MI Podiatrist Joins Botsford Hospital Staff
Janet Ajrouche, DPM, CWS, has joined the medical staff at Botsford Hospital in Farmington Hills. Dr. Ajrouche earned her doctorate of podiatric medicine and surgery from Barry University, Miami, Fla. She completed a three-year residency at Botsford and held the position of chief resident during her final year of clinical training. Dr. Ajrouche won Botsford's Alvin Yarrows Research Award in 2010 and 2011.

She is an associate of the American College of Foot & Ankle Surgeons and a member of the American Board of Wound Management, the American Podiatric Medical Association, the American Board of Podiatric Surgery, and the American Board of Podiatric Medicine.

Drs. Daniel T. Hall, IV and Mallory M. Przybylski of Louisiana Foot and Ankle Specialists, LLC recently opened a new practice in Lake Charles. Dr. Hall recently completed a residency at South Miami Hospital of Baptist Health in Miami, FL. Dr. Przybylski recently completed a residency at theYale-New Haven Hospital/VACT in New Haven, Connecticut.

Query: Pruritic Lesions S/P Caribbean Cruise
A 35 year old woman presented to my office with a complaint of itching and a skin rash that has become progressively worse over the last month.

She had applied OTC antifungals without any relief. Shortly before the rash began, she relates that she was on a cruise to Mexico and the Caribbean. Any thoughts?

Query: Surgical Procedure Not Carried Out
I had a 9 year old patient in my office today who was scheduled for a partial nail removal. Everything was set up to perform the procedure, but when it came time to anesthetize the toe, he was not willing to allow me to follow through with the procedure. I did not do a full work-up on this patient, as I had already done this in a previous appointment. I spent 30 minutes trying to convince the patient and his father to allow me to perform the procedure. I was unsuccessful. Can I bill for anything today?

Response: As is often the case, it's all in the documentation. If your documentation states that you spent 30 minutes "trying to convince them to do it," there's not much there. However, if your documentation states that you spent 30 minutes in "counseling and coordinating", explaining why the procedure is important, educating them as to what could happen if they do not have the procedure done, what the potential risks/benefits were to doing it and not doing it, alternative treatment options, what would be involved in sedation, explaining the option to do it in an operating room, etc...THEN, in my opinion, you could bill for an E/M using the time guideline of counseling and coordinating (involving more than 50%) dominating the visit. CPT 99214 indicates that you spent 25 minutes with the patient and greater than 50% of that time was spent in counseling and coordinating.

RE: Beware Of a New Cause for Patient Non-Adherence

The new Affordable Health Care Act insurance plans are throwing us a new hurdle; increased patient non-adherence. Two weeks ago, a middle-aged, type one diabetic patient presented with an abcess in his left third toe. I did an incision and drainage, cultured the exudate, and then placed him on oral antibiotics. He missed his follow-up visit, and when we called him, his response was that his toe feels better and he cannot afford another seventy dollar co-payment as mandated by his insurance policy. I documented the conversation and told him that I would not charge him for any follow-up visits.
This patient was upfront with me as to his reasons for canceling. I suspect that all of us will encounter other patients with similar serious conditions, who will not follow up because of huge co-payments. We should all be aware of this and set up a system for flagging patients who miss follow-up visits and then inquire about and address their financial issues, should they be the reasons for the cancellations.

I echo Dr. Young. I have been doing elective cosmetic surgery on a very well informed patient for years. I'm sure each DPM in practice has done a bunion or any extra hammertoe procedure on a questionable painful toe. The injection for dermal fillers PSTTA (pedal soft tissue temporary augmentation) is something I have done for almost 9 years with outstanding results so far (search PM News archives). Most patients who are getting laser nail procedures for OM are having cosmetic procedures.
I believe that each of us should be able to use our skills and talents to provide care to patients that is appropriate and well informed and follows good medical advice.

With all due respect to Doctors Peacock and Young, here’s my issue. Let's assume the doctor is contracted with an insurance company. A patient presents with an HAV and HD5. Both of these deformities are covered under her plan but she (he) says, "I don’t like the way my foot looks, but I don’t have pain.” Does it now mean that this is a cosmetic situation with lack of pain, and therefore the doctor can bill the patient directly any fee he or she wants? Or, is the doctor contractually obligated to accept the insurance payment contingent with the plan's rates?
