PODIATRISTS IN THE NEWS
Don't Ignore Foot Health: NM Podiatrist

According to a survey by the American Podiatric Medical Association (APMA), 77% of Americans have experienced problems with their feet — and half of them say it has been disabling enough to affect quality of life. "It's not surprising to see how many people are affected by foot pain, when survey results show that we view our feet as the least important body part in terms of our overall health and well-being," says podiatrist Frank Spinosa, DPM, former president of the APMA. "Our feet are literally and figuratively the furthest things from our minds."

Dr. Frank Spinosa

"For those who have diabetes or are at risk for the disease, regular checkups by a podiatrist are one of the easiest ways to prevent most foot complications," said  Dr. Spinosa. "Including a podiatrist in your care can reduce amputation rates by as much as 85 percent."

PODIATRISTS AND SPORTS MEDICINE
TX Podiatrist Discusses Recovery and Return to Play After a Torn Achilles

According to Jeffrey Ross, DPM, MD, "The NBA playoffs saw three superstars tear their Achilles tendons during play: Damian Lillard of the Milwaukee Bucks (Round 1, Game 4 v. Pacers), Jayson Tatum of the Boston Celtics (Round 2, Game 4 v. Knicks), and Tyrese Haliburton of the Indiana Pacers (NBA Finals, Game 7 v. Thunder). A torn Achilles tendon can be a devastating injury; however, with immediate attention and appropriate surgical repair, the outcomes can be much better and the recovery time hastened."

Dr. Jeffrey Ross

"For a complete rupture in a high-performance athlete, surgical repair is essential. However, for older or sedentary individuals, a partial or even complete tear may not require surgical intervention. Surgery may involve re-opposing the torn fibers, or, in some cases, grafting tendon to provide integrity to a defect in the repair. Post-operatively, a period of about three to four weeks of immobilization and non-weight bearing to allow the tendon fibers to heal properly is important. This is followed by a walking boot and early mobilization to prevent excessive scar tissue formation," says Dr. Ross.

PODIATRISTS AND FOOTWEAR
NY Podiatrist Provides Tips on Finding Shoes with Support

Selecting shoes that offer the right amount of support for your arch type — low, neutral, or high — is crucial for the health of your feet. According to Samantha Landau, DPM, arch-supportive shoes should have firm heel counters and good side-to-side control, all of which help limit excess motion. She also recommends opting for shoes designed with forefoot rockers, as most people can benefit from them. “A forefoot rocker is beneficial as this allows and helps facilitate forward motion,” Dr. Landau said.

Dr. Samantha Landau

“If the shoe bends at the forefoot, then less stress will be placed on the foot to bend. This is especially beneficial in those with forefoot arthritis or forefoot injuries or pain.” According to Landau, the majority of people don’t have perfect feet and shoes alone are typically not enough to provide all the necessary support. For that reason, she finds pre-fabricated (off-the-shelf) or custom foot orthotic devices are often needed to “further neutralize the deforming forces, reducing the potential to cause injuries during gait.”

INTERNATIONAL PODIATRISTS IN THE NEWS
Yearly Diabetic Foot Exam Can Save Your Leg: Guam Podiatrist

On Saturday, the Guam Regional Medical City (GRMC) hosted a diabetic foot health and wound care outreach event. Melinda Sangalang, DPM, GRMC podiatrist, was one of the presenters at the event who provided valuable insight on foot care for people with diabetes. She spoke about the early signs of complications, how to conduct a self-examination, and what a person with diabetes can do if they find something wrong.

Dr. Melinda Sangalang

She emphasized that when it comes to diabetes, “not knowing” could result in serious consequences for your health. “Fifteen minutes once a year could save your leg. Fifteen minutes. Now, it might take a little bit longer if we have to get some tests done or other procedures done, but being proactive can really decrease the chances of you losing anything," said Dr. Sangalang.

PRACTICE MANAGEMENT TIP OF THE DAY
Tips to Keep Patients Engaged Between Visits

It's important to note that engagement efforts don't have to bust the budget. Most tactics rely on tools you already own — your electronic health record's (EHR's) messaging templates, inexpensive survey apps, a smartphone camera, and five minutes of staff time.

8Create micro-learning videos.

Produce brief (1-2 minute) instructional videos using smartphones and inexpensive lighting and upload them to the portal. The videos could be brief clinical how-tos (e.g., how to change a wound dressing or use an inhaler), introductory videos or even office tours.

Video content can help to answer FAQs (thus cutting support calls), alleviate new-patient anxiety, and help keep existing patients engaged.

RESPONSES/COMMENTS (MEDICARE FRAUD ALERTS)
RE: Alert: Medicare Fraud Scheme Involving Phishing Fax Requests
From: Paul Kesselman, DPM

I received this notice today:

CMS has identified a fraud scheme targeting Medicare providers and suppliers. Scammers are impersonating CMS and sending phishing fax requests for medical records and documentation, falsely claiming to be part of a Medicare audit. Important: CMS doesn’t initiate audits by requesting medical records via fax. Protect your information. If you receive a suspicious request, don’t respond. If you think you got a fraudulent or questionable request, work with your Medical Review Contractor to confirm if it’s real.

DO NOT RESPOND TO ANY FAXES OR EMAILS WHICH REQUEST THIS INFO. Medicare typically does not request this sort of information by fax or email. The entities sending these faxes/emails are attempting to steal patient PHI and then use it to bill for services and DME which were not provided. Don't be part of perpetuating fraud and protect yourself, your patients and the Medicare Trust Fund.

RESPONSES/COMMENTS (STUDENT RECRUITMENT)
RE: Podiatry Camp, Not Your Band Camp
From: Rod Tomczak, DPM, MD, EdD

When I mentioned podiatry camp in my last posting, I was serious about the concept. I’ve had a few phone calls from people in the profession who don’t want to write to PM News but would like to offer comments. So let’s devote a posting to the idea of podiatry camp, the why, the what I think it should be, and what it shouldn’t be.

First of all, I want to extend my kudos to practitioners who want to see the profession return to full classrooms if we are going to keep podiatry schools open and grant DPM degrees and retain the soul of our profession. But, if I’m a 21 year old kid, I don’t want to listen to some semi-polished well intentioned lunch lecturer who starts the presentation with, “You know, I have a couple grandkids your age.” I don’t know if as a visitor going to an office of a podiatrist who has socks older than I am is the best way to be impressed about...

Editor's note:  Dr. Tomczak's extended-length letter can be read here.
