NY Podiatrist Discusses Ingrown Toenails
“An ingrown toenail occurs when the corner or border of a toenail—most commonly the big toe—impinges or otherwise irritates the surrounding soft tissue of the toe, resulting in painful inflammation that can puncture the skin and cause acute infection,” says Michael J. Trepal, DPM, a podiatrist at the New York College of Podiatric Medicine.

The most important thing you can do to stop ingrown toenails from happening is to wear shoes that fit. “Wear properly-fitted shoes with plenty of width in the toe box and socks that do not overly constrict the toes,” Trepal says.

MD Podiatrist Hosts Swing Sports Awards Banquet
The Prince George’s County High School Baseball/Softball Awards celebrated its fourth anniversary at Kentland Community Center in Landover, honoring the top “swing sports” athletes throughout the county. When Dr. Rick Bryson started the unsanctioned event in 2014, Bryson was hoping to bring baseball and softball out from the shadows of more popular sports in Prince George’s County.

Bryson has a passion for baseball, kids, and athletes. The Prince George’s County based podiatrist has worked tirelessly to bring attention to swing sports. Bryson continues to spend his time and his money to bring attention to high school baseball and softball in a county where those sports have become an afterthought.

Her biggest tip for women struggling to choose the right footwear is to avoid the standard styles altogether.

"Make sure that you avoid wearing slip-on shoes and mules. Never wear thongs for anything but the public shower block," she told FEMAIL. "Having to claw your toes to keep a shoe on the foot really can and does lead to lower back pain, as your gait pattern is completely changed."

4 Ways to Reduce the Risk of a Lawsuit - Part 3
3. Huddle together - Another strategy involves implementing a daily huddle where staff come together to plan how to handle whatever’s on the schedule. The effort can be as simple as a five-minute discussion that covers a quick rundown of the day’s challenges—calling attention to elderly patients who might need extra attention, for instance, or developing a plan to deal with an unplanned staffing shortage.
Practices that are proactive about spotting potential problems and having a solution at hand can circumvent a number of common risks. “They make it part of their professional practice to recognize it, understand it’s something they have to deal with, and then work as a group to try to identify their risks and what to do about them,” says Diane C. Fernald, RN, JD, vice president of education and consulting services at Med-IQ, headquartered in Baltimore.

Query: Initial hospital care within 90-day global of surgery
I have a patient whom I did surgery on approximately one week ago. He was admitted to the hospital for a totally unrelated medical issue. They did consult me and I did go see him in the hospital. Am I entitled to an initial hospital care/consultation on this Medicare patient even though he is within his global post-operative care? If so do I need to use a modifier, and if so, which one?

Response: If you were consulted to evaluate something related to the procedure for which he is in a global period for you, no you cannot. It has nothing to do with the reason for his admission. In the situation you describe, it sounds like the patient is being seen by you for a follow-up of last week’s surgery. Unfortunately, even though you had to make the trip to the hospital, and even though the admission is unrelated, this does not meet the requirements of -24 modifier.
The only E/M modifier option available during a global period is the “-24” modifier (unrelated evaluation and management service by the same physician during a post-operative period). The pathology for which the E/M is being performed must be “unrelated” to the procedure that was performed. Some interpret this to mean a different diagnosis, like wound dehiscence or post-op infection, but that is an incorrect interpretation. It must be unrelated.

Query: Computerized Orthotic Lab
For many years, I have been very pleased with custom orthoses with customized top covers that were fabricated from computerized standing and walking gait scans from two Canadian orthotic labs; the most recent one being The Orthotic Group. For some reason, in the recent past, our receipt of new custom orthotic orders and repairs/modifications has been delayed due to customs matters between the U.S. and Canada. I would appreciate it if my colleagues could advise me as to whether there are any orthotic labs in the U.S. that provide this type of technology and who also offer quality customer service.

Regarding the chart review requests from Medicare HMOs, there is an important clarification which must be made. These reviews have nothing to do with reviewing the doctor's care provided or their documentation. That type of review is covered by your contract and happens at their request, on no set timetable. They may or may not ever happen. The doctor must provide such records at no cost to the carrier (usually stated in contract).
Medicare HMOs are required to not "cherry pick" patients and must provide care to patients requesting coverage. There is a minimum "Acuity Profile" for Medicare patients and payment by Medicare to the HMO is based on patients meeting...
