ACFAS Podiatrists Call Cosmetic Foot Surgery "Ill-Advised Trend" 
 
Today a host of women are turning to cosmetic surgery to give their feet a “face lift” and fit them into high-fashion shoes. But physician members of the American College of Foot and Ankle Surgeons, who specialize in foot surgery, are taking a stand and calling this an ill-advised trend. “Undergoing foot surgery solely for cosmetic reasons raises serious concerns,” says Michael Cornelison, DPM, FACFAS, a Cupertino, CA foot and ankle surgeon and Fellow member of the American College of Foot and Ankle Surgeons. “We need our feet for walking and getting around, so breaking these bones to reconstruct them for appearance’s sake is a troubling matter—it often affects function and can bring risks and complications.”

“Those who are compelled by these terms to pursue cosmetic surgery for their feet should be sure to ask for a description of all the possible risks and negative consequences, along with what the procedure is expected to do,” urges Thomas S. Roukis, DPM, PhD, FACFAS, President of the American College of Foot and Ankle Surgeons and a foot and ankle surgeon from La Crosse, WI. “To get a full understanding of the surgery, it’s important to seek the opinion of an experienced foot and ankle surgeon who has done these procedures for medical rather than cosmetic reasons.”

ME Podiatrist Prepares for Rowing Race That Nearly Killed Him
 
Dave Grody and Dr. Dan Benson had scarcely finished the Blackburn Challenge last July when they made a vow to return. Considering the circumstances that surrounded their original decision to enter the 20-mile rowing race around Cape Ann in Gloucester, MA, this was no small promise. The two nearly lost their lives in a training run off Popham near the mouth of the Kennebec River when they became lost in a dense fog and drifted three miles to Seguin Island where they wrecked their boat.

“We were woefully under-prepared last year,” said Benson, an Augusta podiatrist. “We decided to do it with much more preparation so we could compete rather than survive.” Benson, 51, put together a cross-training program for the July 19 race that included weight-lifting, aerobics, and ergonomics. The pair also each have rowing machines in their basements, and after each workout email one another with what they’d done “so we have accountability,” Benson said.

Marathon Will be Worth the Pain for Aussie Podiatrist
 
There was a time when Chris Angel could barely crawl to the shower each morning. The 36-year-old Hobart podiatrist was diagnosed with psoriatic arthritis in his mid-20s. Although simple movement was unbearable, he held on to the belief that one day he would run a marathon. Despite the irony of being a podiatrist who had difficulty walking, Angel took up road cycling as a way to stay healthy and manage his condition. “I started cycling first and then moved on to running,” Angel said. When a recent scan showed further damage to his hips, he decided to cross the item off his bucket list and will compete in the Gold Coast Airport Marathon on Saturday.

Angel wants to raise awareness of his condition and what sufferers go through, and will raise funds for Arthritis and Osteoporosis Tasmania by running the marathon. "It has been fantastic to be able to still be active despite the arthritis,” Angel said. “One of the messages I’m trying to raise awareness about is that arthritis can affect anyone — even 10-year-olds. It doesn’t just happen to older people.”

There are many "review" sites online, rating physicians of all kinds. It seems to be human nature that people want to share their problems (negative outcomes) more so than when things turn out as they expected. Whenever there is a negative review of a provider, what recourse does a provider have?  When does this cross the line to libel or defamation?

Query: Weekend Emergency Office Visit
 
An existing patient, who has Medicare, called me Saturday morning on my emergency line, and needed to be treated on Saturday in my office. Is there a special E/M service code for billing Medicare for after hours or weekend emergency encounters?

Response: The add-on code you're referring to is CPT 99050. That code is defined as "services provided in the office at times other than regularly scheduled office hours or days when the office is normally closed (e.g., holidays, Saturdays or Sundays), in addition to the basic service." However, Medicare has given this code a status code "B" which means that it is bundled into whatever the code(s) are for the basic service(s) you provided. Because it is status code "B", it's not separately reportable to Medicare and it's not separately billable to the patient, even with an ABN. 
 
The bottom line is that you can only bill for whatever treatment you provided. The fact that you had to open your office on a day it's normally closed in order to provide this service is already considered part of the overhead (i.e., is included in a proportional sense in the practice expense RVU for the services you provided). Sorry I don't have better news!

Thank you to those who responded to my original post. As a person who develops a full body reaction to poison ivy, simply by looking at it, I am familiar with wide spread dermatitis resultant from small scale exposure. This is not what my patient complained of. Apparently, my original post was unclear. The patient used econozole cream on her feet. She did not develop a local reaction on her feet. She described a rash on her face (which was not visible upon exam), itching on most of her body (with no visible rash reported), dizziness, and shortness of breath. Short of a patient eating econozole cream, I am wondering if anyone has experienced this type of reaction.

Dr. Suzanne Levine, owner of Institute Beauté and co-chair of the International Aesthetic Foot Society, stated on Good Morning America last week that she performs “Cinderella” foot surgery primarily for “pain.” Well, Imagine that. The principle architect of two foot aesthetic organizations appeared on TV to hock her wares, but opened up the opportunity for the Darth Vaders of foot surgery, namely the orthopedic foot surgeons, to take another shot across the bow of podiatry by marketing this silly term "Cinderella foot surgery!" Had Dr. Suzanne just spoke about reconstructive foot surgery which would allow the wearer to possible fit into stylish Monolo Blahniks or Christian Louboutins, that would have been fine. However, why must we have to recreate classic bunion and hammertoe surgeries into a catch-phase monicker to entice patients?
