by such an amount since the HMO is not paying for such records.

2. There are Stark laws that may come into play. To have access to a patient population by a contractual provision that waives the right for a provider to charge for a record release just might depart from federal law. As a suggestion, perhaps the APMA can investigate if all podiatrists are legally entitled to bill for such records. It just might be illegal to deny payment as per a contractual clause that might not be in compliance with federal law.

RESPONSES/COMMENTS (NEWS STORIES)
From: Steven J Berlin, DPM

I want to congratulate Dr. Robert "Bob" Hilkemann on his dramatic rise from retired podiatrist to the highest office in the State of Nebraska (even if it was just for two days). Bob and I served many years together on The Fund For Podiatric Medical Education (now the APMA Educational Foundation). I just knew he had more successful days ahead. My sincerest congratulations to you on your service to the citizens of Nebraska and to your profession.
