KY Podiatrist Discusses What Parents Should Observe in Young Walkers
“The most important thing for parents to realize,” says Kevin Bryant, DPM, of Progressive Podiatry, LLC, “is that they need to understand what is normal in order to be able to recognize what is abnormal.” Whether Baby starts walking at 9 or 14 months, parents will want to keep an eye on his gait as well as his feet. “Toe walking is very, very common in kids,” says Dr. Bryant, adding that kids who walk up on their toes typically stop around age 4 or 5. Some kids will walk on their tip-toes because of a neurological or physiological condition — for example, the muscles on their backs of their legs may actually contract the tendons in their ankles, forcing them to walk on their toes. Bryant says that most kids just think that’s the way to walk, and a little retraining with physical therapy resolves the issue.

Flat feet is another common problem, according to Bryant, and usually requires a visit to a specialist if your little one is in pain, or can’t seem to keep up with other kids while walking or running around. An orthotic insert, which today can be customized from advanced, lightweight materials, can help.

CPMA Announces New Board of Directors for 2014–2015
The California Podiatric Medical Association (CPMA) introduced its new 2014-2015 Board of Directors for the coming year following elections held on June 18, 2014 during its Annual House of Delegates at the Disneyland Hotel in Anaheim, CA. Thomas J. Elardo, DPM assumed the office of CPMA President, following Carolyn E. McAloon, DPM. Dr. Elardo practices podiatric medicine in Los Gatos, CA and will serve as the chief representative of CPMA’s more than 1,000 podiatric physician members for the next year.

Howard, DPM was elected to a two-year term as Director-at-Large, and Vladimir Zeetser, DPM was also elected to a two-year term as Director-at-Large.

CT Podiatrist Completes 15th English Channel Swim
Dr. Marcy MacDonald, a podiatrist from Andover, completed her 15th English Channel crossing Monday morning, from Dover to Cap Gris Nez, France, to become the American record holder for Channel crossings. MacDonald, who tied the record with a double-crossing last year, finished the approximately 21-mile crossing in 11 hours, 50 minutes. She started a little past midnight Monday morning and finished at 12:22 p.m. British time.

MacDonald, 50, swam on the 20th anniversary of her first crossing, June 30, 1994. Captain Mike Oram was her guide boat pilot Monday, as he was 20 years ago. "She keeps telling me she's never going to do it again," Oram said from England. "But she keeps coming back."

NY Podiatrist Uses FluoroScan Imaging System
You might call it old technology wrapped in a new package. It’s also — pardon the expression — a healthy step forward for Batavia podiatrist Dr. Joseph Canzoneri. Canzoneri recently started to use a sophisticated imaging device called a FlouroScan at his State Street practice, Advanced Podiatry Associates. The device takes its name from flouroscopy, an imaging technique that uses X-rays to obtain “real-time” moving images of the internal structures of a patient, like the complicated bones and joints of a foot.

The FluoroScan is described by its manufacturer, Illinois-based FluoroScan Imaging Systems Inc., as a “mini C-arm” imaging device with a flat-panel detector. In practice — and described very simply — the FluoroScan’s imaging arm is positioned around a patient’s foot; the X-rays are collected on a flat detector panel, and converted to digital images displayed on a flat-panel monitor mounted on top of the device.

Health Data Privacy Helped Fuel Supreme Court Cell Phone Ruling
The possible presence of healthcare data on cell phones helped put the devices in a legal class worthy of heightened constitutional protections from warrantless searches by police, the recent U.S. Supreme Court ruling. That unanimous decision, while narrowly focused on balancing the privacy rights of individuals and the “legitimate government interests” needed to enforce the law at the time of a person's arrest, could have broader healthcare implications, healthcare privacy specialists contend.
The decision could be used as a reference point, highlighting the special nature of healthcare information, as the healthcare industry wrestles to find a similar balance between patients' consent rights over disclosure of their medical records and the interests of healthcare providers, researchers, and other commercial entities in getting less-fettered access to those records.

Marcaine Lot Recalled
A recall of one lot of Marcaine (bupivacaine) has been initiated by Hospira, after a customer reported seeing particulate embedded in a glass vial and in the solution. The affected lot (0.5% Marcaine [Bupivacaine HCl Injection, USP], 30 mL, Single-dose Vial — Preservative Free [NDC 0409-1560-29], Lot 33-545-DD) was distributed between November 2013 and March 2014 to wholesalers/distributors, hospitals, and clinics nationwide.
If the particulate is administered to a patient, it could block administration and cause a delay in therapy. However, the identified particulate is so small, it's more likely to pass through the catheter and cause inflammation, mechanical disruption of tissue, or immune response, says the company.

What are the legal issues when I give medical advice to acquaintances. There has been no doctor-patient relationship established. Also, I am retired and no longer carry malpractice insurance?

Editor's Comment: PM News does not provide legal advice. While you may assume that no doctor-patient relations is established, a court of law could differ. The issue will hinge on whether the acquaintance "reasonably believed" that a doctor-patient relation had been established and whether he or she relied on this advice. Therefore, it's safest to say, "I'm now retired. I recommend that you get a complete examination by a podiatrist."

I don't understand the issues associated with cosmetic surgery. If the patient wants "cosmetic surgery" and is willing to pay the surgeon his fees (since this is not covered by insurance), and the patient is read the "riot act" about risks, complications, and follow-up care, I personally see no problem. So long as the patient is of age to make these decisions and has good mental status, and the practitioner provided a fair assessment of the risks and complications without overly aggrandizing the procedure, then I see no reason why cosmetic surgery should not be performed.
Plastic surgeons are doing this all the time. Why is it okay to have a facelift performed many times on the same person, yet it is unacceptable to perform a surgical procedure on a non-painful joint to achieve the desired goal for the patient, provided the patient is correctly and fairly informed? We should've asked Phyllis Diller or Joan Rivers.

There seems to be a lot of negativity toward cosmetic procedures in our field. This is something that I do not understand. Plastic surgeons have long provided worthwhile surgeries for patients seeking aesthetic outcomes. We should do the same. It is my opinion that the future of podiatry will involve more cosmetic endeavors as insurance reimbursement changes. In this regard, minimally invasive surgical correction of foot deformities will gain in popularity. If the orthopedic community decides not to jump on this bandwagon, they will miss out.
My personal interest in MIS began with my desire to provide for a better cosmetic outcome when surgically treating foot deformities. In my opinion, our profession should begin to gravitate towards cosmetic endeavors. Many of us are already doing this with lasers for fungal nails, better cosmetic surgical procedures, etc.
Regardless of whatever name is given to these undertakings, be it the Cinderella procedure or any other name, it doesn’t really matter. There will be a significant subset of the population seeking aesthetic outcomes for foot deformities. I say more power to Dr. Suzanne Levine with her aesthetic surgery interest.
