MA Podiatrist Discusses Study That Correlates Balance to Mortality Rate

Interestingly, according to a new study, the ability to balance for 10 seconds on one foot may predict life expectancy. Results showed that those who failed the test were twice as likely to die in the next 10 years. Alissa Kuizinas, DPM, a board-certified podiatrist explains, “There is also a link between balance and cognitive function, as the body’s ability to receive input from the environment is translated in the brain to allow the body to stabilize itself. If this ability is compromised, cognition can suffer as well. All of these factors can increase mortality rate."

Dr. Alissa Kuizinas

This study provides very useful information as it highlights the need for better evaluation of balance, especially in older adults. Factors such as overly cushioned footwear, lack of sensory input and lack of ground purchase of the foot due to deformities such as bunions all contribute to poor balance, Dr. Kuizinas states. These are easily treatable when identified, and there are simple ways for at-risk individuals to improve their single leg balance with specific exercises and lifestyle habits.

PODIATRISTS AND FOOTWEAR
IL Podiatrist Lists 3 Criteria for Buying Shoes

Board-certified podiatrist Patrick McEneaney, DPM, has three criteria he asks every patient to follow when shopping for shoes. First is fit. "You want to make sure that the shoe fits adequately. It should be wide enough in the front, but not too wide in the back," says Dr. McEneaney. If the back of your footwear isn't snug enough, it will cause your feet to rub and may result in blisters. And conversely, if the shoes is too narrow, it will squeeze your toes and cause joint pain.

Dr. Patrick McEneaney

Support is also important, says Dr. McEneaney. Rather than going for a shoe that's the foamy, squishy equivalent of wearing clouds on your feet, he recommends picking up something foamy, yet firm. "[Squishy] shoes feel comfy initially, but they may lead to chronic overuse problems or can actually accentuate preexisting problems," says Dr. McEneaney. Lastly, Dr. McEneaney recommends picking something up that has a stiff midsole. "So when you look at the bottom of the shoe, the middle one-third should be stiff and difficult to bend. The more that it bends, the more it's going to load pressure from the back of the foot to the front of the foot. If the shoe has some more stiffness, it's gonna load the pressure through the shoe, and take some of that stress off the middle of the foot," says Dr. McEneaney.

PRACTICE MANAGEMENT TIP OF THE DAY
Cryptocurrency Investing Rules for Busy Doctors

Cryptocurrencies can be complicated. As a busy medical professional, you likely do not have time to research tokens or the nuances of this specific asset class. As you invest, how will you know whether you are doing the right thing? As prices zoom up and down, it feels like you should do something in response to market movements, but what exactly?

1. Stick to your plan

These markets are very volatile and will go up and down frequently. You cannot control that but you can dictate when and how much you invest. Be consistent and focus on what you can control. Investors who chase good performance or run away from poor performance are less likely to be successful.

RESPONSES/COMMENTS (CLINICAL)
From: John Throckmorton, DPM

The never-ending problem in college sports is related to what is the best athletic shoe for each athlete, or the multimillion dollar funding by athletic shoe corporations to college sports. As a previous podiatric consultant for a Division One sports college, it was a challenge treating stress fractures and foot and ankle injuries in athletes, since each athlete had different biomechanical needs and the team had to wear all the same shoe brand to continue getting donations to the athletic fund of the college.

Most college athletic departments rely on these funds to continue all their various sports programs (track & field, rowing, swimming, and skiing. The biomechanics of each sport’s dynamics in each athlete is different in affecting the human foot an ankle. I found that use of individually specific orthotics, custom made to each athlete's biomechanics pathologies greatly reduced the number of stress reactions that lead to fractures in the athlete’s foot. They almost always were still able to use the designated shoe brand, therefore keeping the donations coming in for our sports at the college. This may change now with the new NCAA ruling as to compensation to individual athletes. It will be interesting to follow.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Michael DiGiacomo, DPM

As we celebrate Independence Day, it reminds me of my independence from a tyrannical foot orthopedist. When I applied for surgical privileges at Merritt Hospital in Oakland, I was told that I would have to show evidence that I had done at least three of the procedures before. As I had a residency at Kaiser, I readily complied. As I began to schedule cases, he demanded that he interview each patient pre-operatively. About two months into this onerous procedure with him breathing down my neck during the cases, a pre-operative lady reported that his interview consisted of a lecture telling her how much better trained he was! I asked her if she would testify to her statement and she said she would gladly do so.

I contacted Joseph Alioto, a San Francisco restraint of trade attorney. He said if she testifies, he would financially ruin the orthopedist as well as the hospital. He advised me, however, that when we prevailed, I should consider leaving Oakland. The next day, I informed the hospital CEO of this development. The next day, a courier delivered a letter to me stating that I would no longer need to be proctored for any procedure in my scope of practice.

RESPONSES/COMMENTS (PODIATRISTS IN THE COMMUNITY)
From: Michael M. Rosenblatt, DPM

Heartiest congratulations to Dr. Stuart Kigner, DPM who is now retiring after 38 years of employment at Massachusetts General Hospital! I actually had to read this announcement twice to be certain that I understood it.

I admit I was shocked that DPMs have been on the staff of MGH for so many years. It is unlikely that those misinformed DPMs who create a flood of anti-podiatry rhetoric online know anything about this podiatrist's distinguished history. Or that such recognition would even be "possible" for a DPM or have any idea a DPM achieved this status.

RESPONSES/COMMENTS (APMA STATE COMPONENT NEWS)
From: Eric Thuen, DPM

Having practiced with Dr. Lawrence for more than thirty years, I was not surprised to learn of his generous contribution to The Legacy Fund. Bruce is a very unassuming individual who has given generously of his time, vast knowledge and funds to improve the lives of countless others both in and outside of our profession, usually below the radar and without mention. I am fortunate to call him my friend, and we are all fortunate to have him as our colleague.
