Unmatched Residency Placements Currently Stand at 86
The American Association of Colleges of Podiatric Medicine (AACPM) provides periodic status reports on residency placements from Match Day. The following is residency placement data as of June 28, 2013:
Class of 2013 Placements: Placed in Residencies 511 (89.3%)
Not Eligible/Not Seeking Training 10 (1.8%)
To Be Placed 51 (9.7%)
Total 572 (100.0%)
Class of 2012 and Earlier Placements:
Placed in Residencies 24 (40.7%)
To Be Placed 35 (59.3%)
Total 59 (100.0%)
When taking the overall placement stats into consideration, 85% (535) of the 631 residency applicants have found residency positions thus far this year. With nearly 2% (10) of residency applicants not seeking or ineligible for placement, it leaves just over 13.6% (86) total applicants that have yet to find a residency position for the 2013-14 training year.

CA Podiatrist to Run Again for Congress in 2014
Lee C. Rogers, DPM has filed with the Federal Election Commission (FEC) and has announced his intention to Run for Congress in the 2014 election cycle. Dr. Rogers, a Democrat, will run in the district that contains Santa Clarita and The Antelope Valley. Rogers has a history of APMA advocacy and was the recipient of the APMA 2011 Rising Star Award.

MA Podiatrist Exhibits X-Ray Art
Inside a small podiatrist office in Westminster, Massachusetts, podiatric surgeon Dr. Paula Fontaine and her business partner Joseph Moisan are getting creative from a different perspective. They’re making prints, jewelry, home goods, and accessories from x-ray images, using digital map painting which allows coloring over the black and white.

Collaborative Care Diagnostics Buys PADnet & VEINnet Technology from BioMedix
Collaborative Care Diagnostics LLC (CCD), a privately held Minnesota organization dedicated to the advancement of disease management technology, announced it has entered into an agreement to purchase the Disease Management operations, including PADnet and VEINnet medical devices and related services, from BioMedix Vascular Solutions, Inc.
The founders of CCD are comprised of leaders from healthcare (including healthcare pioneer Dr. Tom Fogarty), finance, industry and service businesses, all of whom share the belief that the PADnet and VEINnet devices and the related network and services are well aligned with expanding efforts to reduce healthcare system costs through early detection and treatment. CCD will operate from its facility in Maplewood, MN. The financial terms of the transaction have not been disclosed.

The lack of adequate positions is a major tragedy. That being said, graduates must continually strive to advance their careers and not give up hope!

I must respectfully disagree concerning Joel Lang’s observation that “it is very rare to find a neurologically intact adult with significant rotational deformities of the lower limbs”.
My experience after nearly 30 years of specializing in pediatric and orthopedic podiatric medicine has been exactly the opposite. I routinely find, on routine lower extremity musculoskeletal exam, residual tibial and femoral torsion (as well as residual metatarsus adductus) in adolescent and adult patients with recent onset of podiatric pathology and symptoms. It has been my experience that many of these patients with residual transverse plane deformities often do not exhibit a significant in-toe gait angle (although a subtle one is often there) because the abductory component of their midtarsal joint compensatory pes plano valgus deformity is masking the underlying adductory deformity of the superstructure.
Functional biomechanics is a puzzle and the skilled practitioner must fit many pieces correctly together to get to the complete diagnosis and to direct an appropriate treatment plan. Gait is the final expression of all the structural and functional issues in a given patient and, by definition, exhibits both the primary structural and secondary compensatory components of any given pathology complex. Back to that puzzle….

RE: Effect of DME Competitive Bidding on Podiatrists (Olga Luepschens, DPM)

At the present time, competitive bidding does not affect most items dispensed by podiatrists, including, but not limited to: shoes/inserts or any items covered under the Therapeutic Shoe Program for Patients with Diabetes, any O&P product normally dispensed by podiatrists (e.g. night braces, CAM boots, AFOs), crutches, canes, or surgical dressings. Walkers are subject to competitive bidding, however, when dispensed by physicians (MD/DO/DPM), they are exempt. The HCPCS codes (too lengthy to list here) must be amended by the "KV" modifier.
Other items typically not furnished but prescribed by podiatrists, which in Round 2 are subject to competitive bidding include but are not limited to: walkers (again exempt if the DPM furnishes them), wheelchairs, NPWT, mattresses, and oxygen. Diabetic testing supplies must be furnished by a contracted mail order supplier. This has just about eliminated all of the local mom and pop pharmacies and surgical suppliers.
The effect here is that for patients who live in a competitive bid region, all new orders must be sent to a contracted supplier (one who was awarded a competitive bid). CMS has an excellent resource on the competitive bidding process for physicians to reference.

In a nutshell, the DMEPOS Competitive Bidding Program will have a dramatic effect on the DME/HME industry but, as of today, I see little to no effect on podiatry. July 1, 2013 starts Round 2 of this program. Round 1 consisted of 9 CBAs and Round 2 consists of 91 CBAs. There are 9 product categories that are affected-CPAP Devices, Respiratory Assist Devices; Enteral Nutrients, Equipment and Supplies; Hospital Beds and Related Accessories; Mail-Order Diabetic Supplies; Negative Pressure Wound Therapy Pumps and Supplies; Oxygen Equipment and Supplies; Standard (Power & Manual) Wheelchairs, Scooters, and Related Accessories; Support Surfaces (Group 2 mattresses and overlays); Walkers and Related Accessories.
In Dr. Luepschens’ query, she mentions diabetic shoes, braces, and wound products. L codes are not included in this program (at this point-CMS reserves the right to add or subtract product categories). She mentions wound products; the only wound care that is currently in this program is NPWT. The only other DME that podiatrists might order are wheelchairs, walkers, and maybe scooters.
More information on the DMEPOS Competitive Bidding program can be found at Medicare.gov. Stay tuned as the DME industry and Congress are actively trying to stop or fix this fatally flawed program.

Schools are taking too many students for the number of residency programs. As a past residency director, I recall the same selfish, self-serving attitude in the '90s. There was a hand-shake agreement back then between the schools that they would not take over a certain number of students for the ratio of training programs available. Iowa was one of the few schools that I know that stuck to this.
Basically, the schools are stealing from the students, and like any thief they...
