A Northwest Indiana podiatrist who has extended his expertise to cracking crimes will talk about his foot work alongside detectives in a crime dcumentary show. The Headline News, also known as HLN, series Lies, Crimes and Video aired across the nation on Saturday. The upcoming show featured Michael Nirenberg, DPM, a foot specialist at Friendly Foot Care, based in Crown Point.

While he normally is using his know-how to solve patients' foot and ankle problems, this isn't the first time the doctor has helped police get a step ahead in crime investigations. “I have worked on a case where there was a bloody footprint, and I was able to match it with a person of interest,” Dr. Nirenberg said. “I testified in court and the suspect was convicted.”

Positano, DPM, MSc, MPH, Professor and Director of the Non-Surgical Foot and Ankle Service at the Hospital for Special Surgery has been notified  that the eligible voting faculty in the Yale School of Public Health (YSPH) and the Yale School of Medicine Board of Permanent Officers approved his appointment as Associate Clinical Professor of Epidemiology in the Department of Chronic Disease Epidemiology within the Yale School of Public Health.

Darryl Cosby traded anecdotes with Dr. Jim Flynn as the podiatrist examined his feet in a small office in downtown Oklahoma City. Nearby, two men soaked their feet in warm, soothing water as they waited to see Flynn. They were being treated to a free foot massage and foot exam at a clinic offered twice a month at Sister B.J.'s Pantry, a food and clothing outreach

A lot of the problems Flynn treats are caused by people wearing shoes that may be too small or simply inadequate to bear up to so much walking. Blisters, corns, calluses, in-grown toenails, and foot ulcers are some of the more common maladies the cheerful volunteer sees during his foot exams. Occasionally someone with a more serious foot condition, like Cosby, comes in for treatment and Flynn said he tries to help them too.

Why and How to Stay on Top of Your Online Reputation - Part 2

2. You might miss something you need to see. Unless you’re keeping an eye on what’s being said about your practice, you may not notice that a patient has posted a damaging review. But, if you don’t see it, you can’t respond to it (Remember to follow proper HIPAA guidance when responding. Sometimes, patients make mistakes and post their reviews alongside the wrong physician name. In those cases, try working through the site to have it corrected first.). More commonly, a negative review might call out an administrative or workflow problem you were unaware of. This can be priceless feedback that allows you to take action before other patients are inconvenienced.

Query: X-rays Prior to Admission
 
We had a patient with a stage 4 ulcer who had developed osteomyelitis. We took x-rays of his foot during his appointment in the office and he was admitted to a nearby hospital later that day. UHC Medicare is requesting payment back for the x-rays we had taken stating they are part of the facility stay.
 
Is there a modifier to attach to the x-rays? This has never been an issue in the past.

Response: The x-rays you took should have your POS as #11 which has nothing to do with the inpatient hospital POS which is 21. A diagnostic test performed in your location technically has nothing to do with your hospitalization unless you had the hospital take an x-ray on the same day. If so, then the hospital billing staff should be informed that this was a repeat x-ray performed as you took one that day before admission.
 
They would need to put a modifier -76 on their x-ray because you took yours first. You will have to explain this to the hospital billers to send a corrected claim from them; otherwise the payer thinks they are getting the same service twice when the two may each be medically necessary. You would think the payer would know the POS is not the same.

I have restless legs, or so my wife tells me; I am totally unaware of it. An overnight sleep study is the best way to diagnose this, and a patient's primary care provider can prescribe it. I had that done years ago to check for sleep apnea. My study did not reveal apnea but did show a great many leg movements per hour. These are important because even though the patient is not necessarily aware of the movements, they interfere with the sleep cycle and don't allow for quality sleep. Actually, my wife tells me that often my whole body jerks or moves. Interestingly, if I nap during the afternoon, that sleep appears restful with little or no body movement or snoring. 
 
Quite a few years back, one of my local colleagues performed a bunionectomy on me and he allowed my wife, who was in nurse practitioner school at the time, to observe. She tells me that during the Mayo block under light sedation, I moved around so much that the anesthesia provider had to sedate me more deeply, although not to where I needed LMA general.

Is there a separate podiatry degree now for podiatrists who are surgeons? I understand that there are some podiatrists who complete extensive residences, but does that obviate their duty to perform general podiatry? I ask because a friend of mine who has a well-known HMO insurer asked me to take care of a new ingrown toenail and a couple of “corns”. He had gone to his HMO podiatrist who told him that he could be scheduled for surgery for his ingrown toenail and that my friend should find someone who could “scrape off the corns” (a seed corn and heloma) because the podiatrist said “I am a surgeon”. 
 
The ingrown toenail was new and a result of the new nail growing in after he had lost the old one. The distal medial aspect of the nail was elongated and relief was accomplished with a simple debridement of the nail. I was under the impression that our profession includes the care and treatment of all issues of the feet within our scope of training...or is there something that follows DPM in our degree?

Doing a survey among podiatric residents and maybe even all podiatrists could reveal important and maybe even life-saving information. That being said, I suspect that the data will reveal that there is a much lower percentage of suicidal ideation among podiatric residents than among MD and DO residents. Why? Although podiatric residents work as hard as any other residents, we do not deal regularly with patients that might be dying of a disease.
 
When I was a student, I attended an inquest at a hospital rotation. A child died. Present at the autopsy were the pathologist, the pediatric resident, and I as an onlooker. When the pathologist was finished raking the pediatric resident over the coals using 20/20 hindsight, I was afraid that that resident would jump off the roof of the hospital.
 
With the exception of missing severe vascular disease or a melanoma, we don't deal regularly with life-threatening diseases which could cause us mental anguish. Nevertheless, the survey that Dr. Deheer is working on could tell a totally different story.

If there are podiatry residents out there thinking of committing suicide, then they probably should NOT have been accepted to podiatry school and pushed through, by school administration, to graduate in the first place. There is nothing stressful about podiatry residency training when you compare it to being an internal medicine resident, general surgery resident, orthopedic resident, etc. 
 
The stakes are higher in those training programs. They just are. Intern residents from these specialty tracks are dealing with patients with more diverse pathology. Intern residents in these specialty tracks are actually medically managing these patients, dealing with complications during inpatient care. Things that most podiatry residents don't have to deal with...
