TX/NM Podiatrist Completes Mayo Interventional Ultrasound Course

Last week, Dr. Angela Drury attended the 2013 American Institute for Ultrasound in Medicine (AIUM) - Mayo Clinic MSK U.S. Course in Diagnostic and Interventional Musculoskeletal Ultrasound. The teaching format consisted of lectures, live demonstrations, and extensive hands-on experiences on live models and unembalmed cadavers.

IL Podiatrist Joins Orthopedic Healthcare Team

Fox Valley Orthopedics has announced the addition of a podiatrist, Dr. Joanna M. Chura, to its orthopedic healthcare team. Dr. Chura received her Doctor of Podiatric Medicine (DPM) after completing a 36-month residency at the Hennepin County Medical Center in Minneapolis, Minnesota. Prior to this, she attended the Dr. William M. Scholl College of Podiatric Medicine.

Query: Freiberg's Infraction in 47 Year Old Male

My patient is a 47 year old male who complains of pain in his 1st and 2nd MPJ. He is disabled due to a seizure disorder following partially successful neurosurgery for brain tumor (no tumor recurrence), but he has seizures and memory issues, but could otherwise be considered healthy. He thinks his foot problem started a few years ago, but doesn't recall anything specific. He walks for exercise. Attempting to move the 2nd MPJ through any ROM is uncomfortable. His 1st MPJ has limited ROM as well, although it looks good on x-ray. He has a lot of swelling, more so than I would expect.

1. Would you image this (MRI) to rule out bone neoplasm, or be satisfied with biopsy during surgery? 2. I have had reasonably good results debriding bone from the 2nd metatarsal head, and in some cases removing the base of the phalanx, or resecting the metatarsal head for the worst cases. Would you offer any other surgical treatment for this?

Query: E/M with Nail Debridement Codes

I need help with trying to figure out why Medicare won't allow me to bill an office visit, CPT 99213, with either of the debridement of nail services codes CPT 11720 or CPT 11721.

Response: If you do a procedure plus an E/M code, you must add a "-25" modifier to justify that the E/M service is of significance in charging for an evaluation and management in addition to the procedure. If the E/M documentation says the patient has problem nails that need debridement, this is grossly insufficient.

RE: First Coast Disallows CPT 76942 for Podiatrists

Here is the link and some of the points that have been issued by First Coast Medicare regarding CPT 76942. Those who scrupulously used this code for a small portion of your patients to guide injections will no longer be able to use the code in pretty much all cases. You can thank our colleagues who use it on every injection in the office. It seems to just be the story of podiatry! And, of course, of all specialties, podiatry is one of the only ones specifically mentioned, "Needle procedures of the foot by podiatry or other specialties. (Most of these interventions are standard office based needle procedures and not special procedures performed on a radiology suite)."

A few other points from the link: "Though MAC J9, based on the low quality of evidence in the peer reviewed literature, could deny many of these billing situations as services not meeting the reasonable and necessary threshold for coverage, there is concern with beneficiary liability. CPT® 76942 is valued in the 220 dollar range, whereas the majority of the office-based procedures outlined above suggest value in a range from 0 to 40 dollars. (This is based on reference value mapping of CPT® codes such as the value assign for CPT® code 76937 Ultrasound guidance for vascular access). In summary, it is the expectation that physicians utilizing ultrasound guidance for standard office based needle procedures will not code separately, or alternatively, bill the unlisted code CPT 76999."

This lay article from the NY Times describes a real medical study (British Journal of Sports Medicine) that was undertaken to determine the roll of biomechanics in choosing running shoes, and its effects on injury rates. 

It raises the question as to what a similar quality study would show regarding foot orthotics.  I suspect I know the answer.
