CA Podiatrist Discusses Plantar Fasciitis
Plantar fasciitis results when that connective tissue is somehow injured or inflamed—a common occurrence among those who engage in “repetitive impact activities” like running, says Dr. Joan Williams, a foot and ankle surgeon at the University of California Los Angeles Medical Center. “People will start to notice some soreness in the heel after a run, but they tough it out and run through it,” she explains. That’s bad. “Inflammation and swelling and irritation of the plantar fascia cause the pain,” she says, and all of them tend to get worse if a person keeps training.

First and foremost, athletes who are experiencing heel pain need to rest. “I tell patients to stop running for 4 to 6 weeks,” Williams says. “It can be hard to get runners to stop, but continuing to run can make the issue worse.” Along with rest, Williams recommends stretches that target the calf, Achilles, and plantar fascia—like pulling the toes back toward the shin. Especially first thing in the morning or after long periods of sitting or lying down—times when foot muscles and connective tissues tighten up—stretching can help limit plantar fasciitis pain, she says.

"I guess what she's doing is trying to avoid any kind of skin breakdown or blisters," Dr. Suzanne Levine, a podiatrist at the Institute Beauté in New York City, told TODAY Style. Another possibility is that one of Meghan's feet is slightly larger than the other. "She may also have a size discrimination between both feet, that they're both not the same size, so she's going up to the larger one," Levine said.

Growing Bad-Debt Problem Illustrates Broken Billing System
Patient payment represents a growing share of health systems' revenue, putting consumers and providers in a precarious position, a new report finds. Patient balances after insurance rose from 8% of the total bill responsibility in the first quarter of 2012 to 12.2% in the first quarter of 2017, according to new analysis from TransUnion Healthcare. Commercially insured patients saw their balance after insurance increase 67% from $467 to $781. For Medicare beneficiaries, it increased from $144 to $314. Medicare bad debt, which is the result of beneficiaries not paying their deductibles and co-insurance, increased 17% to $3.69 billion in 2016, up from $3.14 billion in 2012, according to CMS data.
The TransUnion report recommended engaging patients about payment earlier, even before care, to gauge their coverage level and financial situation. Providers can work with consumers to set up payment plans or see if they qualify for their charity care program. Before writing it off as bad debt, see if patients qualify for commercial or public-payer coverage, the report said. Also, ensure prior authorization is in place before treatment.

RE: The Passing of Sheila A. Gildea, DPM
Sheila A. Gildea, DPM, of McLean, VA passed away peacefully on her 88th birthday, June 26, 2018. Sheila was an athlete and scholar throughout her life. She proudly attended the University of Rhode Island, where she was a varsity athlete in three sports: basketball, softball, and field hockey. She then enrolled at Temple University, earning a doctorate in podiatric medicine at the age of 21.

After graduation, she returned to Rhode Island, where she led a thriving surgical practice until she met and married Joseph A. Gildea, a young U.S. Naval Academy graduate. After Joe’s career path led them back to the United States, she returned to the practice of podiatry and then became a nationally recognized, award-winning Senior Olympian, earning international accolades for her track and field prowess.

A simple phone call to the requesting party is usually all that is required to get things started. This is totally a business matter and it should not become anything more. Express your willingness to comply and provide your expectations; that is, you expect upfront payment and your per chart fee. Let them know you are not interested in their paper, ink, or any other supplies, nor having their employees viewing records on your EHR (even with direct supervision of one of your employees).
If need be, speak with a supervisor, and if you have a previous good track record with the carrier, they will either comply or not comply with your request. The third-party requestor often will contact your local rep to do some further inquiries. As with any contact with any carrier, obtain the name of the person you spoke with, and their employee ID and/or a log number for the call.
