NY Podiatrist Discusses Benefits of Compression Socks
 
Compression socks and stockings help improve blood flow through the veins and reduce swelling, said  Michael Mazer, DPM, a podiatrist and associate of the American College of Foot & Ankle Surgeons. This is particularly beneficial because, as Dr. Mazer pointed out, we tend to develop various health conditions that can contribute to leg swelling as we age.

He said that the increase in circulation from wearing compression socks and stockings can result in less lower extremity discomfort and prevent muscle fatigue, especially for individuals that are on their feet often.

Robert Breiner, DPM recently received the MSGR. Geno Baroni Award for Advocacy from the Catholic Charities. This award is given to an individual who is a strong advocate for vulnerable and oppressed populations through their activities in the community over a long period of time.

Rigidity in Shoe Soles Provides Support: NY Podiatrist
 
If you have flat feet or high arches, finding the appropriate shoe with supportive arches is a must. And why exactly? Amnon Barnea, DPM of Walk NYC Podiatry says, "The arch is the ultimate architectural design [of the foot]," and having adequate support for your arches enables your feet to "function as an accelerator and decelerator of motion, provides structural stability and capability of transitioning from a superb shock absorber at 'heel strike' to a semi-rigid structure needed for 'push off.'"

Dr. Barnea says, "It is widely believed that arches need softer or 'cushiony' materials for comfort. However, the arch actually requires some rigidity for better support, which reduces the compressive forces of the body on the arches. Shoes with soles that are very soft and 'bend' in the middle are less supportive. Soles with firmer materials under the arch or those with a beveled heel and a forefoot rocker offer excellent support."

Authored by a team of renowned experts, this publication aims to equip healthcare professionals, researchers, and individuals affected by diabetes with the knowledge to effectively prevent, diagnose, and treat diabetic foot ulcers.
 
The authors of this publication are internationally renowned experts in the field of diabetes and wound care, bringing a wealth of knowledge and experience to this comprehensive review. David G. Armstrong, DPM, MD, PhD, podiatric surgeon with Keck Medicine of USC, professor of surgery at the Keck School of Medicine of USC, director of the Southwestern Academic Limb Salvage Alliance (SALSA) and also founding president of ALPS noted, “The big idea from this comprehensive manuscript is that it highlights the importance of teams as well as technology in helping to stem the very high volume of DFUs and amputations in people with diabetes, worldwide.”

4 Strategies to Deal with Difficult Patients
 
Here are four ways that physicians can deal with “difficult” patients.
 
3. Control the narrative as best you can and schedule follow-up visits
 
Many patients have the ability to derail your entire day, ask endless questions, or feel they can use a 15-minute visit to share their entire life story. You do want to sit down and look at the patient and make him or her feel cared for, but you can’t spend an hour with each one. Encourage the patient to focus on the issue at hand for one visit but perhaps make a follow-up visit to discuss the rest. You can try empathetic phrases about how you value the patient’s time and the time of those in the waiting room and you want to “focus on the most pressing issue” so that it can be dealt with.

Living abroad, being board certified in both ABPM, and ABPS, I was slightly amused when I saw someone who “trained” 2-3 years post high school write that they are now ABPM International board certified. See below who is eligible according to the ABPM International website and make your own determination whether you’d like to laugh or cry. 
 
“Initially, eligibility to sit for the exam will be limited to podiatrists who have completed at least a bachelor-level education in the UK and Ireland, Canada, Spain, Australia, New Zealand, South Africa, and Malta.”

I read Dr. Rosenblatt’ letter with interest and a bit of confusion. I transitioned my practice to a self-pay 3 years ago. Unfortunately, I did it two months before Covid and this combination made it financially very challenging. However, we persisted and are now doing well. We managed the Medicare issue by having the patients pay us (we do not accept assignment) and filing for them. We file only for appropriate covered charges and Medicare reimburses the patient. We do not file any appeals or deal with Medicare at all, other than filing the initial claim. We do provide fee sheets with ICD and CPT codes if a patient wishes to file themselves with their insurance. 
 
I perform approximately 5 “major” surgeries per month (tarsal tunnel, bunionectomies, exostectomies, hammertoe repairs, etc.). Patients pay for these. There are a few who prefer to go to a practice which files their insurance for surgeries, and I happily refer them to one of my colleagues.
 
I took my staff with me to the IPMA and Midwest meetings. After attending them, my staff is incredibly grateful that they do not need to deal with the insurances…so am I! I think it would be very difficult to do a self-pay if you were a cardiac surgeon or a neurosurgeon, but it is very doable for podiatry. Perhaps I find it easier because I am also a veterinarian, and clients routinely pay bills that are more than a bunionectomy! If a podiatrist wants to have a contract with his/her patients rather than their insurance company, it can be done.

May I say one thing on the subject of change - Knowledge is knowing that a tomato is a fruit and not a vegetable. Wisdom is knowing not to put it in a fruit salad. In my opinion, insurance-dependency is like putting too many tomatoes in your fruit salad and allowing it to overwhelm the sweetness that you could otherwise enjoy.
 
Dr. Udell, many podiatrists all over the country have already included therapies into their practice that are not covered by insurance. They also sell products. That approach is called a hybrid practice. The difference is that these docs depend on services that are covered by insurance and then privately contract with patients for direct payment for ...
